Job summary
Symphony Healthcare Services are looking for an enthusiastic and compassionate Nature-Based Health and Wellbeing Coach to join West Somerset Primary Care Network, supporting patients across our four member GP practices and wider West Somerset communities.
This is a unique opportunity to combine person-centered health and wellbeing coaching with the natural environment to support health. You will provide personalised 1:1 coaching and facilitate group opportunities, supporting people to identify what matters to them, build confidence and self-management skills, and make positive and sustainable changes to their health and wellbeing.
This job role is a permanent contract, with the option to work between 30 and 37.5 hours per week.
The pay rate is from £13.27 per hour, DOE.
This vacancy will close at midnight on 1st November 2026.
Main duties of the job
This is an exciting opportunity to make a meaningful difference to people's lives, helping them take greater control of their health and wellbeing, build confidence and make positive, sustainable changes. You will be supported to undertake the Undertake the Level 3 Community Health and Wellbeing Worker Apprenticeship, including associated training, learning and assessments, to develop the knowledge and skills required for the role.
You will work with a diverse range of people and communities, including those who may experience barriers to good health, social isolation or reduced confidence. You will build trusting, compassionate and non-judgemental relationships, recognising the strengths and potential within each person and supporting them to become active participants in their own health and wellbeing.
As a member of the West Somerset Primary Care Network multidisciplinary team, you will work alongside GPs, Social Prescribing Link Workers, Care Coordinators, Health and Wellbeing Coaches and community organisations, while developing close working relationships with colleagues across Exmoor National Park Authority, Somerset Council and the voluntary, community and social enterprise sector.
About us
Established in 2016, Symphony Healthcare Services (Symphony) provides NHS primary care provision across 21 sites within Somerset, linked to 14 general practice contracts. The organisation cares for approximately 132,000 patients and employs around 600 staff.
Symphony is a subsidiary of Somerset NHS Foundation Trust and manages its services through the support of a central team (including HR, finance, corporate and operational governance).
Providing NHS services is at the heart of what we do. Specifically, our vision is to be at the forefront of sustainable, high quality general practice, collaborating with our communities to improve experiences and health outcomes. We work closely with our practice teams to grow, enhance, and transform our services for the benefit of patients and staff.

Symphony offers an NHS or NEST Pension, a fantastic range of employee benefits and salary sacrifice schemes (see attached poster), along with flexible working from day of employment and an agile approach to hybrid working (where applicable).
We look forward to hearing from you and receiving your application.

Job summary
An exciting opportunity has arisen for an experienced Band 8a Clinical/Counselling Psychologist to take on a combined post shared between our North Powys CAMHS Team and the Powys Therapeutic Children's Home. Would you like to join a passionate and supportive multidisciplinary CAMHS team, as well as taking on a key role in the development and delivery of a bespoke therapeutic accommodation provision for children and young people in the care system in Powys with complex emotional needs?
You should have a passion for working with children, young people and families, as well as significant post qualification experience of providing psychological assessment, highly specialist interventions, supervision and consultation. You will need to be able to work autonomously and as part of a team. Our Psychology and Psychological Therapies Team places a strong emphasis on staff wellbeing and supporting personal and professional development.
Powys is a unique and beautiful place to live and work, with a small population spread across a large geographical area. This presents us with specific challenges that require creativity, flexibility and innovative ideas, and provides opportunities for individual clinicians to have a real impact on services and the experiences of our service users and their families.
Applicants who do not currently meet all the essential criteria for the Band 8a role are encouraged to apply and, if successful, will be appointed under Agenda for Change Annex 21.
Main duties of the job
This post combines two roles, with 0.4WTE devoted to provision of a specialist psychology service to the Powys Therapeutic Children's Home and 0.3WTE for the provision of a specialist psychology service to the North Powys CAMHS Team.
Provide psychological expertise into the Powys Therapeutic Children's Home and North Powys CAMHS Team, including:
Highly specialised psychological assessment, formulation, evidence-based intervention to children young people and families.
Specialist psychological consultation and clinical supervision to colleagues to support them to develop their knowledge and skills in assessment, formulation, and evidence-based treatment of a range of mental health issues.
Analysis of training needs, design of training and skills development programmes, and delivery and evaluation of training
Acting as a core part of the decision-making team considering the suitability of placements within the Powys Therapeutic Children's Home for specific children and young people, and taking a key role in planning and supporting the transition of children and young people both pre- and post-placement.
Taking a leading role in developing an attachment-aware, trauma-informed clinical model within the Powys Therapeutic Children's Home, including supporting staff wellbeing initiatives
The base for the post will be in Newtown, with a combination of face to face and remote working.

Job description
Job responsibilities
Please see the attached Job Description for a full list of duties and responsibilities before applying for this position.
We have full and part time hours available and can support flexible working patterns.
What our current Lifetime Support Workers have to say about their role
I am in awe of the children I care for and the families I support. I hope that my small part in their sometimes short lives has made a difference to their quality of life. They have certainly made a difference to mine. I feel privileged to have been accepted into their homes, its not just a job, I have been doing this for 8 years and wish I had found it years ago. - Lizzie
"I feel honoured to be a part of our special children's lives. Knowing that we make a huge difference to the parents so they get some much needed rest, safe in the knowledge that their child is being cared for is an enormous privilege." - Ally
I am new to Lifetime, and I can say that I have been welcomed and really well supported by a very friendly team. Even though I had relatively little nursing experience the training has been so thorough and I have learnt so much. The children we work with are amazing, and at the end of each shift I feel that I have made a small yet essential contribution to their health and well-being, and provided some much needed rest to their families. - Megan
