Job summary
Reporting to the Associate Director of Property PMO you will provide expert specialist and professional construction and estates project advicse to property and estates staff, directors and senior management of the Trust. You will manage a portfolio of projects as well as other estates project staff. Estates projects range from new build and refurbishment capital projects to moves and business transformation support.
Main duties of the job
Suitable for someone who is already in the construction project management industry. With a background in a field such as design, engineering or construction management ideally in healthcare. You will have good all-around technical knowledge of construction and engineering, a sound knowledge of minor and intermediate forms of contract, knowledge of the financial management of capital projects. You will have recent and in-depth knowledge and understanding of developing and interpreting programme planning tools. You will need to be a confident communicator who is able to influence, inspire and lead others from different backgrounds and all levels of seniority. This role will suit an individual who has the drive, enthusiasm, and ability to motivate individuals to deliver exceptional outcomes in a constantly evolving environment.

Main duties of the job
As a Non-Medical Prescriber (NMP), you will be involved with opiate substitute treatment (OST) interventions, benzodiazepine reductions, alcohol and opiate detoxifications and relapse prevention medications. You will also provide support for health care assessments, administering detoxification medication for clients, administering pabrinex , completing alcohol and opiate detoxification assessments, providing Blood Borne Virus (BBV) screening and vaccinations and will work closely with mental health, physical health and social care services. Youll also help to create a collaborative approach that links Turning Point ever closer to outside professionals and agencies.
You will attend a local prescribers meeting, one to one clinical supervision and national prescriber/clinician meetings which will also support your learning and continuous professional development. Full training and supervision is provided from our Clinical Services Manager and Clinical Lead. You will have a real opportunity to help shape service delivery and contribute to delivering clinically excellent services.
As an employee you will have access to RCNi to assist with revalidation, indemnity insurance and support from the Clinical Lead, Clinical Services Manager, Turning Points Head of Nursing and Head of Medicines Optimisation and Pharmacy Services.

Job summary
Swaffham and Downham PCN is looking for a Healthcare Support Worker to join our nursing team, working flexibly across our member practices to support patients and clinical colleagues throughout the network.
This is a role we welcome applications for at two levels: as a first step into a healthcare career for someone with no prior clinical experience, or as a more established role for a candidate who already holds relevant Healthcare Assistant or Support Worker experience.
The post-holder will work at Support Worker Level Practice as described in the Primary Care and General Nursing Career and Core Capabilities Framework, delivering care and tasks delegated by a registered professional, and will operate under indirect supervision once competent, with direct supervision provided when learning new skills.
Main duties of the job
Deliver opportunistic and planned health promotion advice on diet, physical activity, alcohol and weight management, escalating to a registered clinician where specialist advice is needed.

Support proactive care initiatives, including virtual and in-person care home rounds and case-finding activities, under supervision.

5. Delegated Clinical Care
Deliver basic wound care, including simple wound cleansing, dressing changes and skin care, strictly in line with a care plan set by a registered professional.
Support continence and personal care tasks as directed by a registered clinician, where required.

Medicines Optimisation Pharmacy Technician - Discharge to Assess Pathways / Community Services
Midlands Partnership University NHS Foundation Trust is unique in combining physical and mental health, learning disability, community and adult social care services in a single Trust across Staffordshire, Stoke-on-Trent and Shropshire supporting the provision of a truly integrated care framework across health and social care services.
This is an unique and exciting opportunity to lead on the development and delivery of an Integrated Home First Medicines Optimisation Service to improve safe transfer and use of medicines across clinical interfaces; to meet all legal requirements and the Good Practice Standards prescribed by the Care Quality Commission (CQC) and other relevant agencies; to support safe administration of medicines in community settings; to meet the recommendations published by NICE NG 67 "Managing medicines for adults receiving social care in the community."
Please note, this isalso a developmental post that provides a structured training for a Pre-Registration Trainee Pharmacy Technician (PTPT) transitioning from Band 4 to Band 5 upon completion of a competency development plan. The Pre-Registration Trainee Pharmacy Technician post holder will need to qualify and register with the General Pharmaceutical Council by December 2026.
Main duties of the job
The working hours are 37.5 hours per week Monday to Friday with limited late shift duties and Bank Holiday commitments. This provides a fantastic work life balance whilst being a valued member of the Pharmacy Medicines Optimisation Team. The post holder may be required to work flexibly to meet the needs of the service.
- You will work as part of a multi-disciplinary team within the North and South Home First Hubs and support the Lead Pharmacist in the establishment and management of care pathways for the referral of patients referred into the Home First Service with medicine-related problems for assessment, support and follow-up by a specialist pharmacy team, working across health and social care interfaces.
- This role will involve cross sector working in the community with multiple agencies and Primary Care Networks (PCNs) to support with complex medication reviews for patients in their own homes.
- This role will be extremely rewarding, and requires a motivated, dynamic individual who is experienced in working with a range of stakeholders and has a track record for service development and delivering to agreed targets.
- You will be a GPhC registered pharmacy technician (or being registered by December 2026) with excellent interpersonal and IT skills. This is a permanent full-time post although part- time hours or a job share will be considered.

