Job summary
Join our dynamic team at Foundry Healthcare Lewes as we strive to deliver exceptional care to our community. We have an exciting opportunity and are seeking a passionate and dedicated individual to receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way across all Foundry Healthcare Lewes sites.
Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Main duties of the job
- Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice
- Deal with all general enquiries, explain procedures and make new and follow-up appointments.
- Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
- Contact patients by telephone or letter at Doctors request to arrange appointments.
- Patient notes and correspondence:
- Enter data onto patient computer records as required
- Ensure the kitchen and waiting room are kept clean and tidy in turn with other staff.
Premises:
- Open up premises at the start of the day when first to arrive, de-activate alarm and make all necessary preparations to receive patients. When last to leave at the end of the day, ensure that the building is totally secured, internal lights are off and the alarm activated.
- Take new registration forms and check they are appropriately completed.
- Carry out administrative duties as requested.
- Undertake any other additional duties appropriate to the post as requested by the Team Leader or Patient Services Manager.
- Undertake any training required by the practice
About us
Foundry is a single Practice PCN serving a population of circa 28,500 patients in the Lewes and Ringmer area of East Sussex. We operate from three surgery sites in Lewes and one in Ringmer, a mixture of both purpose built and converted buildings. Foundry aims to provide consistently excellent primary care to our local population and to develop a comprehensive population health management approach with our patients and partners.

Salaried GP - 6 months Maternity Cover
An exciting opportunity has arisen for a motivated and flexible salaried GP to provide maternity cover 6 sessions each per week, days to be confirmed, however Monday will be 1 of the days required for 12 months, commencing ASAP but dates can be flexible
The practice is based over 3 sites all close to each other. Our supportive team are well established, friendly, hardworking and dedicated to providing a good experience for the patient and a pleasant working environment. The opportunity is available to both newly qualified and experienced GPs who want to work in a forward thinking training practice. We are continuously looking to improve what we do and value innovative and new ways of working.
Main duties of the job
JOB RESPONSIBILITIES:
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
- In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Job summary
The Wellbeing Hub Co-ordinator plays an important role within the wellbeing service to help collate referrals, make initial contact, and ensure participants are triaged to the correct service. They will work closely with GPs and primary care professionals to identify and manage a caseload
The Wellbeing hub consists of;
Social Prescribers who empower people to manage their health and wellbeing via referral to non-medical link workers who focus on what matters to me, connecting people to community groups and statutory services.
Health and Wellbeing Coaches who support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them.
The Pain Cafe, which is a seven-week course designed to empower people living with persistent pain to better understand and manage their condition. Through a combination of education, practical strategies, peer support and goal setting, participants can build confidence, improve their wellbeing and develop techniques to help them live well despite ongoing pain. The course focuses on self-management, helping individuals take control of their symptoms and improve their quality of life.
Main duties of the job
The Wellbeing Hub Coordinator is focused on coordinating referrals, supporting patients to access the right wellbeing services, managing communication between professionals and patients, promoting personalised care, supporting the delivery of the Pain Cafe and helping to deliver the Wellbeing Hub service across the PCN.

Main duties of the job
The Advanced Nurse Practitioner/Advanced Clinical Practitioner will be a key member of the Operose Health team, tasked with delivering excellent primary care.
