Job responsibilities

- Provide autonomous clinical assessment, treatment and onward referral within scope of practice.

- Safely prescribe medication in line with national and local guidelines.

- Offer high-quality, person-centred care to patients presenting with acute and chronic conditions.

- Support the Duty Team with urgent care demand.

- Maintain accurate, concise clinical documentation.

- Work collaboratively with the multidisciplinary team.

Person Specification

Experience

Essential

• Essential
• - Registered ANP or Nurse Practitioner with the NMC, or Paramedic Practitioner registered with the HCPC.
• - Minimum two years' experience working autonomously in primary care or an acute clinical environment.
• - Independent Prescriber qualification.
• -Demonstrate experience managing a wide range of minor illness presentations.
• - Proven ability to work both autonomously and as part of a wider clinical team.
• - Strong communication skills and the ability to build positive rapport with patients and colleagues.

Desirable

• - Experience in chronic disease management.
• - Commitment to promoting equality, diversity and the rights and dignity of patients, carers and colleagues.
• - A welcoming, non-judgemental manner that respects each individual's needs, beliefs and circumstances.
