Deben Health Group PCN is looking for a careful, organised and motivated Data Administrator/Clinical Coder to join our award-winning, friendly team. This position is offered on a full or part-time basis so please apply to discuss hours.
Supporting approximately 38,500 patients across Wickham Market Medical Centre, Peninsula Practice, Little St John Street Surgery and Framfield House Surgery, you will help ensure important clinical information is recorded accurately and consistently.
Main duties of the job
- Reviewing and summarising patient records
- Coding clinical correspondence into SystmOne
- Managing documents and electronic tasks
- Supporting data-quality audits, QOF and shared coding workflows.
Training and support will be provided, and unclear or clinically sensitive information will be referred to an appropriate clinician or senior colleague.

Job summary
South Rural Primary Care Network are looking to recruit a PCN Clinical Pharmacist to join their established team based at Holbrook and Shotley Practice. Offered on either a full or part-time basis, the successful candidate will be expected to work across both practices.
The post holder will be an integral part of the general practice team, working in each practice and across the PCN, as well as part of a wider pharmacist network and multidisciplinary team. This includes being a conduit of patient medicines information into and out of hospitals. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.
Main duties of the job
- Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas
- Act as the PCN point of contact for all medicine related matters, establishing positive working relationships
- Liaise with the practices and, where practicable, to standardise the medicines management process across the PCN
- Consult patients within defined levels of competence and independently prescribe acute and repeat medication
- Receive referrals and directed patients from triage services and other clinicians
- Receive and resolve medicines queries from patients and other staff
- Provide medication review services for patients in the practice and during domiciliary visits to the local nursing home
- Manage a caseload of complex patients
- Manage therapeutic drug monitoring system and recall of patients taking high risk drugs, i.e. anticoagulants, anticonvulsants and DMARDs etc.
- Deliver long term conditions clinics and home visits particularly for patients with complicated medication regimes and prescribe accordingly
- Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team

South Rural PCN provides services to 56,000 patients across rural Suffolk and is headed by 2 proactive, supportive and committed Clinical Directors. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre, The Holbrook and Shotley Surgeries and Needham Market Country Practice.
Bildeston Health Centre is a friendly, medium sized, rural dispensing practice. We are CQC rated GOOD and score very highly on the Patient Experience survey. We have a growing list size of 7,600+ patients with two GP partners. We currently have 2 practices nurses, 2 HCAs, and a very supportive care navigation and admin team.
We are active members of the South Rural PCN who provide us with a practice paramedic and clinical pharmacist.

Job summary
Botesdale Health Centre are looking for a professional, conscientious and highly effective Pharmacy Technician to join our friendly team on a part-time basis for 24 hours per week.
Working within their professional boundaries, the successful applicant will work alongside the clinical and dispensary team within the practice and the wider Blackbourne PCN. They will support the practice clinical teams with queries regarding medication from patients, complete audits regarding patient safety in the prescribing of medication and supports public health and screening programmes including immunisations and vaccination programmes whilst participating in quality improvement initiatives to enhance patient outcomes and reduce health inequalities.
Main duties of the job
- Support medication reviews and medicines reconciliation for new care home patients.
- Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.
- Assist in conducting prescribing audits, responding to national safety alerts (e.g., MHRA), and implementing Quality Outcomes Framework (QOF) and prescribing incentive schemes.
- Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g., use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.
- Work with the multi-disciplinary team to ensure efficient medicines optimisation, including implementing efficient ordering and return processes, and reducing wastage.
