Job summary

New Beginnings is a specialist detoxification unit for drug and alcohol service users combined with a structured day programme accessible to service users undergoing detoxification. The staff on the detoxification unit work as part of an integrated team with the structured day care staff to offer a comprehensive and seamless programme to service users.

The post holder will provide high quality support services as part of the inpatient clinical team and structured day programme within Aspire New Beginnings.

To support the clinical and the wider New Beginnings team in the delivery of the service, whilst performing a range of duties under the direction of the nurse/manager in charge.

To act as first point of contact for the service providing an effective information service for professionals and service users.

Main duties of the job

To provide comprehensive, high quality support services to the clinical team, under direct supervision, to ensure the smooth safe delivery of services.
To support the team in the admissions and discharge process and facilitation of both clinics and group work sessions with patients.
To support the clinical team by completing appropriate screening tools and records prior to consultations/ admission taking place.
To give verbal and written feedback to senior clinicians in respect of patients individual care plans.
To escort patients to outside appointments.
To undertake collection of samples and specimens from patients and test as directed, sending samples for testing to the pathology department when required.
To assist the Nurse to conduct physical health checks and the management of symptoms through appropriate care planning.
To provide advice and information to service users about the range of services on offer both within the statutory and non-statutory sector and assist service users in accessing these.
To provide harm minimisation advice and promote healthier lifestyles.
To provide access to activities and support patients in cooking and other life skills.
To deliver an agreed programme of supported group activities at the weekend.
To support service users during their detoxification to maximise their recovery.
Ensure that appropriate communications are actioned and documented on the clinical database (SystemOne)

Magnolia Ward at Tickhill Road Hospital is a Neurological Rehabilitation in patient facility with 8 beds offering rehabilitation to individuals who have neurological illness and disability. Our patients can experience physical difficulties, cognitive difficulties and social difficulties so the rehabilitation that we provide is truly holistic to their needs.

Our patients benefit from a multi-disciplinary rehabilitation approach with a team which comprises of Medical staff, Nursing staff, Physiotherapy, Occupational Therapy, Psychology, Speech and Language Therapy and Rehabilitation Assistants.

Main duties of the job

We have an opportunity for a motivated and enthusiastic Staff Nurse to join our team in delivering neurological rehabilitation. The successful applicant will be an autonomous practitioner and work alongside the team whilst taking responsibility for coordinating and managing the shift. They will provide clear direction to the Rehabilitation Assistant staff and maintain a flexible patient focused approach in managing complex situations.

Neurological Rehabilitation is both rewarding and challenging and the post holder will be supported to use clinical assessment skills to develop patient centred care plans. They will be supported to further develop by having access to:

regular clinical and managerial supervision;
specialist rehabilitation nursing staff knowledge;
facilitated reflective practice sessions;
in house and external training;
post-graduate neurological rehabilitation training.

The successful applicant should be a Registered Nurse. They should have recent evidence of competence in caring for people with physical disabilities and physical health needs and experience of working in a multi-disciplinary team. Experience working within neurology or rehabilitation would be desirable.

Main duties of the job

To carry out the repair, maintenance or installation of all plant, equipment, systems and building structure/fabric associated with all Trust properties. This will consist of planned preventative maintenance (PPMs), testing and calibration, fault finding, emergency response, defect correction and minor works.

You will be employed to work appropriate to your core skills around building and engineering plant and equipment maintenance. You will progressively be trained, where required, to acquire other skills with the intention of reaching levels of multi trade flexibility and competence. You will work closely with the Senior Technicians to improve planned maintenance.

Job summary

This is a new additional role to the East Primary Care Network (PCN) to join well established band 7 and band 6 practitioners.

A PCN is a group of GP Practices coming together to share their experience, workforce and services with the aim of improving access and meeting the needs of local patients and communities. The East PCN covering areas including Armthorpe, Dunsville, Hatfield, Stainforth, Thorne and Moorends.

The role is 24 hours per week, working over 3 or 4 days top include Monday/ Friday.

Main duties of the job

Provide a role in supporting Primary Care Networks (PCN) in triaging mental health concerns and providing specialist advice, signposting and forward referral as appropriate.
Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system
To act as a point of contact for GPs to gain advice and support regarding people's mental health.
To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.
To provide a comprehensive Primary Mental Health Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery.
