Job Summary
Broom Lane Medical Centre is looking to recruit an experienced Band 7 Practice Nurse to join our clinical team. The vacancy arises out of the retirement of a longstanding member of staff.
You would be joining our large practice team delivering primary care to our 15,000 patients over two sites. Working independently but with the support of a variety of clinical colleagues, you will deliver primary care nursing to our practice population, with a focus on the management of chronic diseases.
We are ideally looking for 25-30 or 37 hours worked per week and experience in general practice. Independent prescriber status aspirational but not essential.
Main duties of the job
Main duties of the job
To deliver nursing care to our practice population, working alongside our Lead Nurse, fellow band 7 nurses, a new treatment room nurse role and our HCAs and phlebotomists. A particular focus will be chronic disease management of asthma, COPD, diabetes and hypertension as part of a wide range of nursing roles. The ideal candidate would be fully qualified in theses areas and have experience of working in primary care.

We are a long established large GP practice based at Broom Lane, and a large branch at Kimberworth Park. In total we have around 15,000 patients across two sites. We have just completed a major rebuild and remodel at the Broom Lane site making us a desirable place to work .
Our staffing team consists of six GP partners with a variety of specialist interests, four salaried doctors, Practice Management team, administration department and two reception teams working alongside our nursing team as above. We are well connected with our Primary Care Network and utilise ARRS staff including pharmacy team, social prescribers, physios and mental health practitioners to better resource our patient care.

We also have access through our PCN to our first contact Physio, Social Prescribers and Health and wellbeing coaches.
Our Practice list size is 5600 patients:

o High QOF achiever and wide range of enhanced services
o Dispensing Practice to 95% of our patient list
o In house minor surgery and LARC clinics
o Rated Good in all areas by CQC in June 2017

Job summary
An exciting opportunity has arisen to appoint a Band 5 Staff Nurse to join our friendly, Paediatric nursing teams.
36 hours permanent position available.
The Paediatric Department consists of a team of experienced and dedicated Nursing and Medical staff providing care to children and young people from birth to 16 years.
The Children's Ward includes 17 in-patient and 4 day case beds . There is also a 6 bedded Paediatric Observation Assessment Unit co-located next to the ward.
An Enhanced DBS Certificate, with the checking of the Children's Barred Lists is required for this position.
Main duties of the job
- To be responsible in association with the team leader for the formulation and implementation of individual patient care plans.
- To participate in the holistic assessment of patients, and to formulate and review individual patient care plans and to act as 'named nurse' or associate for a group of patients from admission to discharge as determined by the Ward Manager.
- To commenceand participate in research/special projects as directed by the ward manager.
- To contribute to the setting, maintaining and monitoring of standards of care within the ward.
- Help to ensure that all equipment is in safe working order in conjunction with the department equipment controller.
- Internal rotations onto night duty and twilight shifts are required.

Job summary
We are looking to recruit a Band 5 Registered Nurse at 37.5 hours for our Gynaecology outpatients department. Fixed term role of 12 months.
We are looking for a passionate, motivated nurse who has an interest in women's health. Are you a caring and self-motivated nurse who is a strong team player but also has an enthusiasm for working autonomously with excellent communication skills?
The service is currently Monday to Friday, though you will need to be flexible as clinics may change due to demand. You will also be asked to travel to support some of our community clinics.
There will be opportunities for learning and development especially in nurse led clinics. You will be responsible for working with the multi-disciplinary team to ensure smooth running of the clinic, ensuring patients are kept informed of waiting times, prioritising tasks as appropriate, keeping well-coordinated stock levels or equipment and patient information leaflets.
Due to the short term nature of the role, the successful candidate must hold full registration with the NMC and have existing right to work in the UK, at the time of application.

Main duties of the job
To ensure that an effective plan of care/care pathway is implemented and maintained for the patient.
That the patient's plan of care is reviewed, evaluated and update in accordance with the clinical condition and that all documentation is completed to the required standard. To advocate patient involvement so that care can be reflected as individualised.

Main duties of the job

This is an important role within our practice and wider Primary Care Network (PCN), supporting patients to navigate health and care services and helping them to take an active role in managing their health and wellbeing.

Job summary
An exciting opportunity has arisen for a first contact physiotherapist practitioner to join our forward-thinking, multidisciplinary team and support PCCO Primary Care Network.
We have a well-developed MSK team, comprised of three GPs with a specialist interest in MSK and three senior physiotherapists. The post holder will manage a complex caseload of patients, working independently to assess, diagnose and manage patient's musculoskeletal problems.
This physiotherapist will be given the opportunity to become an advanced practitioner using their own special interests, a clinical case load and being hands on in group work.
At PCCO PCN, you will be given the agency to work as an independent professional, working with the PCN management team to analyse both qualitative and quantitative data to identify areas of need within our population and formulate plans and projects to create support pathways for these patients. You will also receive referrals from other PCN and practice members to provide 1-2-1 support to patients who require an MSK assessment.
You will have access to clinical supervision from one of our Clinical Directors and support from the rest of our PCN and practice teams. You will work closely with GPs, community services, the city MSK service, EMAS, Social Care, Practice staff and wider MDTs.
Main duties of the job
The post holder will be given the autonomy to specialise in specific health conditions in line with population health analysis and work with the wider MDT for conditions such as chronic pain.
If you have a specialism, such as chronic pain, chronic back pain or in tackling health inequalities, this will be considered as an advantage, and we would love to hear about what you believe you could personally bring to our PCN.

Rotherfield and The Brook Surgeries are seeking an experienced and enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our friendly and supportive team on a flexible bank basis, with the potential for a permanent position.
This role is ideal for an autonomous, experienced primary care clinician who enjoys managing undifferentiated presentations and providing high-quality patient-centred care. You will undertake telephone and face-to-face consultations, assess, diagnose and treat a wide range of acute and chronic conditions, prescribe independently, and work collaboratively within our multidisciplinary team.
Our practices use Total Clinical Triage to manage all incoming patient requests, and you will play a key role within this model. Full training and support will be provided to ensure you feel confident working within our systems and processes.
We are looking for clinicians with significant experience in primary care who are confident working independently while recognising the value of teamwork, clinical governance and continuous learning.

Main duties of the job
The successful applicant will work as an autonomous Advanced Clinical/Nurse Practitioner within our multidisciplinary team, delivering safe, effective and patient-centred care across a broad range of clinical presentations.
You will undertake telephone and face-to-face consultations, assess, diagnose and manage patients presenting with undifferentiated and acute illness, provide appropriate treatment, prescribe independently, request and interpret investigations, and refer patients to other services where appropriate.

We are looking for an enthusiastic and reliable Practice Nurse to support our practice team in delivering primary care service to our diverse patient population.
You will be working with a varied patient demographic and an experienced Multi-disciplinary team.
