Main duties of the job
We are looking for a caring, enthusiastic and motivated Healthcare Assistant (HCA) to join our friendly and supportive practice team.
The HCA will play an important role in supporting our clinical team and providing high-quality, person-centred care to our patients. The role will involve a range of clinical duties appropriate to the candidates skills and experience, working alongside GPs, nurses and the wider practice team.

Job summary

St Peters Health Centre is looking for an enthusiastic Patient Care Advisor (PCA) to join our practice team. As one of the first points of contact for our patients, you will play an important role in making sure every patient feels welcomed, listened to and supported.
This is a varied and rewarding role combining excellent customer service with reception and administrative duties. You will help patients access the right care by managing appointment requests, providing clear information, supporting the use of digital services such as SystmConnect and the NHS App, and signposting patients to appropriate NHS and community services.
Youll be joining a busy, supportive practice where teamwork and providing an excellent patient experience are at the heart of what we do. We are looking for someone with strong communication and customer service skills who can remain calm under pressure, use their initiative and work positively as part of a wider multidisciplinary team. Previous general practice experience is desirable, but a willingness to learn and develop is equally important.

Main duties of the job
As a Patient Care Advisor, you will be at the heart of the practice, helping patients access the care and support they need while ensuring their experience with us is positive, efficient and compassionate.
Your day will be varied and will involve communicating with patients face-to-face, by telephone and through our digital systems. You will listen to patients, understand what they need and help them navigate the most appropriate route for their care. This may include arranging appointments, supporting online requests, providing information or directing patients to other NHS and community services.
Alongside patient-facing work, you will carry out a range of administrative tasks to support our clinical team and keep practice services running smoothly. Accuracy, confidentiality and good communication are therefore essential.
You will work closely with GPs, nurses, other healthcare professionals and administrative colleagues, supporting one another during busy periods and responding to the changing needs of the practice.
This is a busy and responsible role where no two days are quite the same. It would suit someone who enjoys working with people, can prioritise effectively and takes pride in providing excellent service to patients.

We are looking for a part time practice nurse help us care for our 18,000 patients in the centre of the city.
Main duties of the job
The Practice Nurse is to support other healthcare professionals within the organisation and across the wider network to enhance integration with community, secondary, and social care services.
This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening, Furthermore, they embed population health management approaches within general practice, targeting health inequalities and improving access.
The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team. They are to participate in formal training events and to promote best practice in their area of expertise as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

We are a training practice, currently hosting 2nd year Medical Students, Foundation Year 2 Doctors, shortly ST3s, nursing and pharmacy students.

The following are the core responsibilities of the Practice Nurse in delivering health services:
a. Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
b. Consult with patients and their families to undertake assessments of patient need
c. Support in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives
d. Support the development ofan effective care management programme for the frail and housebound patients registered with the organisation working with the PCN team
e. Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice
f. Embed population health management approaches within general practice, targeting health inequalities and improving access
g. As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
h. Support new-to-practice nurses and other healthcare professionals within the organisation
i. Teach and advise patients and their families on how to manage their condition
j. Participate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locally
k. Work within national and local protocols where these exist
l. Recognise the boundaries of their practice and know when and to whom patients should be referred
m. Support in the delivery of enhanced services and other service requirements
n. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
o. Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events
p. Undertake all mandatory training and induction programmes

t. Lead on public health campaigns (e.g., flu clinics)
u. Provide travel advice services St Peters also has a private travel clinic and there is an expectation that the post holder will work in this area as well.
v. Provide wound care (ulcer/Doppler, etc.) to patients
w. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively
x. Contribute to practice targets (QOF, etc.), complying with relevant guidance
y. Maintain chronic disease registers and implement an effective call/recall system
z. Maintain a clean, tidy, effective working area at all times
aa. Chaperone patients where necessary
bb. Support the team in dealing with clinical emergencies
cc. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels
