The children and young people can present with acute illness in addition to requiring skilled nursing intervention to manage highly complex health and developmental needs throughout childhood and at transition to adult services.
The CYPCNS works closely with social care, education and third sector organisations to implement the statutory requirements of the Children and Families Act 2014, and we are actively involved with partners in developing children and young people's urgent care pathways. We have an opportunity for an experienced Registered Nurse to join our well-established CYPCN team as a Band 6 Children and Young People's Community Nurse. This post will be based in Chichester at the Westhampnett Centre.
This is a challenging but highly rewarding role, and the post holder will be supporting the Team Lead with caseload and local service management.
Main duties of the job
- Manage the delivery of quality nursing care, support, information and advice, for a delegated CCNT caseload, helping to ensure that appropriate resources are available to assist in providing this care.
- Support and educate staff offering clinical specialist knowledge and skills for the delivery of nursing care for children in the CCNT caseload.
- Maintain excellent communication channels with service stakeholders.
- Multi-disciplinary team working and liaison
- Help develop, implement and review clinical protocols and standard operating procedures which assist in delivering safe care and best nursing practice.
- Develop clinical skills and knowledge within the team, ensuring the relevant clinical skills required for management of safe nursing care are maintained.
- Provide teaching regarding the use of equipment and treatment regimes in the community for children, families/carers and staff involved.
- Excellent record keeping.
- Act as a resource and role model of children's community nursing specialist practice to all other professionals and work colleagues.
- Attend essential and relevant clinical practice updates in tertiary centres and special interest groups both within the Trust, locally and nationally to help develop and contribute towards establishment of clinical practice standards.

Job summary
This service is a collaboration between Sussex Community NHS Foundation Trust SCFT and the Sussex Federations: Brighton & Hove Federation, Alliance for Better Care and South Downs Health & Care. The Federations will be delivering the GP-led aspect of the Virtual Ward at a place-level to match SCFTs Urgent Community Response service across Sussex. SCFT provides the service in West Sussex, Brighton and Hove and the High Weald, Lewes and Havens area of East Sussex.
B&HF are the lead contractor for the GP Service delivery and have sub-contracted to ABC for delivery within the federations geographical areas of Mid Sussex, Worthing and Chichester.
Since 2022, SCFT have delivered Virtual Wards Hospital at Home in line with the NHS Englands national definition that a virtual ward is a safe and efficient alternative to NHS bedded care that support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
As of August 2024, SCFT re-modelled the Virtual Ward delivery and mobilised General Virtual Wards within Urgent Community Response UCR, which supports Sussex patients within its catchment area with a higher level of acuity to be safely managed at home/chosen place of residence. This could be patients on either an admission avoidance pathway or early supported discharge pathway, where the alternative would be NHS bedded care.
Main duties of the job
The Virtual Ward activity is now prioritising admission avoidance whilst still delivering early supported discharge for patients from the Acute hospitals. It is delivered 365 days per year, 8am 8pm. UCR teams within the SCFT footprint provide urgent support to help prevent unnecessary hospital admissions and ultimately keeping patients safely at the place they call home. There is a consistent service across Sussex and is made up of multi-disciplinary teams including General Practitioners, Nurses, Pharmacists, Therapists and Carers, ensuring they can meet a range of needs quickly offering the best level of support to patients.
The multi-disciplinary team that oversees and deliver care the General Virtual Ward caseload is led by both medical a General Practitioner and clinical senior decision makers. Virtual and On Call GP cover is available seven days a week, 9am to 6pm 365 days a year, including bank holidays. This cover is linked to the respective teams local UCR teams with a virtual board round each morning
The UCR Hub Based GP will work within their assigned place based SCFT UCR base. We are recruiting for a GP for the Worthing Virtual Ward. The GP will present within the allocated UCR base working face to face with the SCFT MDT delivering GVW including supervision of ACPs and team development. Patient visits may be required. Monday, Wednesday and Friday morning

Job summary
The Therapies In Schools (TIS) service is an innovative PT and OT model of service which is commissioned by West Sussex County Council and delivered by Sussex Community NHS Foundation Trust. It's aim is to provide additional therapeutic input into schools to support the teachers and carers to integrate therapeutic goals into heart of curricular activities and at home.
TIS is an additional service to the current NHS provision and aims to work flexibly to meet ever changing demands as they arise. A key part of TIS is to provide accredited training to all education staff and offers opportunities for families and carers to access the training too.
We wish to recruit a part time Band 7 Occupational Therapist with the necessary skills and experience to join our friendly and innovative TIS team, working in a Special School and College in Burgess Hill.
We offer regular in-service training, peer supervision and clinical support. There are dedicated paediatric special interest groups and the Paediatric Occupational Therapy service is supported by a Countywide Professional Lead.
The hours available are three days a week term time only. Term time only roles are paid spread across a full year to ensure your pay does not change from month to month.
Main duties of the job
This is a wonderful opportunity to gain experience of working with a diverse range of children and young people within specialist school settings.

Your role will be to provide a universal approach through delivering accredited training to school staff, some targeted work through group work and ad hoc needs, and a small caseload of intensive work to support families in crisis or needing enhanced support due to a change in circumstance.
Your role is in addition to the existing NHS provision provided by the Child Development Centre (CDC) therapists, and so working collaboratively with the CDC therapists is vital. You'll work in your main allocated school and college, do occasional home visits where needed, and may at times support your team members in other schools.
You will need to be flexible in your availability to attend occasional parents evening events, parent carer forum liaisons and host parent coffee mornings/network groups.

We are seeking a Band 6 Speech and Language Therapist to join us working across our Pre-School Complex Needs and schools teams within the Central locality.
This is an exciting opportunity to build on your Band 5 or Band 6 clinical experience and become part of the Speech and Language Therapy team based within our Child Development Centres and schools. You will manage a varied clinical caseload, carrying out assessments and reviews, delivering therapy, and providing advice, support and training to parents, carers, teachers and other professionals. The role will involve working across different clinic bases/school settings throughout the week.
As a Band 6 Speech and Language Therapist, you will be supported by experienced and approachable Clinical Leads, alongside friendly and knowledgeable colleagues within and the wider Speech and Language Therapy service. You will also benefit from a well-established supervision programme, opportunities for CPD and access to a range of learning and development opportunities to support your ongoing career progression.
