Are you organised, enthusiastic and motivated, with a passion for delivering excellent patient care? We are looking for a dedicated Medical Secretary/Administrator to join our friendly practice team.
This is a varied and rewarding role, working closely with our clinical team and Administration Hub Lead to provide essential secretarial and administrative support and help ensure the smooth running of our practice.
Your responsibilities will include coordinating patient referrals, completing Advice and Guidance requests, maintaining referral records, supporting audits, summarising clinical records and processing correspondence. You will also provide administrative support across the practice, working closely with colleagues to deliver a professional and efficient service.
We are looking for someone with excellent organisational and communication skills, strong attention to detail and the ability to manage competing priorities. You will be a positive team player who is approachable, flexible, respects confidentiality and is keen to learn and develop.

Main duties of the job
- Deal with all aspects of referrals, working with the clinicians and Administration Hub Lead
- Ensure all referrals are sent within the timeline the referral is requested.
- To complete all Advice and Guidance for clinicians
- Maintain a referral data base and support audits on referrals.
- Read code relevant information in relation to the patients care from the patients clinical letters, into the clinical computer system.
- Summarise clinical notes
- Deal with day-to-day post including electronic post and discharges; scan the post directly onto the computer system.
- Answer the telephone at busy periods when requested to support the reception team to provide safe patient access to the service.
- Ensure total familiarity with all appointment systems including regular and incidental variations
- To provide administrative support and services to all members of the practice team including reception.

About us
We are a modern, friendly practice,East Park Medical Centre (LS9) and Halton Medical Practice (LS15) have around 11000 patients across the two sites, with 10 GP/ANPs offering a mix of routine and urgent on-the-day appointments.
Our nursing team consists of Nurses, HCAs and a dedicated Mental Health Nurse.
The Care Navigation Team supports our patients with Care Navigators, Prescribers, Care Coordinators and a Safeguarding Lead.
We have one GP Partner, a Director of Clinical Quality, a Director of Nursing and a HR, Target & Finance Lead and a Communication, Health &Safety Lead all overseeing day-to -day operations.

The role requires a flexible approach and somebody that thrives in an extremely busy and care focused environment.
You will need to be able to manage your own workload and also work as part of a team.
Experience of working in a GP Practice or NHS setting is desirable but full training will be given.
The position will comprise a variety of early and late shifts as below:
- Early shift: 08:00 - 16:00
- Later shift: 10:30 - 18:30
Both with a half an hour unpaid break
Main duties of the job
- Managing and triaging all patient queries both over the phone, online and on our busy front desk
- Liaising with other healthcare/social care departments with regards to our patients
- Utilising our clinical systems to keep accurate medical records for our patients
- Actioning requests from our clinical team to support patient care

JOB SUMMARY
We are looking for a Practice Manager to join our dynamic and highly motivated team delivering excellent healthcare for our NHS Surgery
SSP Healths Managers benefit from our substantial network of back office support functions to allow each to really focus on patient services and lead the local team of doctors, nurses and staff.
You will ideally have some experience in a General Practice setting together with an understanding of the rapidly changing healthcare environment. Applications will also be welcomed from non Primary Care Managers who are able to identify transferable skills that can support our practices and demonstrate an understanding of current demands within Primary Care.
Excellent interpersonal and organisational skills together with proven experience in managing people and monitoring performance targets is required. You must be highly self-motivated, organised and able to motivate your team.

You will have the aptitude to support practice engagement with the wider health economy as required, such as local Primary Care Networks, local practice manager networks and community care providers, to ensure that we are consistently kept aware of best practice and new developments.

The practice is currently contracted by the NHS England to provide essential, additional and some enhanced services under the terms of the General Medical Services Contract of April 2023. All members of the practice team should work toward fulfilment of these contractual arrangements and support and fulfil subsequent contract changes as they occur.

