Job summary
Central and West Warrington Primary Care Network (CWW PCN) is seeking to recruit a Lead Practice Pharmacist to develop and support medicines optimisation across our seven GP practices.
This is an exciting opportunity for an experienced and motivated primary care pharmacist to join an established, forward-thinking PCN team. Our pharmacy team includes Clinical Pharmacists, Pharmacy Technicians and a Care Coordinator, supported by a Senior Pharmacist, PCN Clinical Director, and the wider PCN team.
The post will be directly employed by a core practice but will work collaboratively across multiple practices within CWW PCN to support delivery of the Primary Care Network DES and local practice priorities.
We are keen to hear from pharmacists with experience in primary care, who have completed the CPPE Primary Care Pharmacy Education Pathway and Independent prescriber (IP) qualification.
Main duties of the job
The Lead Practice Pharmacist will be committed to improving health outcomes and putting patients first. Key qualities include the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital teams, and practice staff, alongside alongside the ability to lead and work effectively within a multidisciplinary team.
The post holder will:
- Work as part of a multidisciplinary team in a patient-facing role, providing leadership to the practice pharmacy function
- Support delivery of Primary Care Network DES specifications
- Conduct and oversee structured medication reviews (SMRs) for patients with complex polypharmacy, including patients in care homes
- Provide expert clinical medicines advice to patients and practice teams
- Lead initiatives to improve prescribing practices across the Practice(s) and PCN
- Drive quality improvement and service development, including audits and implementation of best practice
- Work with community and hospital colleagues to resolve medication-related issues and improve care pathways
- Take overall responsibility for pharmacy governance and patient safety within the Practice(s), including overseeing the timely actioning of safety alerts and ensuring safe drug monitoring processes are in place
- Provide clinical leadership, support, and supervision to junior pharmacy colleagues and trainees

Training Support
Most roles in our PCN require a training element. Staff partaking in training that meets certain requirements will be required to sign a Training Agreement that outlines associated costs to the training. Staff may be liable for reimbursing those costs to the Practice / PCN if they leave their PCN role before, during or after the training. Job responsibilities
- Working as part of a multidisciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas. The role will contribute to successful clinical management of patients and build capacity in the workforce at the right levels.
- Using clinical skills to provide specialist medication review and assessment of patients with long term conditions having complex medicine regimes and produce the best outcomes for the patient.
- To further proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular Frailty, COPD and Asthma); people with Learning Disabilities or Autism (through STOMP-Stop Over Medication Programme)
- Providing specialist expertise and advice in respect to the management and use of medicines and medicines management to patients, carers, practice staff as well as a wide range of health and social care professionals in helping to tackle inequalities
- Be an Independent Prescriber and safely prescribe within your scope of competence, ensuring evidence-based, patient-centred care.
- Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the Practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement whilst contributing to the quality and outcomes framework and enhanced services
- Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care
- Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload

Job summary
We have an exciting opportunity for a four-session Salaried GP to join our experienced clinical team. This post is supported by the 2026/27 Practice-level GP Reimbursement Scheme.
We are a friendly practice on the outskirts of Sheffield, serving a mixed urban and rural community of over 8,700 patients.
We are flexible on working days for the right candidate, although we would particularly welcome applicants who are able to work on Fridays. Each session consists of 12 consultations, comprising a mix of telephone and face-to-face appointments. There is no requirement to contribute to on-calls or duty doctor commitments, and 15-minute consultations can be facilitated on request.
We have a multidisciplinary clinical team, including an ANP and paramedic team. Home visits are occasional, with the majority undertaken by our paramedics. GPs contribute to the remaining visits as required.
This is a fixed-term contract until 31 March 2027, with the option to extend subject to the national funding continuing into future years.
The position is eligible for the NHS Pension Scheme. Salary is dependent on skills and experience, with 6 weeks' annual leave pro rata.

