Our team is looking for a Recovery Worker to join them in providing high-quality recovery care to a diverse client group in the North and East area of Cornwall. Your office base will be Liskeard. We are part of a large integrated drug and alcohol service working with some of Cornwall's most excluded and disadvantaged people. Successful candidates will work as part of the adult community team, alongside in-house services which include outreach, hospital, clinical, criminal justice and a residential rehabilitation facility. With You is all about working with our clients to find the right path for them to get back to where they want to be, and you will be joining an amazing team of people who go the extra mile to make that happen. This role is suitable only for candidates who have had personal lived experience of drug and alcohol issues and who are willing to draw on this experience in their day to day work. This is likely to involve disclosure of this personal experience. We are part of a large integrated drug and alcohol service working with some of Cornwall's most excluded and disadvantaged people. Successful candidates will work as part of the adult community team, alongside in-house services which include outreach, hospital, clinical, criminal justice and a residential rehabilitation facility. The successful candidate will work alongside Recovery Workers, using your knowledge and experience to encourage people to use the service and support them when they are not, working with individuals to achieve their goals. Using your lived experience, you will support our clients with their journey either into recovery or maintaining their recovery. Our Lived Experience Support Workers are an active and integral part of our teams, bringing ideas and solutions so we can develop and improve our recovery communities and helping our clients to get onto the path to recovery. In your role, you will reach people through outreach, harm reduction, structured interventions and activities and will show through your own experience, knowledge and learning that change is possible.

This is a key role providing essential administrative support to ensure smooth service delivery. You'll be responsible for managing referrals, maintaining accurate records, and supporting team communications. Our Virtual Ward is a new and innovative service which offers face to face acute care in the patient's own home. The Virtual Ward has an agreed admission criteria which works in partnership with supporting care services and the aim of the service is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives. This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient's experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.

You will be able to prioritise your own workload without supervision, have an excellent patient focused approach and use your initiative as well as work as part of a multidisciplinary team.

We have an exciting opportunity for a Paramedic to join our Primary Care team at HMP Altcourse a category B Prison located in Fazakerley, Liverpool.

HMP Altcourse is the busiest remand prison in the country and you will be at the heart of prison healthcare, managing triage and unplanned care for a diverse and complex patient population. You will assess and treat patients, responding to a wide range of clinical needs, including, Emergency response and acute illness, Chronic disease management, Injuries, trauma, and use of force assessments and Patients under the influence of drugs or alcohol and Mental health crises.

Hours:

• Monday - Sunday service - 37.5 hours per week
• 7am - 8pm / 9am - 9pm / 8.30am - 9.30am
• 1 weekend in 3
• Rota's are done 2 months in advance
• Occasional nights

Salary

This position is full time, youll receive an annual salary up to £43,609 - £49,175 per annum FTE which will be pro rota - depending on experience.
