This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. This is an exciting time to join the existing AP team in the Emergency Department here at the Queen Elizabeth Hospital, Gateshead and we are looking for a Trainee Junior Nurse Practitioner (leading to Trainee AP role) who is able to complement our existing team.

Job Title: Registered Manager - Reablement at Home

As Registered Manager you will be responsible for the day-to-day management, leadership and ongoing development of the service, ensuring the delivery of safe, effective, high-quality and person-centred health and social care services. You will have responsibility for the leadership and coordination of multidisciplinary teams, ensuring services are delivered in accordance with regulatory requirements, organisational priorities, contractual obligations and professional standards.
You will support the Operational Manager in achieving strategic objectives and driving continuous quality improvement across the service.
You will ensure the effective application of the Mental Capacity Act, including the completion and oversight of capacity assessments and best interest decision-making, supporting staff to implement evidence-based practice and best practice guidance across service delivery.
You will collect, analyse and present service quality and performance data to inform decision making, service planning and continuous improvement.
You will be involved in liaising and working with other professionals: E.g. Hospital Discharge Team, Focus, CUCT, voluntary organisations, GPs, Therapists and Care Agencies.

The North Somerset CIT team provides specialist mental health support for older adults experiencing severe and enduring mental health difficulties as well as those who have dementia. The driver is an integral part of the blood collection/delivery operation. Duties include the transportation of blood to the WBS centre, delivery of blood and blood components to customer hospitals in the South/Mid Wales area and the delivery of the Public Health Wales (PHW) service level agreement. Main duties of the job
The Welsh Blood Service requires a competent individual to undertake driving and other transport related duties. Knowledge of main and local routes within South, Mid and North Wales would be an advantage. The contracted hours are 18.75 per week, with flexible start and finish duty times based on the operational requirements of the Service. Length of duties will vary and may include occasional night working hours and a possibility of weekend working. Any time worked in excess of 18.75 hours will be paid at the appropriate rate.
Applicants will be expected to work flexible hours. Effective communication skills are essential, as part of the role will involve dealing with external contacts. A current driving licence with C1 and D1 entitlement is essential.

Applications are invited for a fixed term, 6 month, locum consultant post in breast screening, breast symptomatic and general imaging radiology at St George's University Hospitals Foundation Trust.
Main duties of the job
The successful post holder will join a friendly and cohesive team of radiologists with diverse expertise in a busy department that has a strong record of teaching, training and research.
The applicant will work within the existing breast imaging team providing support for the co-located symptomatic and screening services, which are part of the SWL Breast Screening Service. The successful candidate will be expected to have appropriate experience in breast imaging in both the screening and symptomatic settings.
A wide range of experience in general radiology is required.
It is expected that applicants will have the Fellowship of the Royal College of Radiologists and a CCT in Radiology or equivalent training and experience, with specialist accreditation/experience in Breast Imaging.
This post will be 8 PA in total.

Job summary
We are looking for an enthusiastic and highly motivated Assistant Psychologist to work at Callington Road Hospital, assisting with the delivery of psychological interventions as well as contributing to the development and evaluation of Inpatient Psychology as part of the Inpatient Transformation, which aims to make admissions during a mental health crisis therapeutic and Trauma-Informed. We use Comprehend Cope Connect as our core model for delivering CBT-based interventions adapted for acute mental health needs. The job requires excellent interpersonal skills to work with service users and multidisciplinary colleagues, as well as skills to effectively organize and maintain data sets and managing a varied caseload. Experience of working on wards or with people experiencing acute mental health problems would be valuable.
The post-holder will join a supportive Inpatient Psychology team with opportunities to link with peers through team meetings, shared projects, and peer supervision groups. In addition, you can join the dynamic CPD network for inpatient psychological practitioners across the Trust. Opportunities for CPD will be available, tailored to the acute care pathway. We have good links with regional Clinical Psychology training courses and there will be opportunity to work with others in the early stages of their careers including Trainee Clinical Psychologists, other Assistant Psychologists and Undergraduate Placement Students.
Main duties of the job
Service development and evaluation work will make up approximately half of the post-holder's workload; this will include contributing to the development of resources and processes as well as gathering, analysing and reporting on data. Clinically, the post-holder will work on a dedicated ward for part of the week as well as contributing to the running of the site-wide psychology group programme where required. This can include working with intense emotions and distress as people recover from a mental health crisis.

Care Home Pathway Coordinator
The Care Home Pathway Coordinator will be working within the At Home Team closely with the Transfer of Care Hub (TOCH) and will coordinate all aspects of supported discharges and the single point of discharge from St Helier Hospital to care homes.
The service is to ensure care home residents are not unnecessarily admitted to, or delayed in leaving, hospital to the detriment of their wellbeing and independence. The post holder will seek to improve patient flow, reduce delayed transfers of care (DTOC) and avoid unnecessary readmissions, LOS reduction and reduce 4-hour breach for care home residents.
We are looking for an enthusiastic individual to take up the role of Care Home Pathway Coordinator. The post holder will be based at the St Helier Hospital and will coordinate the activities related to the Red Bag journey for patients coming into the Hospital from care homes and ensure the Red Bags are received in the hospital and returned to the care homes on discharge of patients after hospital episode.
The post holder will work closely with all stakeholders to enable the successful delivery of the Hospital Transfer pathway and the Discharge Process for care home residents. The post holder will facilitate the use of the Transfer Pathway and Discharge activities to support safe and quality care to patients admitted from and to care homes.

NELFT is the largest providers of Mental Health Support Teams in England. Our services cover 144 schools. Children experiencing mild to moderate mental health difficulties aged 5 to 18 years old. The teams also have a strong focus on whole school approaches to improve mental health outcomes for children and young people.
Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.
