Community Support Worker for D2A/TOCT/TOCH - Transport Support - Band 3 - 30 hours
The post holder will provide support to patients to ensure safe and timely discharges from hospital to their community discharge destination.
Clear communication between the Hospital Discharge Team, Discharge to Assess Hub, D2A, ICAH and the wards to ensure safe timely discharge of all patients .
Safe, compassionate and person centred care to be given to support patients.
Excellent communication skills with patients, relatives and colleagues
Main duties of the job
Main Duties and Responsibilities
- To support the Discharge Team in facilitating timely discharges for complex patients
- Provide safe, compassionate and person centred care and support to patients during their transfer from hospital to their community discharge destination
- Home visits and lone working without direct supervision
- Transfer patients to the discharge lounge if required prior to supporting on community transport
- Workload allocated by a registered professional
- Flexible working pattern to meet the needs of the service
- Excellent communication skills with patients, relatives and colleagues
- Have an understanding of services available within the community
- Maintain excellent written patient records, demonstrating patient confidentiality in accordance with Caldicott guidelines
- Maintain close liaison with colleagues and to communicate appropriately with other departments / services as required. Ensuring effective feedback regarding patient conditions to appropriate health care professionals
- Provide support and mobility including personal care, if needed

Job summary
Specialist Occupational Therapist Rotational - Fixed Terms: 12 months, 37.5 hours
Stockport NHS Foundation Trust is looking for an enthusiastic, innovative, experienced Occupational Therapist to join the Band 6 rotation for a 12 month fixed term or secondment opportunity commencing in November 2026. The successful candidate will work in two of the clinical rotations on offer (acute wards, outpatient settings, community neuro and Discharge to Assess.) Rotations are 9 month s in duration.
Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and to GP's across Manchester, Tameside, East Cheshire and Derbyshire and have access to a suitable vehicle for business purposes in order to provide time critical visits. Where possible, adjustments can be considered in accordance with the Equality Act.
Main duties of the job
The post holder will work as part of a committed team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dietitians, and Therapy Assistants delivering a service at Stepping Hill Hospital and in the Stockport community.
The post holder will provide 12 month vacancy cover within two of the specialist areas on a 9-monthly basis within Stepping Hill Hospital and other Stockport Foundation Trust sites. They will participate in CPD, clinical audit and in-service training.
They will be an autonomous practitioner, both professionally and legally accountable for all aspects of their professional and clinical work.
They will assess, diagnose, plan and deliver treatment programmes to patients in their care.
Excellent communication, organisational and team working skills are required as well as the ability to work autonomously and under pressure.
There will be a responsibility to ensure the service demand is met with adequate cover via a 7 day rota, covering weekends and bank holidays.
You will have the opportunity to support and develop Band 5 therapists within the team and act as a clinical educator to students. Undertake evidence-based audit and research projects to further their own and team's clinical practice.
They will make recommendations to clinical team leader for changes to practice. They may lead the implementation of specific changes to practice or contribute to service guidelines.

Porter - Community for D2a/TOCT/TOCH: Fixed term - 30 hours
To provide safe, compassionate, and person-centred care and support to patients during their transfer from hospital to their community discharge destination. The post holder will be responsible for driving Trust transport vehicles and safely transporting patients to their discharge destination. They will assist with patient mobility, comfort, and wellbeing throughout the journey, ensuring patients are transported safely, with dignity and respect. The post holder will work closely with clinical teams, patients, families, and community services to support timely and effective hospital discharge, ensuring appropriate handovers and adhering to NHS policies, procedures, and safeguarding requirements.

