An exciting opportunity has arisen for highly motivated individual to join our Community Nursing Team covering the North East locality of Shropshire in the provision of planned care to patients in the community.
We are looking for a caring, motivated and friendly Community Nurse to join our team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.
Please note this position is community based, requiring the successful applicant to see patients in their own homes. Therefore, a valid driving licence with access to a vehicle for work purposes is essential.
Main duties of the job
You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We also actively encourage newly qualified nurses to join our team. In return we offer a preceptorship program and support professional development in a nurturing environment, allowing you to reach your full potential.
The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
We are committed to clinical support, CPD & training. The post is covering 7 days, 8am to 6pm service.

Main duties of the job
You will play a crucial role in improving mental health outcomes for our service users. You will bring a distinctive social and rights based perspective to the work that you do and promote this within the multi-disciplinary team and beyond. You will liaise with other agencies to ensure that the needs and rights of people with lived experience of mental health are at the front and centre of services and interventions.
You will use your advanced relationship skills to work with people to make positive and self-directed change and support them to work towards recovery as defined by them. You will provide advocacy support and understand the important role of carers, working in partnership to support them especially at time of crisis.
Providing statutory social care assessments under the Care Act and s.117 after-care Mental Health Act is a key feature of the role and you will provide a strengths based, rights focused assessment to identify and meet need, ensuring timely review of packages of support.
Working with complexity and risk is a key feature of the role and you will use your safeguarding adults knowledge to identify abuse and neglect and work with the person and other agencies to seek ways in which to protect and safeguard the person. At times, you will need to use legislative frameworks such as the Mental Capacity Act and Mental Health Act to support people who are unable to make decisions for themselves, balancing and protecting the rights of people and others.

Job summary
We are seeking a Consultant psychiatrist to join the Primary Care Liaison Service in Swindon. The post is an established post which has become vacant due to the previous incumbent moving to another service within AWP. The post involves working as the Primary Care Liaison consultant for a single CMHT catchment are in Swindon.
The post holder will provide clinical leadership in the team, help triage referrals, assess patients and make recommendations on care, provide advice to colleagues on assessment and management, liaising and working closely with the Intensive Service, Community Mental Health Team and Primary Care. The consultant will work closely with the PCLS Manager to ensure that safe, efficient and high-quality service is delivered by the Team. The Consultant will also provide clinical supervision and management supervision for doctors reporting to them.
Main duties of the job
The Consultant will deliver advice to service users including undertaking assessments, anddetermining the most appropriate service provision to meet identified needs, including briefinterventions where required. The Consultant will supervise any medical staff providing inputinto the service. Work collaboratively with the Recovery, Intensive and Inpatient Services tofacilitate transitions through the care pathway The Consultant is expected to contribute tothe development of services in line with best practice and nationally accredited standards. The post holder will be expected to support the senior management to and participate instrategic development both within the local service and the wider Trust. Job summary
We are seeking a consultant psychiatrist to join the Primary Care Liaison Service in Wiltshire. The post is an established post which has become vacant due to the previous incumbent moving to another service within AWP. The post involves working as the Primary Care Liaison consultant for a single CMHT catchment are in North, West and East Wiltshire. The post is supported by a Specialty Doctor.
The consultant (post holder) will provide clinical leadership in the team, help triage referrals, assess patients and make recommendations on care, provide advice to colleagues on assessment and management, liaising and working closely with the Intensive Service, Community Mental Health Team and Primary Care. There is a weekly interface meeting which is attended by PCLS, CMHTs, Primary Care Psychology and North Wiltshire Intensive Service. The consultant will work closely with the PCLS Manager to ensure that a safe, efficient and high quality service is delivered by the Team. The Consultant will also provide clinical supervision and management supervision for doctors reporting to them.
Main duties of the job
The Consultant will deliver advice to service users including undertaking assessments, (jointly where required), making diagnoses, and determining the most appropriate service provision to meet identified needs, including brief interventions where required.
The Consultant will supervise any medical staff providing input in to the service.
- To actively participate in clinical supervision.
Work collaboratively with the Recovery, Intensive and Inpatient Services to facilitate transitions through the care pathway according to the identified care cluster.
- The Consultant is expected to contribute to the development of services in line with best practice and nationally accredited standards.
- Responsible with the team members for the identification of appropriate care cluster and identifying service users who require access to specialist services
- Understand and participate in the maintenance, monitoring and evaluation of service specific performance indicators and targets.
The Consultant is expected to respond to all Advice and Guidance requests within 3-5 working days in accordance with the new PCLS Standard Operating Procedure (SOP). Ashmount
An exciting opportunity has arisen for a Support Worker Band 2 to join Ashmount, within the Trust's Learning Disabilities Directorate. Ashmount is a residential service for up to seven individuals who have a learning disability, autism and challenging behaviour.
We provide intensive support and a wide range of activities from our care service, which has excellent facilities and is set in its own large, secluded grounds.
We are looking for team members who are focussed on promoting people's rights, choices and independence and ensuring they live full and active lives.
The post holder will work positively and proactively with colleagues and will develop up-to-date knowledge of best practice and ranges of approaches within learning disability services and CQC requirements.
The post-holder would be involved working alongside the registered nurses and supporting people to access the community. The post will offer excellent opportunities for development.
Opportunities for joint working with other disciplines and agencies would be an integral part to the role.
The post-holder would need to hold a full driving license and be prepared to work flexibly in various locations across the Trust.
Excellent communication skills and confident use of IT would be key in this role.

