We are currently recruiting additional Salaried General Practitioners, with the potential for future partnership opportunities. We are ideally seeking candidates who can offer up to 8 sessions per week. A competitive salary will be offered, commensurate with experience, with scope for enhanced remuneration for special interests or additional responsibilities.

This opportunity is well-suited to both newly qualified and experienced GPs. Comprehensive support is available, including mentorship if required.
This post will involve cross working between both sites, and the opportunity to work within our Acute Care Hub.
Main duties of the job
To use professional skill and knowledge to provide a high standard of primary and continuing medical care to patients in line with the GMS contract
Contribute to the delivery of patient services in line with ICB requirements
Effective clinical administration, audit and management, appropriate to the role
Contribute to the Practice team by being an effective team member

Undertake all the duties associated with the role of a General Practitioner required by the GMS contract, including patient consultations, duty doctor cover and home visits. This should take in to account physical, psychological and social factors when diagnosing illness, treating diseases, tending to injuries and recommending treatment. Work in accordance with the Practice rota system in place.
Collect, record and maintain patient information, such as medical history, reports and examination results. Information is recorded on the practices clinical system and this information should be recorded accurately, using templates and read codes and in a timely manner. Comply with all the objectives of the GMS Contract, QOF targets, ICB requirements arrangements
Work in accordance with Practice clinical procedures and protocols. All work within the practice is governed by protocols and procedures alongside other additional governance requirements from outside organisations
Request, perform and/or interpret tests and records, reports and examination information to assist in diagnosis of patients condition. Prescribe or administer treatment, therapy, medication, vaccination and other appropriate medical care to treat or prevent illness, disease or injury. Monitor patients condition and progress and re-evaluation and/or change treatments as necessary
Refer patients to medical specialists or other practitioners appropriately, when necessary and in a timely manner ensuring compliance with protocols and procedures for referral
Promote health education in conjunction with other health care professionals and have an awareness of the local health and social issues.
Complete all allocated and self-generated clinical administration.
Carry out clinical audits as appropriate.
Completion of reports for solicitors, insurance companies and other third parties as required.
Liaise with other health care professionals when required, including support to the nursing team during minor illness clinics

Sound PCN (Primary Care Network) are seeking a dynamic, enthusiastic, patient-focussed, and independently motivated pharmacy technician to join our existing pharmacy team to provide quality patientcare.
The successful candidate will work across the 5 GP Practices in the PCN, developing medicines optimisation services.

Main duties of the job

To advise Practice Management and other non-clinical staff concerning administrative aspects of medicines management as appropriate
To optimise medicine management, resolve queries where possible and produce prescriptions for clinical review and signature
To work within own competencies as a Pharmacy Technician and ensure that registration and CPD (Continuing Professional Development) is kept up to date.
To address the following areas without the need to consult a GP/Clinical Pharmacist in each case (although clinical support and advice will always be available if required):
1.Update regime review dates when conducting level 1 and 2 medication reviews
2.Medicine reconciliation of discharge summaries and other clinical documents.
3.Optimisation of repeat medication, cost effective switching, commencing repeat dispensing and synchronisation of medication
4.Ordering of required monitoring i.e., blood tests, blood pressure, height, weight etc. for medicine/condition monitoring.
5.Addition/removal of medication for Nursing Home patients under GP/Pharmacist Prescriber guidance
6.Co-ordinate District Nurse Medication and Dressing requests for the practice population
7.Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds, and stoma care products.
8.Work alongside colleagues or independently to deliver project work relating to medicine optimisation, an example of this may be deprescribing Gabapentanoids or Bisphosphonate reviews
9.When requested by a clinician to be able to have appropriate conversations with patients and initiate medications such as statins, Vitamin D, Folic Acid, iron supplements etc as per PCN protocols.
To work in conjunction with the nominated Clinicians who support Nursing Homes in the area
Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate
