Job Purpose
To provide high-quality nursing care to patients registered with the GP practice, working as part of the multidisciplinary primary care team. The Practice Nurse will assess patients, deliver treatment and preventative healthcare, support long-term condition management, and promote health and wellbeing in accordance with NHS standards and professional requirements.
Key Responsibilities
- Conduct patient assessments, including clinical observations and nursing reviews.
- Provide wound care, dressings, suture and clip removal where appropriately trained.
- Administer vaccinations and injections in accordance with national guidance.
- Carry out NHS health checks and health promotion activities.
- Support the management and monitoring of long-term conditions such as: Diabetes Asthma and COPD Hypertension Cardiovascular disease Chronic kidney disease

- Take blood samples and other specimens where trained.

Main duties of the job
Book in, amend and cancel patient appointment inline with practice appointments procedures ensuing optimum efficiency of the appointment system.
Respond and/or redirect all patient and visitor requests accordingly.Set-up of new patients onto the computer system.Produce repeat prescriptions according to practice procedures and ensure timely distribution of completed prescriptions.Ensure correspondence, reports, results etc. are filed electronically in correct patient record.Answering incoming telephone calls, ensuring calls are documented and redirected accordingly.
