Our ASYE programme is excellent and you will have a protected case load for the duration of the supported year of practice.
Main duties of the job
You will play a crucial role in improving mental health outcomes for our service users. You will bring a distinctive social and rights based perspective to the work that you do and promote this within the multi-disciplinary team and beyond. You will liaise with other agencies to ensure that the needs and rights of people with lived experience of mental health are at the front and centre of services and interventions.
You will use your advanced relationship skills to work with people to make positive and self-directed change and support them to work towards recovery as defined by them. You will provide advocacy support and understand the important role of carers, working in partnership to support them especially at time of crisis.
Providing statutory social care assessments under the Care Act and s.117 after-care Mental Health Act is a key feature of the role and you will provide a strengths based, rights focused assessment to identify and meet need, ensuring timely review of packages of support.
Working with complexity and risk is a key feature of the role and you will use your safeguarding adults knowledge to identify abuse and neglect and work with the person and other agencies to seek ways in which to protect and safeguard the person. At times, you will need to use legislative frameworks such as the Mental Capacity Act and Mental Health Act to support people who are unable to make decisions for themselves, balancing and protecting the rights of people and others.

We are proud to be part of Symphony Healthcare Services, Somerset's largest provider of GP services. This means you'll benefit from the close-knit atmosphere of a local practice while having access to the expertise, shared learning, innovation and professional development opportunities available across a network of 19 practices.

- Flexibility and work-life balance in a part-time role
- Access to shared expertise, innovation and career development through Symphony Healthcare Services

Main duties of the job
As a Symphony GP, you would undertake all the clinical duties associated with a GP working in primary care, without the responsibilities and risks associated with being a partner. We can offer opportunities depending on your experience and the level of responsibility you take on at the practice.
All GPs working for us are paid as employees, unlike Partners, which means less risk for you and more time to focus on patient care.
We put patients first, allowing GPs to work flexibly according to need, so contributing to areas such as QOF, QI and research projects providing a huge amount of variety to the role.
This would be an ideal role for a newly qualified GP, with full peer support provided.
About us
Symphony Healthcare Services Limited was established in 2016 as part of the award-winning Symphony Programme Vanguard, developing new models of care. We now support 16 practices across Somerset providing care for over 100,000 patients.
Symphony has developed a model where each practice is encouraged to operate autonomously, but as a subsidiary of NHS Somerset Foundation Trust (SFT), and with backing from a central support team (HR, finance, corporate and strategic management matters).
Providing NHS services is at the heart of what we do. Our vision is to ensure that primary care is sustainable for patients and practices in Somerset and we aim to be a national exemplar for primary care provision.
We work closely with our practice teams to grow, enhance and transform patient care and working practices, with innovation and technology at our core.
Symphony welcome applications from people of all backgrounds and underrepresented groups. When undertaking recruitment and selection for our services. We are committed to equality of opportunity for all.
Symphony offers many fantastic employee benefits, including the option to join a NHS or NEST pension scheme, plus varied salary sacrifice options (see attached poster), plus generous annual leave, flexible working from day one of employment and home working options where appropriate to the role.

Job summary
The NSPCN Manager will be employed by QHS, the convener, to provide management support to the four NSPCN Clinical Directors, to enable them to discharge their duties in relation to the National Network Contract Direct Enhanced Service.
The PCN leads on a programme of primary care developments to support at scale neighbourhood service delivery models i.e. geographically coherent groups of service providers working collaboratively to serve natural communities. The PCN works directly with member practices and other health and social care partners in north Southwark across three distinct neighbourhoods: Camberwell and Walworth, Bermondsey and Rotherhithe and Borough.
As one of the Governments Fit for the Future 10 Year health plan, the NHS Network Contract Directed Enhanced Service is transitioning from its focus from PCNs toward supporting the embedding of Integrated Neighbourhood Teams. This is structured around one of the key strategic shifts to transform care: i.e. from hospital to community.
The transition aims to move beyond the current network collaboration between GP practices to true integration with community nursing, mental health, social care, and voluntary sector partners at the neighbourhood level.
The successful postholder will support the four NSPCN Clinical Directors and member practices during the transition year 2026-27 and beyond.
Main duties of the job
The postholder will act as the secretariat and facilitator for all PCN meetings, communications and correspondence, supporting CDs in preparation of the annual workplan, and monitoring delivery of the national PCN DES. On the financial side, the role will manage the PCN Additional Role Reimbursement Scheme claims, submission by the portal, reconciliation against the PCN statement and practice invoices, and all associated administration of the scheme. It will provide practices, from and through a number of effective channels, with relevant data, information and analytics to increase understanding of operational challenges to inform any required changes. More importantly, the role will provide governance, assurance and reporting whilst managing risk mitigation and action planning. Alongside information management, the role will involve workforce planning, line management of non-clinical staff, including health and wellbeing coaches, social prescribing link workers and care coordinators, where appointed, working with other professionals to ensure staff development and progression is person-centred, escalating any patient safety concerns for PCN centralised roles
About us
QHS is a Community Interest Company (CIC) jointly delivering services that improve outcomes for over 200,000 registered patients. 13 North Southwark member general practices have come together as a Primary Care Network (PCN) that provides the building blocks for at scale service delivery and wider system integration through three Neighbourhoods. These are the delivery units for integrated place-based care, bringing together local health and care organisations (including social care, the voluntary sector and patient groups) to work collaboratively to deliver more joined-up, patient-centred services
As part of the NHS 10-year Plan, QHS has joined with the south Southwark GP Federation, Improving Health Limited (IHL) to partner with Guys & St. Thomas NHS Foundation Trust (GSTT) to deliver the Southwark integrator function.
Neighbourhood health service integrators have been tasked with establishing the foundations and building the relationships needed to deliver proactive, integrated and equitable neighbourhood health services shaped by local communities. Integrators will provide the vital infrastructure, connections and coordination needed to turn policy into practice and bring care closer to home.
QHS holds contracts to deliver a range of primary care services, as well as being the employer for all PCN staff.

Job responsibilities
For the main duties and responsibilities for this role please read the attached job description and person specification. When completing your application please ensure your supporting statement reflects the criteria set out in these documents by showing how your experience and skills apply to this post.
We understand that using AI can be a useful and supportive tool when completing your application. However, we have also seen an increase in the over-reliance of AI tools which is negatively impacting applications as they remove candidates individuality, and it becomes much harder to get a sense of who you are and your authentic experience. If you do use AI, it should be used to enhance your application and not to create your supporting statement in its entirety. If we suspect you have used AI to create your supporting statement and this does not match your experience, we may withdraw your application.
As part of our vision to have a skilled, diverse and caring workforce, Dorset HealthCare offers support for people getting into work. We offer an employability service to support individuals with application writing, interview skills and other employment support. If you would like or know of anyone who would value this support, please do [email protected]