Job summary
Lewisham Personality Disorder Service (LPDS) is looking to recruit a Band 7 or Band 6 MBT practitioner for a one-year fixed term post. This may be a secondment from elsewhere in the NHS. The aim of the post is to deliver individual MBT in order to reduce a long waiting list of assessed patient. It is an opportunity to join a well-established and effective MBT team of MBT therapists, contributing towards a treatment programme consisting of individual and group MBT.
Specialist supervision is provided, and the role would suitsomeone with an interest in this patient group who likes working as part of a team and is wants to practice MBT. Support for MBT in the wider organisation is excellent with Trust wide special interest groups and strong links to the Anna Freud Centre.
In terms of career progression, it will enhance your intervention skills working therapeutically with personality disorder or other mental health difficulties in collaboration with other team members. You will either by MBT trained, or training will be provided during supervision to enable you to pursue MBT after this initial experience.
The post is suitable for a qualified clinical or counselling psychologist, or a mental health worker (eg CPN, OT, practitioner psychologist, social worker) with good NHS mental health experience.
If you are Band 6, you would be employed at Band 6.

We have an exciting opportunity for a Clinical Pharmacist to join our multi-professional Primary Care Team working within our Health & Wellbeing Hub under the direct supervision of the Clinical Director. An exciting opportunity has arisen for an ambitious Senior Assistant Technical Officer to join the Pharmacy Production team in the Cytotoxic Chemotherapy production Department (Cytolab) at Nottingham University Hospitals NHS Trust (NUH). You will receive full training from our dedicated and experienced pharmacy production team. You will also be required to deliver cancer treatments to NUH wards and departments and the City Campus and support the department in ordering and managing stock.

Job summary
We are seeking an enthusiastic and experienced Paramedic Practitioner to join our Acute Home Visiting Service, delivering high-quality care to patients in their own homes across the Primary Care Network (PCN).
Working autonomously within a multidisciplinary team, you will assess, diagnose, treat and manage patients with acute illness, frailty and complex health needs who are unable to attend surgery. Using advanced clinical skills, you will develop management plans and facilitate admission, referral or community-based care where appropriate.
This is a part-time role working 10:00am to 6:00pm. Working days will include every Friday, which is fixed due to service demand, together with either Wednesday or Thursday, to be agreed with the successful candidate.
Working closely with GPs, community teams, care homes and secondary care services, you will deliver integrated care, support patients to remain safely at home and help prevent avoidable hospital admissions. You will also contribute to service development, quality improvement and patient education.
The successful candidate will be expected to complete the First Contact Practitioner (FCP) Roadmap to Practice if not already achieved. Full support, supervision and development opportunities will be provided to enable completion of the pathway.

Main duties of the job
The Band 7 Physiotherapist is responsible as the clinical lead for the Primary Healthcare Rehabilitation Facility (PCRF) and will oversee the provision of high-quality primary care rehabilitation across the combined practice. Their duties include the delivery of advanced clinical practice and the management of rehabilitation staff. The Combined Practice is formed across 2 sites, Aldershot Medical Centre as the larger site and Minley Medical Centre as the smaller site.
To work as a Team Clinical Lead managing staff. To be a source of highly skilled clinical expertise in the management of musculoskeletal (MSK) dysfunction. Reporting to the Senior Medical Officer (SMO), the Band 7 Clinical Lead provides clinical supervision and advice to senior physiotherapists, General Practitioners, and the service persons' military Chain of Command.

Job Purpose
The IT & Digital Manager will take responsibility for the effective day-to-day management, development and improvement of the practice's IT and digital systems.
The postholder will provide practical technical support to staff, oversee digital systems and workflows, manage relationships with external IT and software providers, and identify opportunities to improve efficiency, patient access and the overall use of technology within the practice.

Key ResponsibilitiesIT & Hardware Support
- Provide day-to-day support for practice IT equipment and digital infrastructure.
- Troubleshoot and resolve issues with PCs, laptops, printers, scanners, touch-screen patient displays and other connected equipment.
- Identify recurring technical problems and take appropriate action to prevent them from reoccurring.
- Liaise with external IT support providers where issues cannot be resolved internally.
