Job summary
PHL Group are looking for a Forensic Healthcare Professional working in custody suite covering Durham working alongside police officers to assess and treat individuals detained in Police Custody. Full training will be provided.
We are currently looking FHCPs to work on a Full Timebasis covering 42hrs per week.
PLEASE NOTE YOU MUST BE A REGISTERED RGN/GENERAL NURSE OR PARAMEDIC
Could you identify if a detainee is suffering from a mental health condition? If they are uncooperative or lacking diabetes medication? If they are a substance misuser or suffering from a head injury? Our Forensic HCPS are responsible for assessing and addressing the health needs of detained persons, as well as conducting forensic medical examinations gathering evidence for use in the criminal justice system.
Main duties of the job
You will be conducting clinical assessments, identifying, and implementing appropriate interventions and collecting forensic samples. Providing advice and guidance to ensure appropriate care is maintained and keeping detailed and accurate records to ensure the health, safety and welfare of detained persons held in police custody.

Forensic Healthcare Professional Skills and Experience:
- Effectively deliver the requirements of the forensic practitioner service following the Codes of Practice of the Police and Criminal Evidence Act 1984 and locally agreed protocols.
- Keep accurate, complete and contemporaneous notes of work carried out, including the recording of injuries.
- Provide advice and support to Custody Officers in determining the health care needs of detainees and the priorities of care. Seek telephone advice and refer cases to the Consultant FME when required.
- Provide a professional, responsive and caring service to detained persons in the custody environment to support the Custody Sergeant and Officers in assessing the healthcare needs of detained persons and an effective care pathway throughout their time in custody.
- Determine an individuals fitness to be detained, fitness to be released, fitness to be interviewed and competence to understand charge.
- Assessment of alcohol and drug intoxication and withdrawal.

PHL Group are looking for a Forensic Healthcare Professional working in custody suite covering Cleveland working alongside police officers to assess and treat individuals detained in Police Custody. Full training will be provided.

Our Forensic HCPS are responsible for assessing and addressing the health needs of detained persons, as well as conducting forensic medical examinations gathering evidence for use in the criminal justice system.

You will be conducting clinical assessments, identifying, and implementing appropriate interventions and collecting forensic samples. Providing advice and guidance to ensure appropriate care is maintained and keeping detailed and accurate records to ensure the health, safety and welfare of detained persons held in police custody.

To work within an occupational therapy mental health pathway aimed at supporting people to achieve their recovery goals/ functional stability
To carry out assessments and interventions, utilising the OT process, from initial assessment to discharge planning and evaluation. Develop clinical formulation and apply clinical reasoning.
To undertake risk assessments; document, report and act on identified concerns.
Treat, monitor, develop each individuals OT programme, measure outcomes and discharge appropriately.
To use MOHO as a core framework for practice (including MOHO based standardised assessments and formulations for pre-post therapy outcome measures and patient-centred goal planning)
Educate, guide and supervise patient as individuals or in groups.
Educate carers on the support and management of the patients needs.
Negotiate, agree expectations and set appropriate, realistic and individualised therapy goals in partnership with the patients, carer and colleagues.
Participate in multidisciplinary case review meetings to discuss progress and to plan treatment and safe discharge.

This is an exciting opportunity to become a member of the Welsh Ambulance Service Non-Emergency Patient Transport team delivering non-emergency patient transport at locations in the Powys health board area.

We are looking for candidates that can deliver a high standard of professionalism and compassion when providing a transport service to meet the needs of patients and other service users. Main duties of the job
The post holder will be required to drive an equipped NEPTS ambulance or other vehicle as necessary, and carry out non-urgent and non-critical, planned patient journeys usually working with a colleague but sometimes working alone when dealing with patients who can walk unaided or with minimum assistance.
The post holder deals with a variety of patients and conditions, most of whom will require different levels of assistance, lifting or carrying to transfer to and from the Trust vehicle.
The post holder transports patients safely between their place of residence and designated treatment centres and clinics, provides where necessary appropriate care and assistance which is within the post holder's level of training and competence, and adheres to the Trust Code of Conduct and Standards of Professional behaviour, Service Instructions and the Trust's Standing Procedures and Policies.

Brockington Parent and Baby Unit are recruiting 2 x Band 3 mental health support workers.In the role you will work on a Perinatal Mental Health Inpatient unit, delivering a high-quality clinical service to patients and their families. You will be working within a specialist multi-disciplinary team and will have access to regular supervision.
You will be delivering supportive interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties in the Perinatal period.
You will work in conjunction with the multi-disciplinary team and will provide interventions to support the person to meet their agreed goals and coordinate and / or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

The Patient Flow Coordinator will work as part of the Integrated Complex Assessment Team (ICAT), supporting the effective and timely flow of patients through Discharge to Assess (D2A) and community hospital pathways.
The postholder will work under the guidance of the Senior Patient Flow Coordinator and Patient Flow Team Lead and will work collaboratively with Discharge Facilitators, clinical teams, social care, patients, families and wider health and social care partners.