The Clinical Administrator will provide a confidential and efficient administrative support to the teams within Universal services, providing reception cover when required. Inclusion Drug and Alcohol Services are delighted to be supporting a new full-time trainee Mental Health and Wellbeing Practitioner role, with the required training course planned to start on 5th January 2027.
Over the 12-month training period individuals will attend study at University of Sussex (in person and remote learning) as well as spending time working clinically in a Drug and Alcohol service. These are exciting roles created by NHSEWTE to support the transformation of community mental health and drug and alcohol services. Your training will be fully funded, and you will be eligible for a full-time qualified permanent position on completion of the training course. In return, we ask that you commit to staying with us for at least two years after you qualify.
The successful candidate will be required to work across our sites in the North of Hampshire (including Aldershot, Basingstoke and Andover)

Job summary
The enhanced care team offer additional support across all the wards and departments for patients who may require additional support due to, unfamiliar surroundings, medication, mental health issues or challenging behavioural needs.
Enhanced care is an integral part of a therapeutic plan and ensures the safe and sensitive monitoring of a patients physical and psychological wellbeing, whilst building therapeutic relationships. Enhanced care could be supporting 'cohorted' or patients who require 1-1 support.

Main duties of the job
They post holder be responsible for the effective and efficient operational management of the enhanced care team. The enhanced care team leader will be responsible for working within the set budget, clinical governance, and staff development and training.The post holder will be expected to provide professional leadership within the clinical area and provide direction, and inspirational leadership to all members of the multi-disciplinary team working with the enhanced care team. The post holder will be involved in the development of protocols / guidelines and patient information.
They will work collaboratively with ward teams and the Liaison Psychiatry Service to ensure patients receive effective, high quality clinical support to enhance mental health care, in accordance with the principles of least restriction.
The post holder will be responsible for ensuring that there are systems and processes for the planning, provision and review of enhanced care. The post holder will ensure that the enhanced care team deliver therapeutic engagement and interventions to patients with mental health conditions, cognitive impairment or those experiencing psychological distress.
They will provide regular reports on the effectiveness of the team in delivering quality care by reducing restrictive practices and incidents of harm.

This vacancy is in addition to the current complement of seven Partners, three Salaried GPs, supported by an experienced nursing team of three Nurse Practitioners, one Advanced Paramedic, five Practice Nurses and two Health Care Assistants. We are looking for a highly skilled Practice Nurse will help carry out Treatment Room duties, chronic disease care, immunisations, health promotion and family planning. They will work as part of the primary healthcare team providing Personal Medical Services to the Practices patients, using Systmone.
We welcome applications from nurses with a broad practical experience in the management of long-term conditions including COPD, asthma, diabetes etc. in a primary care setting.

We are excited to share an opportunity for a Band 7 Clinical Team Lead.
The role involves working in our hub located in Frimley and providing a crisis and home treatment service for all emergency, urgent and routine referrals in the Surrey Heath and Northeast Hants area. We are looking to recruit a Band 7 Clinical Team Lead to be part of and lead the service, providing senior clinical and managerial leadership. You will be accountable for the day-to-day management of the service, overseeing referrals, assessments, allocations and discharges to ensure a high standard of patient care.
Working closely with the Service Manager, you will drive service governance through audit, quality-improvement initiatives and performance-data audit, and ensure compliance with CQC standards.
You will support the management of operational functions such as incident investigations, complaints and escalations. You will provide supervision, appraisals and professional development for the team, and oversee recruitment, induction and training.
You will collaborate with inpatient wards, community mental health teams, primary care, social services and voluntary partners to ensure seamless care pathways and drive service improvement.
This is an integrated role to support the triage and/or rapid response as well as the Home Treatment elements of the service and the post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach.

We have an exciting opportunity for a General Adult Consultant Psychiatrist to join our West Sussex Division. The post is based at New Park House in Horsham, West Sussex which forms part of our newly formed Horsham Neighbourhood Mental Health team (NMHT).
West Sussex Community services are divided into two main areas: North West Sussex and Coastal Sussex. North West Sussex has three NMHTs located in Crawley, Horsham and Mid Sussex. Each NMHT covers a geographical area aligned with GP surgeries to facilitate liaison with Primary Care colleagues. There are two other consultant posts in Horsham NMHT.

Main duties of the job

Promoting health and independence for people who use our services and their carers/support staff by delivering high quality psychology assessments, formulation and interventions; providing consultation, supervision and guidance to band 7s, trainees, assistant psychologists and others as required.

Job summary
We are delighted to introduce psychology across our all-age Home Treatment Teams as part of the service delivery model designed to enhance community-based crisis care and improve outcomes for people over 18 years who are experiencing a mental health crisis.
This is a unique opportunity to shape and develop psychology provision within a dynamic multidisciplinary team, bringing a psychological perspective to crisis intervention, recovery-focused care, and admission avoidance pathways. As a newly established role, the successful candidate will have the opportunity to influence service development, strengthen psychological leadership within the team, and contribute to the future direction of home treatment services.
As the Senior Psychologist within the team, you will be providing expert psychological assessment, formulation, and interventions for people who are experiencing an acute mental health crisis and their families. Your psychologically informed interventions will support recovery, resilience, and wellbeing, helping individuals to better understand and manage their difficulties. You will work collaboratively within a multidisciplinary team to support people to remain at home during a crisis and facilitate timely discharges from hospital where appropriate.
