Job summary
Taunton Deane West Primary Care Network is seeking an experienced and motivated Advanced Nurse Practitioner to join our multidisciplinary team in an Additional Roles Reimbursement Scheme ARRS funded post The role is hosted by Lister House Surgery on behalf of the Primary Care Network and will support the delivery of high quality patient centred care
You will work as an autonomous clinician providing advanced assessment diagnosis treatment and management for patients presenting with acute illness long term conditions and complex health needs You will work closely with GPs practice nurses pharmacists first contact practitioners and other members of the multidisciplinary team to provide safe effective and timely care
The successful candidate will help improve access to primary care support prevention and wellbeing and contribute to the continued development of integrated services across the Primary Care Network You will play an important role in improving patient outcomes reducing health inequalities and delivering proactive coordinated care
Working pattern
Tuesday 8 30am to 6 00pm
Thursday 8 30am to 2 30pm
Friday 9 00am to 5 00pm and 6.30pm to 8.00pm (Late night can be remote)
Main duties of the job
We are looking for an experienced and motivated Advanced Nurse Practitioner to join our PCN team You will work at Lister House Surgery to provide high quality patient centred care for people with acute and long term conditions. The PCN Roadmap will need to have been completed.
You will work using advanced clinical assessment examination diagnosis and treatment skills You will support the delivery of safe effective and accessible primary care while working closely with clinicians and the wider multidisciplinary team
This is an excellent opportunity to contribute to innovative models of care and help improve health outcomes across the local population
Main duties
Provide autonomous assessment diagnosis treatment and management of patients presenting with undifferentiated and undiagnosed conditions
Manage acute presentations and LTCs within your scope of practice
Prescribe safely and appropriately where qualified as an independent prescriber
Request interpret and act on investigations
Develop management plans and make referrals where appropriate
Work collaboratively with the practice and the wider multidisciplinary team
Support service development quality improvement and clinical audit
Maintain accurate clinical records using electronic patient record systems
Act as a resource and support learning for colleagues where appropriate
Participate in safeguarding infection prevention clinical governance and continuing professional development

Job role:

KEY RESPONSIBILITIES
Clinical Primary Care Network
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with highly complex needs
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based Primary Care Network and national and Primary Care Network protocols, and within scope of Primary Care Network
Communication
Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
Communicate with and support patients who are receiving bad news
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
Anticipate barriers to communication and take action to improve communication
Maintain effective communication within the Primary Care Network environment and with external stakeholders

Job summary
Taunton Deane West PCN is looking for an experienced and motivated Paramedic to join our team under the Additional Roles Reimbursement Scheme (ARRS)
The role involves working within a multidisciplinary team to deliver high-quality, accessible patient care, helping reduce GP workload and improve outcomes.
Responsibilities include urgent and same-day care and home visits and managing acute and long-term conditions while working autonomously within the paramedics scope of practice and under appropriate clinical supervision.
A Paramedic under the Additional Roles Reimbursement Scheme the PCN must ensure that the paramedic ais educated to degree or diploma level in Paramedicine or equivalent experience bis a registered Paramedic with the HCPC c has completed their two year Consolidation of Learning period as a newly qualified parmedic; , has a further three years experience as a band 6 or equivalent paramedic; and is working towards developing masters level or equivalent capability in paramedic areas of practice and, within six months of the commencement of reimbursement for that individual or a longer time period as has completed and been signed off formally within the clinical pillar competencies of the paramedic FCP AP roadmap to practice.

The PCN QOF, Research & Care Coordination Data Analyst is a non-clinical role that combines data analysis, quality improvement, research support, population health management and care coordination within Wellington Medical Centre and Taunton Deane West Primary Care Network (PCN). Wellington Medical Centre is part of Taunton Deane West PCN, which brings together GPs, nurses, pharmacists, social prescribing link workers, health and wellbeing coaches and other healthcare professionals to provide integrated care for local communities. Main duties of the job
Key responsibilities include supporting Quality and Outcomes Framework (QOF) performance through clinical searches, audits, reporting and data analysis, enabling the practice to monitor achievement, identify gaps in care and ensure patients receive timely reviews and interventions. Assisting to improving data quality and clinical coding by identifying inaccuracies, inconsistencies and missing information within patient records, helping to ensure that clinical data is accurate, reliable and meaningful for patient care, service planning and reporting.The role also uses population health data, risk stratification tools and clinical intelligence to identify patients who may require monitoring, follow-up or proactive support.
