We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Community Matron/Advanced Visting Practitioner. If you love building relationships, enjoy working autonomously, and want to make a real difference to frail or housebound patients, we would love to hear from you.

We have already seen fantastic results from our current Community Matron, who works primarily in local care homes. Because of this success, we are expanding the service to include our housebound patients too giving them better continuity, more personalised support, and high-quality care in the comfort of their own homes.

Main duties of the job
- The post-holder will be an experienced nurse/paramedic, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patient within the general practice.
- The post-holder will provide this service to our care home residents, patients in their own homes and at the surgery.
- The post-holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
- The post-holder will build effective working relationships with other health care professionals.
- The post-holder will work within the team of GPs, nurses and health care assistants to deliver excellent primary health care services within the Practice.
About us
Operating from our main site in Paignton and our branch site in Torquay we care for upwards of 25,000 patients in an organised and strong team environment.
