We are looking for Patient Services Advisors to join our busy and supportive team. This is a varied role where you will be the first point of contact for patients, providing excellent customer service both over the phone and in person.
As a Patient Services Advisor, you will undertake a range of administrative duties, including managing appointments, updating patient records, handling enquiries and supporting the smooth day-to-day running of our services.
We are seeking someone who:

We have both full time and part time hours available, covering 08:00 until 18:30, Monday to Friday.

Main duties of the job
This role requires courteous, calm and effective communication skills both over the phone and face to face, plus methodical, accurate administration capabilities.
You will assisting patients in accessing the appropriate services or healthcare professional and undertake a variety of administrative duties, in accordance with agreed procedures, protocols and timescales.
You will be someone who genuinely cares about delivering an excellent patient experience and who enjoys building positive relationships with patients, colleagues and healthcare professionals and understand the importance of confidentiality, teamwork and professionalism.

Job summary
Burnham, Berrow and Highbridge Medical Centres (known as Symphony North) are looking to recruit a Treatment Room Nurse or Practice Nurse with chronic disease qualifications,to join their dedicated Nursing Team on a 12-month fixed term basis, working 24 hours per week across all three sites.
The pay rate for this role ranges from £19.28 to £22.73 per hour,. depending on experience and qualifications.
You will be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.
You will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services. They will work collaboratively with the whole general practice team to meet the needs of patients and supporting the delivery of policy and procedures.
You will also demonstrate safe, clinical decision-making, assessment and expert care.
Main duties of the job
- Assist the whole practice team to help deliver effective implementation of management of patients and their needs.
- Managing the nursing care of patients with chronic disease, employing recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required
- Contribute to the development of policies / protocols / guidelines within the practice.
- Aid the practice team with implementation of new initiatives in the delivery of chronic disease management services.
- Encourage the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
- Recognise the opportunities for working in partnership with the Primary Health care team.
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

In this exciting role, you'll be responsible for delivering high quality care from first presentation through to assessment, diagnosis, treatment and follow up.
You'll manage patients with minor illness, minor injuries, and a range of acute presentations, using your advanced clinical assessment and diagnostic skills to make safe, effective decisions.
You'll be a confident critical thinker, able to independently assess and manage minor trauma and injury, while working collaboratively with our multidisciplinary teams to deliver safe, patient focused care in line with local policy and clinical best practice.

Main duties of the job
You will be an experienced ECP (prescribing Nurse or Paramedic) who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. This includes patients presenting with minor illness and minor injuries within both general practice and a Minor Injury Unit setting.
They will demonstrate safe clinical decision making and expert care, including advanced assessment and diagnostic skills, for patients within general practice and the Minor Injury Unit. The post holder will demonstrate critical thinking in the clinical decision making process and will be competent in the assessment and management of acute presentations, minor trauma and injury.
They will work collaboratively with the general practice and Minor Injury Unit teams to meet the needs of patients, supporting the delivery of policy and procedures.

Job summary
Symphony Healthcare Services are looking for a Patient Services Advisor to join the team at Warwick House Medical Centre, Taunton.
You will be required to provide administration and reception duties for the practice, dealing with patients, clinical staff and colleagues in an efficient and polite manner. You will also be required to receive, assist and direct patients in accessing the appropriate service or healthcare professional and undertake a variety of administrative duties to assist in the smooth running of the practice in accordance with agreed procedures, protocols and timescales.
This role consists of 30 hours per week, working Monday-Friday.
The pay rate for this role is £13.09 per hour.

Main duties of the job
You will also be required to:
- Identify and book appointments and home visits in line with practice protocols, including cross-organisation bookings
- Search for, register if necessary and book onto the computer system patients as they present at the desk
- Deal with enquiries, signposting and requests, either face to face or over the telephone from a variety of external sources, such as patients, relatives, clinical team, hospital, pathology, and work colleagues.
- Deal with distressed/aggressive patients safely and appropriately as they present at the desk or over the telephone
- Be able to prioritise patients in need of urgent medical attention who present to reception in person or telephone in and direct them to the correct clinical intervention/appointment
- Deal with requests from patients and GPs for emergency calls, following protocol and dialling 999 as required
- Maintain an overview of the waiting areas, informing patients of any known delays and ensuring the area remains in a neat and tidy condition

Symphony Healthcare Services are recruiting for Healthcare Assistants to be based at our Symphony North practices (Burnham on Sea, Berrow & Highbridge Medical Centres) and Burnham on Sea War Memorial Hospital (MIU) in Somerset, with the flexibility to work full or part time covering the hours of 08:00 to 20:00.

Main duties of the job

Job summary
Symphony Healthcare Services are looking for an enthusiastic Healthcare Assistant to join our friendly team at Bruton Surgery, a small country practice within Symphony Healthcare.
This is a varied role supporting the clinical team with phlebotomy, health checks, ECGs and other HCA duties according to skills and experience. We are looking for someone who is friendly, reliable, organised and committed to providing excellent patient care.
This is a PCN-funded post, primarily based at Bruton Surgery, with some flexibility required to provide clinics at Wincanton Health Centre and Wincanton Community Hospital as part of the PCN Community Investigation Hub.
Previous healthcare experience is desirable, with training available to support development in the role.
This vacancy is a part time permanent contract, offering 21 hours per week.
The pay rate for this role is £13.50 per hour.

Symphony Healthcare Services are looking for an enthusiastic and compassionate Nature-Based Health and Wellbeing Coach to join West Somerset Primary Care Network and lead the delivery of the Healthy Exmoor programme, a partnership with Exmoor National Park Authority.
This is a unique opportunity to combine person-centered health and wellbeing coaching with the natural environment to support health. You will provide personalised 1:1 coaching and facilitate group opportunities, supporting people to identify what matters to them, build confidence and self-management skills, and make positive and sustainable changes to their health and wellbeing.
This job role is a fixed term contract of 18 months, with the hours varying between 30-37.5 hours per week.
The pay rate is £13.27 per hour.

Main duties of the job
Working across Exmoor National Park and the wider West Somerset community, you will help people to connect or reconnect with nature and their local communities in ways that support physical health, mental wellbeing, social connection, confidence and independence. You will work creatively with existing opportunities and partners, while developing new nature-based approaches that respond to the needs and interests of local people.
You will work with a diverse range of people and communities, including those who may experience barriers to good health, social isolation or reduced confidence. Job summary
Symphony Healthcare Services are looking to recruit aPatient Services Advisor(phone and administration based duties) for Creech Medical Centre in Creech St Michael near Taunton in Somerset.

Job summary
Burnham, Berrow and Highbridge Medical Centres (known as Symphony North) are looking to recruit a Treatment Room Nurse to join their dedicated Nursing Team on a 12-month fixed term basis, working 24 hours per week across all three sites.
The pay scheme for this role is £21.26 - £24.11 DOE.
You will be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.
You will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services. They will work collaboratively with the whole general practice team to meet the needs of patients and supporting the delivery of policy and procedures.
You will also demonstrate safe, clinical decision-making, assessment and expert care.
Main duties of the job
- Assist the whole practice team to help deliver effective implementation of management of patients and their needs.
- Managing the nursing care of patients with chronic disease, employing recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required
- Contribute to the development of policies / protocols / guidelines within the practice.
- Aid the practice team with implementation of new initiatives in the delivery of chronic disease management services.
- Encourage the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
- Recognise the opportunities for working in partnership with the Primary Health care team.
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
