Job summary
The post holder will support PHD's long-established Enhanced Health in Care Homes team with the work they do with medicines optimisation and support to Care Homes with medicines management. There will be a requirement to support MDTs for Care Home residents to support proper management of residents in relation to best use of medications.
The role is advertised as 37.5 hours per week, although we are happy to consider applications from candidates looking for a 30-hour working week
In this position, you will be employed by Primary Healthcare Darlington on behalf of Darlington Primary Care Network in a role funded through the Additional Roles Reimbursement Scheme (ARRS) and the General Practice Network Contract Directed Enhanced Service (2026/27). As part of your role, you will work closely with the PCN and the General Practices across Darlington
Main duties of the job
Be an independent prescriber or commit to completing training to become a prescriber.
Support the Pharmacy Technicians and Care Co-ordinators in the EHiCH team.
Support colleagues and patients with medication queries and monitoring.
Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
Focus on people in care homes, those with multiple co-morbidities (frailty, COPD and asthma) and people with learning disabilities or autism.
Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients.
Using audit and action planning, help to tackle inequalities.Provide leadership on person-centered medicines optimisation (including ensuring prescribers in the PCN practices conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement.

Take a central role in the clinical aspects of shared care protocols, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulation services.
Undertake the Centre for Pharmacy Postgraduate Education - CPPE Primary Care Pharmacy Education Pathway or equivalent primary care training pathway.
Provide Clinical Supervision for Pharmacy Technicians within the PCN Enhanced Health in Care Home team.
Assist with care home medication requests, medication advice, covert administration advice from the Enhanced Health in Care Home Team.

NHUC is a not-for-profit Community Benefit Society, working in partnership with the NHS. NHUC provides services through two divisions, Same Day Care (SDC) and TalkPlus.
SDC provides services through the Integrated Urgent Care (IUC) contract which covers the in Hours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision for North Hampshire, North East Hampshire and Farnham & Surrey Heath places which are part of the Hampshire and the Isle of Wight ICB and Surrey & Sussex ICB respectively. Job summary
We are looking for a Health Records Coordinator to join the Health Records department to support the provision of patient Health Record folders for inpatient and outpatient activity, to be scanned for conversion to electronic media, or for audit/research and reference requests. The role will also provide comprehensive administrative support to all St George's Outpatient Services for EDM patients. The post holder will support the preparing, scanning and indexing and QC patient case notes that are being converted to electronic viewing, and all subsequent paperwork produced for patients once they are EDM. The successful candidate will also provide excellent customer service to both internal and external service users and across all areas of the Health Records Department.
Main duties of the job
The post holder will provide a high standard of customer care, acting in accordance with the Trust Values at all time. They will maintain strict patient confidentiality and demonstrate competence in the effective use of NHS systems. The postholder will organise daily workloads to meet service targets and escalate issues to the Team Leader where appropriate, and use initiative to problem-solve. Responsibilities also include preparing health records and patient documentation for scanning, identifying documents unsuitable for scanning, scanning records using specialist software and high-volume scanners, carrying out routine cleaning and upkeep of scanning equipment, and completing quality assurance checks. Accurate logging, indexing, and quality checking of scanned records will be required to ensure digital records meet required standards.

Job summary
The Trust is seeking to appoint a Consultant in Intensive Care Medicine. This is a substantive post, and applicants will be expected to have suitable experience within Intensive Care Medicine. The new appointee will share the caseload, facilities, and trainee grade support with the existing consultants. Requirements of the post-holder are listed below.The post is whole time and offers 8.5 Direct Clinical Care Programmed Activities (PAs) and 1.5 Supporting Professional Activities (SPAs).
This post has been created to support the expansion of the Intensive Care Unit and to improve care for patients with tracheostomies, as well as to provide cover for the ITU follow-up clinic. The post holder will hold a CCT in Intensive Care Medicine.
Experience working as a locum consultant in Intensive Care, or a fellowship in Intensive Care, would be advantageous.
Main duties of the job
The appointment is to the Trust, not to specific hospitals. The appointee will take his/her share of clinical responsibilities within the Department of Intensive Care Medicine, working alongside the existing 29 ITU Consultants. This will include provision of the ITU follow-up clinic and tracheostomy ward round, as outlined in the job plan above. In addition to any duties within a subspecialty of interest, the appointee will be required to provide critical care for adult patients as appropriate to their training and experience.
There is a need to develop a team with a strong emphasis on delivering a cross-site service across the Queen's Hospital and King George Hospital sites. Post-holders will be required to work on each site.