Main duties of the job
- Transport patients anywhere in hospital using wheelchairs, trolleys and/or beds
- Use the Trust IT / electronic systems
- Change oxygen and other gas cylinders following training
- Move mattresses and beds, complying with Trust manual handling techniques
- Move dirty laundry and replace with clean linen
- Collect/deliver drugs and samples to/from pharmacy/wards/departments

The Community COPD Service are looking for a Respiratory Physiotherapist to join our Pulmonary Rehabilitation Service/Team. Main duties of the job
Be clinically responsible for a designated caseload of COPD/chronic lung condition and heart failure (HF) patients undergoing rehabilitation, ensuring high standards of care.
Organise workload effectively, and efficiently with regard to clinical priorities and use of time, and in accordance with service demand/requirements on a changing basis.
Provide advanced physiotherapeutic assessment of patients with diverse presentations and complex physical, psychological and emotional conditions.
Use advanced assessment techniques, interpretation of diagnostic test results and use advanced clinical reasoning skills, to confirm a patient's diagnosis and formulate an evidence based, goal orientated treatment plan.
Use of relevant and appropriate screening tools.
Review medical therapy, supported by Respiratory ACP, Non-Medical Prescribers (NMPs) and Lead Physiotherapist to ensure safe and effective treatments/management.

This role will work across two NHS organisations - Tameside & Glossop Integrated Care NHS Foundation Trust and Stockport NHS Foundation Trust. Knowledge and Library Services (KLS) facilitate access to the most up-to-date and relevant healthcare information. This enables high quality, evidence-based decision-making in patient care, personal development for our colleagues and research innovation activity across our trusts. This role is based in the library at each hospital site and involves all aspects of working on a library enquiry desk.
Main duties of the job
The post holder will be the first point of contact for library users, both in-person and online. You will work with a small, highly motivated KLS team to provide exceptional customer service to NHS colleagues ensuring they can access the information they need. You will play a key role in promoting the library service and resources, raising awareness within the trusts and encouraging their use, with the end goal of improving patient care. We are seeking a professional, motivated, flexible person to join our team. The successful applicant will be expected to be computer literate, have effective communication skills and maintain strict confidentiality.
The role will require a range of duties, both administrative and reception, to assist the Dr's, Nurses and Support staff within our busy Neonatal Ward, helping maintain the smooth running of the ward.

Main duties of the job

The Cellular Pathology Department has a vacancy for a Cancer Pathway Tracker/Admin Medical Laboratory Assistant to work full time hours supporting the Biomedical Scientists and Consultant Histopathologists.
The post-holder will monitor cancer pathway samples and facilitate their progress throughout the Cellular pathology department to ensure timely and efficient turnaround, liaising with Cancer services, Biomedical Scientists and Consultant Histopathologists. As this is a combined role the time will be split between this duty and an admin MLA role, whose main responsibilities include, archiving slides and blocks, answering phone calls and emails, preparing MDT's and general administrative tasks.You must possess good organisational skills and be able to work as part of a team as well as under your own initiative. The work is interesting and varied and the emphasis is on accuracy and attention to detail. Full training will be given.

- To update Cancer Services Pathology Escalation spreadsheet as appropriate, ensuring all relevant information is available for the PTL meetings.
- General clerical duties associated with the work of the Laboratory, including filing, telephone answering, photocopying etc.
- To ensure Pathology diagnostic tests have been correctly identified as "Cancer Pathway" and prioritised accordingly.
- To work closely with cancer services and Cellular Pathology technical and clinical team.
- To ensure office policy is adhered to, i.e. telephones answered promptly, queries dealt with promptly and being courteous at all times to all members of staff.

Quality Control North West is part of Stockport Foundation Trust and is an NHS regional specialty department responsible for providing pharmaceutical quality control services to NHS and non-NHS clients. The laboratories at Stockport arewell equipped and advanced in analytical instrumentation and are accredited to ISO 17025 by UKAS.
Main duties of the job
- To work at all times in accordance with the requirements of the Quality Manual, Good Laboratory Practice and UKAS. All testing must be performed in accordance with established procedures to assess compliance with approved specifications.
- To perform routine chemical testing on raw materials, in-process samples, finished pharmaceutical products and environmental samples. Testing would include HPLC, GC, pH, Titration, Dissolution, Polarimetry, Karl Fischer.
- To perform quantitative and semi-quantitative analysis of pharmaceutical clean down wipes from a range of pharmaceutical and environmental origins using the LC/MS .
- To perform method validation work, and prepare validation reports for all analytical techniques as directed by senior staff and laboratory procedures.

