We are looking for a compassionate, dedicated, and proactive Senior Support Worker to join our friendly team within a specialist social care home (without nursing), supporting seven gentlemen aged 60-76 who have severe learning disabilities, autism, and other complex needs.
This is a fantastic opportunity to play a key role in helping people live meaningful, fulfilling lives while progressing your career within a supportive and person-centred environment.

You will support them both within their home and out in the community, helping them to live life to the fullest while ensuring their safety, comfort, and wellbeing.

Main duties of the job
- Assist individuals with eating and drinking.
- Support personal hygiene needs and all aspects of personal care, such as bathing/ showeringand support with continence needs.
- Support physical care needs which may be due to a range of acute or chronic difficulties andincludes manual handling using equipment provided.
- Involve and engage individuals in all aspects of their care.
- Ensure that the dignity and respect of individuals are maintained at all times.
- Act as Shift Leader and delegate responsibilities to colleagues at handover in line withpeople's needs and preferences, ensuring the shift planner is updated.
- Support people with their medication.
- Contribute to, and record, health checks, risk assessments and care plans, using physicalrecords and/or digital tools (depending on registered home).

We are now recruiting Forensic Custody Healthcare Professionals (RGNs and Paramedics) to deliver specialist healthcare within police custody.
This is a rare opportunity to step into a respected forensic role where no two shifts are the same, no custody experience is required as full accredited training is provided. Your clinical decisions will directly support detainee welfare, safeguarding, and the wider criminal justice process.

Main duties of the job
As a Forensic Custody Healthcare Professional, you will work autonomously within police custody suites, assessing and caring for detainees with a wide range of physical health, mental health, substance misuse and vulnerability-related needs.
This is a specialist clinical role where no two shifts are the same.
You will assess fitness for detention, interview, charge and release; respond to acute medical episodes; provide healthcare advice; support safeguarding decisions; and work closely with custody teams, police officers and partner agencies.

Job summary
The Physician Associate or Physician Assistant (PA) will be accountable for ensuring that high quality patient care is always delivered, where they can be a source of continuity for service users by monitoring physical as well as mental health requirements.

Job summary
This is a fantastic opportunity for a Data Manager / Research Assistant Psychologist for a Wellcome funded research study, SloMo2. SloMo is a digitally supported, cognitive-behavioural therapy for psychosis that improves fear of harm from others (or paranoia). The SloMo2 study is investigating if SloMo therapy can be delivered by real world clinicians, by conducting a clinical investigation of its implementation, effectiveness, and cost-effectiveness in three NHS Trusts.
The postholder will join a friendly study team that is led Dr Amy Hardy. The post will be supervised by Dr Katie Taylor-Ryan, Study Coordinator and Dr Sarah Swan, Therapy Lead. The postholder will work alongside a lived experience researcher, and liaise with routine care psychological practitioners to facilitate data collection, promote uptake, and for provision of technical support.
Main duties of the job
- To provide data management and reporting support for the SloMo2 study, ensuring data integrity and completeness.
- To support research delivery in line with Good Clinical Practice through quantitative and qualitative data collection, safety monitoring, and Patient and Public Involvement.
- To support SloMo as a therapy service through provision of technical support, liaison with therapists, and assistance to the Therapy Lead.
- To contribute to team functioning and project management through effective communication, organisation, and use of supervision.

Job summary
Exmoor Surgery is looking for an enthusiastic and experienced Clinical Pharmacist to join our friendly and supportive multidisciplinary primary care team for 2 to 3 days per week. Ideally, the working days will be Thursday and Friday, although there is some flexibility, including the option to work from home on a Friday.
The Clinical Pharmacist will support the delivery of safe, effective and cost efficient medicines management within the practice, working closely with GPs, nurses, our existing Clinical Pharmacists and the wider practice team.
The usual working day is 9:00 a.m. to 5:00 p.m., with a balanced mixture of telephone consultations, clinical reviews and protected administrative time.
The successful candidate should be organised, clinically confident and comfortable working both independently and collaboratively. Previous experience in general practice or primary care would be an advantage, and an Independent Prescribing qualification is desirable.
Experience of working with SystmOne would be beneficial.
This is an excellent opportunity for a Clinical Pharmacist to become an integral part of a supportive multidisciplinary team and contribute to the delivery of high quality patient care and medicines optimisation within Exmoor Surgery.
Main duties of the job
The role will include:
- Conducting repeat medication and structured medication reviews
- Managing acute medication requests and prescribing queries Undertaking planned care reviews, including cardiovascular, renal and metabolic and diabetes reviews
- Reviewing hospital discharge letters and implementing medication changes
- Monitoring high risk medicines, including DMARDs
- Supporting safe and effective prescribing and medicines optimisation Providing clinical advice and support to patients and colleagues
- Working closely with our two existing Clinical Pharmacists, GPs, nurses and the wider practice team
- Contributing to the ongoing development and objectives of the practice

A staff nurse position is available to join our Hospital at Home/Virtual Ward team within Central Lancashire. This service provides patients with safe, hospital-level care in their own homes or place of residence, helping to prevent unnecessary hospital admissions and supporting earlier discharges where appropriate.
Working in collaboration with Lancashire Teaching Hospitals and Lancashire and South Cumbria Foundation Trust, the service takes a multidisciplinary approach. Main duties of the job