We have an exciting opportunity for a General Adult Consultant Psychiatrist to join our West Sussex Division. The post is based at Ifield Drive in Crawley, West Sussex which forms part of our Crawley Neighbourhood Mental Health team (NMHT).
West Sussex Community services are divided into two main areas: North West Sussex and Coastal Sussex. North West Sussex has three NMHTs located in Crawley, Horsham and Mid Sussex. Each NMHT covers a geographical area aligned with GP surgeries to facilitate liaison with Primary Care colleagues.

Main duties of the job
We are a large multi-disciplinary community mental health team, which provides specialist assessments and treatment to patients presenting with a range of complex and unstable mental health and social needs.
As a consultant psychiatrist in our team, your main duties will be to:
- Provide specialist medical /psychiatric assessment and evidence-based treatment to patients referred to the service
- Provide clinical supervision to trainees and informal advice and supervision to non-medical members of the team
- Fulfil the duties of Responsible Clinician for patient's subject to Community Treatment Orders including attendance at Managers Hearings and Tribunals.

Job summary
As a Consultant Psychiatrist you will join the Thanet Community Mental Health Team which has a dedicated Duty team, assessment and CAPA assessment team. There is a robust system of medical support to the team and to ensure medical leadership.
The Trust regards this as an opportune moment to develop the functioning of the team. The service covers the eastern area of the town, an area of particular social deprivation with considerable drug and alcohol-related difficulties in the local population. You will carry no responsibility for inpatients.
You will learn to interact with other specialties such as the Mother and Infant Mental Health team and Neuropsychiatry.
Successful applicants will ideally have MRCPsych or equivalent and be on, or within six months of inclusion on, the Specialist Register. Main duties of the job
- To conduct assessment and treatment of mental health conditions who are under the secondary mental health services.

- To participate in service and business planning activity for the locality and, as appropriate, for the whole mental health.

We require highly skilled and motivated practitioners to work as part of a dynamic multi-disciplinary team providing high quality, specialist evidence-based treatment to Veterans. The post holder will be innovative, enthusiastic and flexible and able to work both autonomously and as part of the team.
Key elements of the roles will include;
- delivering CBT on a one to one and group basis including intensive trauma focused CBT
- contributing psychological expertise as part of a multi-disciplinary team

Job summary
You willbe a core member of the multi-disciplinary/multi-agency Hope Service, participating in the response and provision of crisis/urgent assessments, care planning, risk assessing and developing a risk management plan and treatment plan of Children and Young People (CYP) requiring support for emotional, mental health and social care needs. Often this response will be to facilitate a diversion from Psychiatric Hospital admission or an out of area placement and allow for recovery as the CYP remains in their home/placement/community/school. There is also a significant element of Care Coordination for CYP known to the service for an indefinite period of time.
This position would suit an individual who is passionate about delivering high-quality, evidence-based interventions and achieving positive outcomes for CYP experiencing moderate to severe mental health difficulties.
Applicants with a suitable level of knowledge, skills, and experience may be appointed directly at Band 7. Alternatively, the post is available as a Band 6 developmental role, with a structured development pathway to Band 7 within 12 to 24 months, subject to the achievement of agreed competencies and successful performance against the developmental framework.

Job summary
The post holder will be based within the Learning Disability and Forensic directorate. Working closely with the Learning disability Service Manager to review the current business activities within the service. The post holder will also be the lead in promoting the service, identifying good practice, and working closely with the team to implement any recommendations. The post holder will have direct line management responsibilities for the agreed service and robust management of the team. The community team manager will assist and oversee the day-to-day operational responsibilities for the clinical services within the community team and the wider multi-disciplinary team.
Main duties of the job
To contribute to the development of the community health team services.
To lead and embed the principles of transforming care across the learning disability service.
To provide support to the service manager to meet the aims and objectives of the service including deputising and delegated responsibilities
To provide support to the service manager in the effective day to day management of the team within the resources available and agreed governance frameworks.
To review staffing levels and skill mix as necessary.
To be responsible for the leadership, mentorship, and support of staff daily.
To manage and provide management / clinical supervision to staff under your line management.
Work within the infection control agenda to improve standards.
Improving environments for patients.
To work with patients and their carer/relative in addressing concerns by building closer relationships.
To promote partnership working with patients, carers, and stakeholders
