We are looking for an individual to be part of our fast-moving, innovative and ambitious service, dedicated to supporting our communities to manage their own health both at home and in our dedicated clinics.
You will be responsible for providing skilled nursing care to patients across the locality and work flexibly with multi disciplinary teams across our service to support our patients journey.
You will need to demonstrate excellent communication and interpersonal skills and be adaptable, reliable, resourceful and approachable. You'll need to be organised and confident and able to cope with changing environments as you'll be working independently in patients homes. Support is always available.
Main duties of the job
Our community teams of Nursing and Allied Health Professionals deliver care to people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in the patient's own home, including care homes, enabling them to be independent, self-caring and live their optimum lives, reducing hospital admissions.
Services range from community nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry.

We have an exciting opportunity for x2 Senior Healthcare Assistants to join our team on the Inpatient Unit. Were looking individuals with NVQ Level 3 and previous experience working as a healthcare assistant. You must also be, enthusiastic, flexible, and have a passion for palliative and end of life care, have excellent communication skills and be a good team player. This is an exciting time to join the community nursing service in Hertsmere, covering Potters Bar, Borehamwood, Bushey, Radlett and surrounding villages. Main duties of the job
- To give skilled nursing care, based on principles of best practice and clinical evidence. This includes: Chronic disease management, end of life care, continence promotion and wound management. Specific skills include phlebotomy, administration of medication via syringe drivers and intravenous therapy, PEG management, urinary catheterisation and compression bandaging.
- To take continued responsibility for the holistic health needs assessment including care planning, evaluation and appropriate implementation of health care and ensuring continuity of care following referral from acute care, rehabilitation units, GPs and other agencies.
- Undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process and including the needs of carers. To provide individualised and person centred care plans that provide monitoring and promotion of self-care to prevent unnecessary hospital admissions.
- Act as the patient's advocate and to facilitate the patient's own choices with regard to nursing care, promoting independence and self-care, as appropriate.
- To identify and record new problems and other relevant information reporting back to the DN Team Lead as appropriate.
- Maintain accurate, comprehensive and contemporaneous records in accordance with Trust and NMC guidelines.

We are looking for friendly, organised and proactive Receptionist/Administrators to join our busy and supportive team at Crown Medical Centre. We have two part-time vacancies available, each offering up to 30 hours per week.
This is a varied and rewarding role where no two days are the same. As one of the first points of contact for our patients, you will play a vital role in ensuring they receive a professional, compassionate and efficient service while supporting the smooth day-to-day running of the practice. You will work across reception and administration support, using excellent communication and organisational skills to help deliver high-quality patient care. Main duties of the job
