Job summary
Shelley Manor & Holdenhurst Medical Centre is seeking an enthusiastic, highly motivated and organised Care Coordinator who has had previous experience of working in primary care. We are looking for someone to support our more vulnerable patients through their health care journey by being a patient advocate/supporter, as well as having excellent IT skills to be able to undertake searches and reports, manipulate data in excel and communicate with colleagues to support the management of patient registers, QOF and IIF.
Main duties of the job
Our Care Coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations and other primary care professionals. You will work closely with the GPs, managers and colleagues within the practice to identify and manage a cohort of identified patients, making sure that appropriate support is made available to them, inviting patients for appointments, managing clinics and ensuring that their needs are addressed. The focus is on the delivery of a comprehensive model to reflect National clinical pathways, local priorities, health inequalities and patients at risk.

We are a supportive and successful training practice, with a strong ethos of staff development. You would be joining a robust team up of 9 partners, 4 Salaried GPs/Retainers, Trainees, ANPs, Nursing team, Pharmacy team, First contact physios as well as our management and admin team.

Beaufort Road Surgery, Littledown Surgery, Shelley Manor and Holdenhurst Road Medical Centre, Southbourne Surgery

Main duties of the job
Provide up to 3 clinical sessions Thursday 16.00-20.00, Saturday 09.00-13.00/13.00-17.00 from the Treatment Centre seeing patients from all 4 practices.
Working with the Lead GP for Treatment Centre.
Provide clinical advice and guidance to the multi-disciplinary TC team.
Maintain the highest clinical standards at all times and practices in accordance with accepted good medical practice and service standards.
Supports the development of PCN service delivery at the TC
Our Network teams include:

Main duties of the job
Working in the treatment centre under the direction of qualified clinicians.
To collect venous/capillary blood samples without harm to patient or self by following the phlebotomy standard operating procedures.
To obtain such blood samples to a standard fit for laboratory testing ensuring collection, maintaining appropriate files & records as required.
To carry out observations including but not limited to height, body mass index, pulse rate and rhythm and blood glucose monitoring, BP monitoring manual and electric, temperature, urinalysis, Phlebotomy, ECGs, Stitch removal, minor simple dressings.
Undertaking respiratory diagnostics; Spirometry and Feno testing.

Take on board the need to use alternative communication skills when required to ensure patients fully understand when transmitting information. Participate in practice meetings as and when appropriate

We are recruiting for an experienced Healthcare Assistant.
23 hours per week, including OOH sessions on a permanent contract, working with our PCN Treatment Centre.
Must include, Monday 13.00-20.00, Thursday 08.00-12.00, Friday 13.00-20.00
£13.50 hourly rate, OOH includes 30% enhancement on hourly rate.

Main duties and responsibilities (this list is not exhaustive):
Working in the treatment centre under the direction of qualified clinicians.
To collect venous/capillary blood samples without harm to patient or self by following the phlebotomy standard operating procedures.
To obtain such blood samples to a standard fit for laboratory testing ensuring collection, maintaining appropriate files & records as required.
To carry out observations including but not limited to height, body mass index, pulse rate and rhythm and blood glucose monitoring, BP monitoring manual and electric, temperature, urinalysis, Phlebotomy, ECGs, Stitch removal, minor simple dressings.
Undertaking respiratory diagnostics; Spirometry and Feno testing.
Conducted catheter fittings while maintaining infection prevention and patient-centred care- Training will be provided

Job summary
The PCN has a vacancy for an enthusiastic, highly motivated and organised Reception/Admin (Care Co-Ordinator) on a permanent, part time contract basis, who has previous experience of working in a Healthcare setting, to work within our PCN teams including Pharmacy team support.
The Reception/Admin (Care Co-Ordinator) main role is to provide key administrative support to the PCN Teams and practices teams within the PCN, manage the reception desk, facilitate effective communication and support to our patients, and work closely with the PCN clinicians to co-ordinate a range of patient services following their appointment.
Also, this role will include cross cover between all PCN teams full training will be provided.
Our Reception/Admin (Care Co-Ordinator) team will offer general assistance to the clinical team who work within our PCN teams. Projecting a positive and friendly image to patients and other visitors, either in person or via the telephone.
The role will involve working closely with the PCN Pharmacy Team and other PCN Clinicians where required.
The job role will require excellent IT skills with all MS office, strong meeting notes dictation, analytical and strong organisation skills, as well as the ability to multi-task and be able to communicate with colleagues at all levels in a friendly and professional manner and demonstrate empathy and compassion to our patients.

Job summary
We are recruiting for an experienced Phlebotomist.
16 hours per week on a permanent contract, working with our PCN Enhanced Team.
£12.89 hourly rate.
This role will require an Enhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.
As the role involves patient home visits, you are required to be able to drive, own a vehicle for work, and have Business Car Insurance.
Main duties of the job
Main duties and responsibilities (this list is not exhaustive):
-Working in the community with housebound patients under the direction of a qualified nurse.
-To collect venous/capillary blood samples without harm to patient or self by following the phlebotomy standard operating procedures.
-To obtain such blood samples to a standard fit for laboratory testing ensuring collection, maintaining appropriate files and records as required.
- To ensure that all phlebotomy equipment and accommodation is properly maintained and suitable for use.
- To perform basic observations including SATS and BP.
Experience of home visiting is desirable however further training will be provided.