Main duties of the job

The role includes maintaining effective quality systems, contributing to monitoring and reporting arrangements, and preparing reports for senior leaders and committees. You will contribute to service reviews, audits and improvement activities, supporting recommendations and action plans that promote learning and improve standards of care. Main duties of the job
- To undertake specialist paediatric assessment of children on own caseload, who may have complex presentations and from this assessment to formulate a treatment plan and carry out this treatment as an autonomous practitioner.
- To work closely with parents/carers and school staff to support the child.
- To work closely with children's mainstream school and local inclusion teams and other relevant members of the MDT.
- To supervise and support undergraduate students, and be involved in teaching staff as well as those from other agencies.
- To support the implementation of evidence-based projects, including making recommendations for change in practice and participating in departmental research and audit as appropriate.

Job summary
Work within the team that supports the radiology IT systems to enable clinicians managing the North Cumbrian population to have radiology images and results at the point of care. Take responsibility for the Picture Archiving and Communication System (PACS) which stores all the radiology imaging for the Trust and Radiology Information System (RIS) which records the radiology pathway for all patients. Work with local partners to ensure the electronic requesting system is clinically effective for radiology examinations and interfaces with radiology systems. Ensure the radiology IT systems are managed with regards to performance, quality assurance, safety and risk management.
Undertake Clinical radiography at base hospital and other sites if required, taking responsibility for the daily organisation of the service as required, amending workload and schedules to adjust for emergencies. Take responsibility for the wellbeing and positive experience of patients and carers in your locality. Participate in research and suggest areas where formal audit and research could aid service quality and development.
It is anticipated that the role will be an average of 60/40% split between clinical radiography and IT system support, although there will be periods of peak activity in either area and thus flexibility in the rota of the post holder is essential.
Main duties of the job
To provide a radiology service across North Cumbria comprising general radiography, dental, fluoroscopy, angiography (incl. coronary), Bone Densitometry (DXA), CT, MRI and ultrasound facilities. Referrals for radiological opinion come from a wide range of specialities typically encountered within a district general hospital, including 24 hour emergencies. General and Dental practitioners also refer patients into the service. The establishment includes consultant radiologists, Trust Lead, radiographers, nurses, clerical and radiographic support staff.

Job summary
We are recruiting a Autism Pathway Navigator, to support and guide families across east and west Cumbria during the diagnostic Autism assessment process.
The Autism Pathway assessment is the process for children and young people who are being assessed for an autism spectrum disorder. The pathway begins with an Early Support Offer whilst patients and their families wait for an autism assessment, and is completed when family carers and the young person have been offered opportunity to discuss the outcome of the assessment. The role of the Pathway Navigator has been developed specifically to support family carers during the pathway and to ensure a better experience in response to feedback from family carers that the Pathway is difficult to understand and navigate.
Our team provide an effective system of support and supervision, which may include 1:1, pair or group supervision, covering management, clinical and safeguarding supervision.
You will work closely with the Pathway Navigator's already working around the county, to provide equity of service for families across Cumbria.
Main duties of the job
As an integral and highly valued member of the Autism Pathway team, the Pathway Navigator, they will support the Autism Pathway team , working within the Community Paediatric Service to manage the waiting list for the autism pathway, being a point of contact / liaison for parents and families whose children are awaiting assessment. This will include performing welfare checks and ensuring families are kept up to date with their children's progress along the pathway.
The Pathway Navigator will support children/ young people and family carers understanding of the Autism Pathway through the provision of information and advice and enhance connection to local communities.
The Pathway Navigator will take a lead role in advocating for the child, young person and family carers within the service and wider MDT Team, and will also support the understanding of the autism assessment process with external agencies, partner organisations and referrers.
The post holder will establish and facilitate a point of contact for families and carers supporting children / Young People who are being assessed for Autism.
Liaise with clinical colleagues when specific cases need action i.e. pursuing reports, appointments etc
Ensure Autism Tracker is updated to correctly reflect current position for reporting.
