We are looking to recruit an experienced full-time First Contact Physiotherapist (37.5 hours per week) to join our established and supportive team.

As a First Contact Physiotherapist, you will work on-site in GP practices as part of a dynamic multidisciplinary team. You will assess, diagnose, and manage MSK patients at the first point of contact - with full autonomy and clinical support.

You will be embedded in your practice(s), working closely with GPs, receptionists, nurses, and other MDT colleagues to deliver integrated, personalised patient care.

You will have the opportunity to use or develop advanced skills, including injection therapy and independent prescribing, supported by protected CPD time and structured supervision.

We are delighted to be recruiting an Advanced Physiotherapy Practitioner (APP) to join our established, forward-thinking team on a full-time basis (37.5 hours).

This is fixed term posiiton until 31st March 2027.

In this onsite role, you will work within GP practices as part of a multidisciplinary team, providing expert assessment, diagnosis, and management of musculoskeletal (MSK) conditions at the first point of contact. Main duties of the job

As an Advanced Physiotherapy Practitioner (APP), you will be an autonomous, highly skilled clinician working within GP practices to assess, diagnose, treat and manage musculoskeletal (MSK) conditions and undifferentiated presentations. You will independently manage patient care and, where appropriate, discharge patients without the need for medical referral.

Patients may access the service directly through self-referral, or be signposted by GP practice staff, enabling rapid assessment, accurate diagnosis and timely intervention. Be the Mental Health Leader Patients Need - Right at the Heart of Primary Care

Are you passionate about transforming mental health support where people live, work, and thrive?

We are looking for 2 dedicated Registered Mental Health Nurses to join our innovative, compassionate, and community-focused prevention team in Liverpool - the role will be based at a community hub and the exact location is TBC. We are interested to hear from applicants who are available 7.5-37.5 hours per week.

In this role, you'll support people with mild to moderate mental health needs right at the heart of primary care where early support can have the greatest impact.

You'll work as part of the MDT, alongside GPs, Physiotherapists, Clinical Pharmacists, Occupational Therapists, Podiatrists, Social Prescribers, and other allied professionals to offer joined-up care that puts patients first. Together, were rethinking how mental health is managed in General Practice and we want you to join us on our journey.

This role is well suited to clinicians who enjoy autonomous decision-making, variety, and working at pace within primary care, rather than those seeking a traditional therapy-based caseload.

Main duties of the job

As a Mental Health Nurse working at Enhanced Level Practice (level 7), you'll play a central role in delivering the PCN's mental health offer acting as a first point of contact for patients with mental health concerns in primary care.

This role operates within a busy primary care environment, where timely access and early intervention are key. Clinicians work within structured clinic templates, supporting a consistent and manageable flow of patients.

Up to approximately 80 appointment slots per week will be available within your clinic template. As is typical in primary care, not all patients attend, and not all slots will be filled. This generally equates to delivering around 13-14 patient contacts per day across a mix of new assessments and follow-ups.

You'll be supported to deliver high-quality, structured consultations that prioritise safe, effective, and person-centred care, while helping patients access the right support at the right time.

You will work closely with GPs, pharmacists, district nurses, palliative care teams and care home staff, contributing to regular MDT meetings, structured care home rounds and proactive reviews. A key part of the role is helping to prevent avoidable hospital admissions by identifying deterioration early, supporting personalised care plans and ensuring continuity for residents with complex needs.

Alongside clinical work, you will provide leadership within the MDT, support quality improvement activity and help strengthen frailty and care home pathways across the PCN.