Main duties of the job
- Provide senior clinical decision-making on a general medicine ward designated for patients who are not ready for immediate discharge or transfer, including conducting consultant ward and board rounds.

The Clinical Decision unit (CDU) , is a fast paced acute assessment and short -stay unit comprising 8 inpatients beds and 15 -space Fit to sit(F2S) area.The unit works closely with the emergency department (ED) to provide rapid assessment, treatment , observation and timely discharge or admission for patients requiring short term management
We are seeking a highly motivated , experienced ,compassionate and professional band 5 Registered Nurse to join our dynamic Clinical Decision Unit (CDU).The successful candidate will deliver high quality , patient -centered care to adults with a wide range of acute medical and surgical conditions requiring rapid assessment , treatment , observation and timely discharge and admission.
Working within a fast -paced environment, the band 5 staff nurse will be responsible for assessing , planning , implementing and evaluating evidence -based nursing care while workings collaboratively with the multidisciplinary team to ensure safe , effective and efficient patient flow.
The post holder will demonstrate excellent clinical judgment ,effective communication skills and the ability to prioritise care for patients with complex and changing needs.They will contribute to maintaining high standard of patient safety , clinical governance and infection prevention while promoting dignity, respect and compassionate care .
Main duties of the job
The post holder will be responsible for effectively delivering skilled nursing care to patients using a systemic approach of assessing planning, implementing and evaluating to ensure a high standard of care. The post holder will assist in the admission and discharge processes for patients in line with local procedures.
They will provide support with the effective patient admission pathways and effective discharge planning using their clinical skills to agree admissions and manage/report any impact on shifts/ workload and staffing levels.
They will proactively engage with patients, relatives and carers to seek their views, gain agreement and keep them informed about their treatment and care. They will demonstrate compassion by recognising and responding to individual needs in a warm, thoughtful, sensitive and helpful way.
Maintain registration, and nursing competencies, and comply with NMC code of Professional Conduct and related documents.
To be flexible to work within other areas or departments to meet the demands of the trust workforce model and changing service needs.

An exciting opportunity has arisen for a motivated, dynamic and committed individual to join our Emergency Department educational team a Band 6 Clinical Practice Educator.
If you are an experienced Emergency Nurse who is passionate about training and education and wants to make a difference in the development and support of our clinical workforce this is the post for you. You will work as part of the ED educational team alongside the Lead Clinical Practice Facilitator. This will include supporting staff to meet the RNC ECA National Curriculum and competency framework for Emergency Nursing, delivery of training sessions, clinical competencies assessments in practice and the ED MDT with other educational needs.
There are opportunities for attending courses like ALS/APLS, NTACC, AIMS and other courses to support you as the educator for the department.
Main duties of the job
- Work clinically in the Emergency department to support the junior workforce to ensure safe, effective and quality care.
- Contributing to the delivery of RCN ECA National Curriculum and competency framework for Emergency Nursing core clinical educational programmes including:clinical induction programmes, AIMS course, Triage training delivery, minor Injuries training.
- Support, coaching and assessment in the workplace for various members of the clinical workforce.
- Working alongside ED Nursing Teams and Specialists to provide tailored educational support to clinical teams as part of quality improvement and organisational change initiatives.
- Working with the Lead CPF and Matrons to formulate plans to meet identified educational needs for specific staff/groups.
- Provide training and assessment in the use of medical devices/ ANTT and Clinical competencies.
- Working alongside lead CPF to ensure departmental mandatory training figures are up to trust standard.