Job summary
This role will be responsible for supporting practices across the Primary Care Network (PCN) and will work in locations within the community to provide clinical services. The areas covered by the PCN are Holt Park, Horsforth, Burley, Kirkstall, Headingley, Woodhouse, Hyde Park.
Main duties of the job
The Social Prescribing model aims to improve physical, mental and social health and wellbeing, through a holistic approach to improving quality of life and confidence in self-management and supporting independence. Working with our local population to improve the way the needs of local populations are met. This will be both on a 1:1 basis and where appropriate through the development of group work.
The team take referrals to support individuals to build independence and personal resilience, enabling self management of their health and wellbeing tailored to individual need. They will also take a lead in supporting administrative staff to develop their signposting skills raising awareness of the different services on offer locally.
The post holder will lead a team of 6 social prescribers and work closely with GP practices, the PCN Leadership Team and the PCN Manager.

We have an exciting opportunity for a Clinical Pharmacist to join our multi-professional Primary Care Team working within our Health & Wellbeing Hub under the direct supervision of the Clinical Director. Woodsley & Holt PCN would like to recruit a Podiatrist to work within our Primary Care Network (PCN) multidisciplinary healthcare team. This role will be based across the PCN area of Headingley, Horsforth, Burley, Kirkstall and Cookridge areas.
Our ambition is to improve management of patients suffering with various foot conditions and other at-risk groups that have limited access to our community podiatry service.
Main duties of the job
This role will work closely with other PCN roles: Pharmacists, Physiotherapists, Dietician and Social Prescriber in addition to the core practice teams.
You will assess, investigate, manage foot conditions and refer patients who have been directed to your service by a primary healthcare worker or directly by reception where appropriate. You will be an experienced practitioner who is comfortable working independently as well as part of a wider multidisciplinary team. You will be supported by the clinical team at each GP surgery and work with the wider clinical team to deliver holistic care to our patients. An exciting opportunity has arisen for an experienced Registered Nurse to join Nexus MK PCN as a Frailty/ICT Nurse.
Working across Nexus MK PCN member practices, the successful candidate will lead on the proactive identification, assessment, and management of patients living with frailty. The role will involve delivering regular care home ward rounds, supporting patients within their own homes and care settings, and representing frailty patients at Integrated Neighbourhood Team (INT) and Multidisciplinary Team (MDT) meetings.
This role is central to the PCN's frailty agenda, working closely with GPs, care home staff, community services, social care teams, and the wider multidisciplinary team to deliver proactive, person-centred care. The aim is to improve patient outcomes, reduce avoidable hospital admissions, and support frail patients to remain independent and well within their usual place of residence wherever possible.
Main duties of the job
- Proactively identify and assess patients living with moderate to severe frailty.
- Undertake comprehensive frailty assessments and clinical reviews.
- Develop, implement, and review personalised care and support plans in partnership with patients, families, and carers.
- Deliver regular ward rounds within designated care homes across the PCN.

Providing senior clinical decision-making for uncertain diagnoses, persistent symptoms or cases requiring specialist ENT assessment.
Managing moderately complex chronic ear disease, recurrent otitis, selected dry tympanic membrane perforations, hearing loss and tinnitus where tertiary otology is not required.
Managing persistent nasal obstruction, chronic rhinosinusitis, uncomplicated nasal polyps and septal deviation, with definitive treatment where appropriate and commissioned.
Managing persistent globus, benign tonsillar disease and low-risk dysphonia, including laryngeal examination.

Main duties of the job
The successful applicant will undertake the full range of duties expected of a Salaried GP, including:
- Routine face-to-face and telephone consultations
- Same-day and urgent care clinics
- Clinical triage using our total triage model
- Duty doctor sessions
- Home visits where clinically appropriate
- Reviewing pathology, correspondence and clinical results
- Working closely with our multidisciplinary team, including nurses, ACPs, pharmacists, paramedics and other allied healthcare professionals
- Supporting safe and effective clinical decision-making across the practice
- Participating in clinical meetings and quality improvement activities
Experience of working within a total triage model or confidence in clinical triage would be advantageous, although support can be provided.