Midlands Partnership NHS Foundation Trust has an opportunity to join our West Older Adult Community Mental Health Team - based at Park House, Cannock. The post is full time Monday to Friday.
The post holder will be joining 12 other psychological professionals within the team, including qualified psychologists, CAPs, a Trainee CBT therapist and Assistant Psychologists. There is regular clinical supervision, therapy specific supervision including CFT, EMDR. and a commitment to training. The post holder would also have the opportunity to be involved in research, service evaluation and delivery.
Main duties of the job
Working in a skilled and motivated older adult community mental health team that values a range of interventions and support to help people in their recovery journey.
Delivering a high-quality service to service users and their families as part of a collaborative and supportive team.
Key-working of an identified caseload with access to regular and robust supervision.
Delivering assessment, formulation and psychologically informed interventions, in accordance with an agreed care plan, to service users experiencing a wide range of mental health difficulties. Interventions are primarily individual but may include groups in the future .
You will be responsible for the assessment of care needs, including risk, and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence-based interventions. You will, in conjunction with the multi-disciplinary team, provide interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
You will be involved in the roll out of psychological interventions training such as our Supporting Wellbeing Programme. You may be involved in undertaking audits and service evaluations. You will be a valued member of the team.

Job summary
The Central Shropshire Community Mental Health Team has an opportunity for a an EMDR and Complex Trauma Specialist Therapist to join the team working 28 hours per week. In the role you will be travelling as required within Shropshire, Telford & Wrekin, providing clinical services directly face to face and remotely.
As an EMDR and Complex Trauma Specialist Therapist, you will assess, formulate and deliver high quality, evidence based psychological therapies for adults experiencing the effects of trauma, including PTSD and Complex PTSD.
You will hold a clinical caseload, providing trauma focused interventions such as EMDR and other NICE recommended approaches (t-CBT, NET) delivered flexibly through mostly face to face and a small number of digital appointments.
You will contribute to assessments, developing collaborative, person centred formulations and treatment plans informed by a psychological understanding of trauma. Accurate use of outcome measures, clinical documentation and electronic records will be an important part of your practice.
Working as part of a multidisciplinary team, you will offer consultation, advice and guidance to colleagues.You will be required to gather outcome data and contribute to on-going audits within the team around the effectiveness of therapy approaches.
Main duties of the job
- To provide specialist EMDR and Complex Trauma therapies within the Central Shropshire CMHT. To work with clients face-to-face alone, in Severnfields or other care environments, or at their home as necessary. Within this to be able to communicate highly complex information effectively in highly emotive situations and overcome psychological resistances to potentially threatening issues.
- To be responsible for conducting psychological interviews, deriving information from self-report measures, rating scales, psychometrics (where necessary), direct and indirect structured observations and semi-structured interviews with clients, family members and others involved in the client's care, interpreting complex data and giving feedback effectively and with sensitively.
- To be skilled in risk assessment and management. These demands will incorporate frequent intense concentration often in emotionally demanding or unpredictable environments. The post holder will be expected to maintain high professional levels of clinical judgement and application of complex skills at all times.
- To provide assessment reports, both written and verbal, to referrers of people with a wide range of psychological problems, in accordance with codes of professional ethics
- Within consultation sessions and following assessment, to provide formulation and treatments for PTSD and C-PTSD to an exceptionally high standard to service users with a range of complex psychological distress and mental health problems.

The Children's and Young Peoples Mental Health Core Stafford team is recruiting maternity leave backfill for 10 months.
In the role you will deliver a high quality, comprehensive mental health service for children and young people with serious and enduring mental health difficulties. The role involves managing a defined caseload with a focus on working with children and young people and their families who experience a complex range of needs and difficulties.
Acting as a key member of the multi-disciplinary team, you will provide assessment, psychosocial, evidence-based interventions using a collaborative approach and will be skilled at-risk assessment and management. You will complement the multi-disciplinary working of the team, providing supervision for other team members, as agreed with the Team Lead.
You will participate in team and service development and evaluation. Liaison, advice and consultation with a wide range of agencies is central to the role.

Main duties of the job
- Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), and care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with mental health needs.
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.

The primary aim of the 0-19 Integrated Health Visiting and School Nursing Service is to deliver the Healthy Child Programme to children, young people and families across Stoke-on-Trent.
This is a dynamic posts that requires you to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.
You will work alongside the Team Leaders and Professional Lead to ensure synergy across the localities. This will ensure that services are delivered to a high standard that are accessible and demonstrate a commitment to equality and inclusion.
Main duties of the job
- Be a Specialist Community Public Health Practitioner and have detailed professional and clinical knowledge. They will be a specialist practitioner and have a focus in public health across the 0-19 agenda.
- Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.
- Be skilled in the assessment of children and young people's care/support needs and in the development, implementation and evaluation of care/support packages.

We have an exciting opportunity for a passionate, caring individual to join our teams within Inclusion Recovery Hampshire, a drug and alcohol treatment and support service.
We are currently recruiting an experienced group work facilitator, working alongside a teams of experienced Recovery Workers across our Winchester and New Forest sites.
The role may involve occasionally covering for other group facilitators in Hampshire, as well as supporting the hub Recovery Workers based on service need.
We have a bespoke structured group work programme which the Group Facilitator will be responsible for delivering, as well as additional groups based on the needs of clients - this can include SMART groups, auricular acupuncture, mindfulness, meditation and substance specific groups. The successful candidate will be provided with relevant training to support the facilitation of groups and will be a key part of our quality assurance processes around groups; participating in groupwork audits, providing peer to peer feedback to other facilitators and co-facilitating groups with others.
The post holder will take an active role in facilitating service user forums and championing co-production, ensuring service users feel heard and that lived experience shapes service delivery and improvement
Main duties of the job

The service is an integrated drug and alcohol service, delivering an asset based community approach to treatment. Supporting people with drug, alcohol, and mental health challenges, we operate over 80 services across England and Scotland, providing free and confidential support to more than 100,000 people each year. You'll work with them to create the best treatment plan for their needs and to guide them on their path to recovery.