Main duties of the job
- Participate in the ongoing monitoring of the health (mental and physical) and behaviour of people living at Ashmount, reporting and recording all observations, reporting to senior staff any changes in the person's condition.

We have an exciting opportunity for a full time 10 PA/40 hours per week Consultant Psychiatrist post in our Spelthorne Neighbourhood Mental Health Team (NMHT).
The NMHT provides neighbourhood care following the Care Pathways underpinned by the principles of clinically focused risk management. The goals are of clinical progression within a recovery framework.

Job summary
NELFT is the largest provider of Mental Health Support Teams (MHSTs) in England. The MHST deliver evidence-based individual and group interventions in educational settings for children experiencing mild to moderate mental health difficulties aged 4 to 18 years old. The teams also have a strong focus on whole school approaches to improve mental health outcomes for children and young people. The MHSTs build on the support already in place from school counsellors, school nurses, educational psychologists and the voluntary sector to support those with mild to moderate mental health issues in school and will help children and young people with more severe needs to access the right support and provide a link to the CAMHS services. The post-holder will be a graduate Education Mental Health Practitioner (EMHP)/Children Wellbeing Practitioner (CWP) qualification and will be fully registered with the appropriate professional or regulatory body (BPS, BABCP).

They will have extensive clinical experience working with Children and Young People (CYP) with emotional wellbeing and mental health difficulties. The Children and Young People's (CYPS) Directorate in Surrey and Borders Partnership NHS Foundation Trust is offering an exciting opportunity for a Band 7 Clinical or Counselling Psychologist to join the Mindworks East Community Hub.
Our Mindworks East Community Hub is split across two sites: Gatton Place (Redhill, RH1 1TA) and Mole Business Park (Leatherhead, KT22 7AD). You will be across both sites, the split will depend on capacity and job planning.
This post will be suitable for a highly motivated psychologist interested in ensuring excellent psychological care is accessible for children and young people with complex mental health needs from diverse backgrounds.
This is a permanent full time (37.5 hours) post.

NHUC are looking for an experienced and motivated Remote Monitoring Service Manager to lead the operational delivery of our newly established Remote Monitoring service. This is an exciting opportunity to play a key role in transforming patient care through digitally enabled services that support people to remain safely at home while reducing reliance on urgent and emergency care services.

Main duties of the job
The Remote Monitoring Service Manager will provide operational leadership for NHUC's Remote Monitoring service, ensuring safe, effective and patient-centred care is delivered across Hampshire and the Isle of Wight. The post holder will lead service mobilisation and ongoing delivery, manage performance against key quality and contractual standards, and support continuous service improvement.

Person Specification Qualifications Essential Educated to degree level or significant equivalent previous proven experience Desirable Postgraduate qualification in leadership, education, management or related subject Experience Essential Experience working within remote monitoring, urgent care, primary care, same day care or integrated urgent care services Experience of leading, supervising or managing multidisciplinary teams in busy / NHS environment Proven track record of service delivery Demonstrable experience of people management skills (incl. leadership, decision making, delegation, negotiating, influencing and persuading skills) ensuring effective team working and quality at all times Experience working under pressure in fast moving environment and able to prioritise workload to meet predefined deadlines.
