Job summary
We are looking for a Registered Nurse or Allied Health Professional to join us in this new, advanced clinical role. This is an exciting opportunity to play a crucial role in providing equitable care for Motor Neurone Disease across the Coventry and Warwickshire region.
This post has been made possible by the MND Association for the next four years, to establish this service and work with patients, carers & other services to enhance MND care and support.
You'll be working collaboratively with the Care Coordinator at Mary Ann Evans Hospice, where there is already an MND service, as well as other regional partners, multidisciplinary teams, neurology, respiratory, speech therapy, & palliative care services.
We would love to hear from you if you have:
- Valid registration with a relevant professional body (e.g. NMC or HCPC)
- Relevant post-registration clinical experience (e.g. neurology, palliative care or community nursing)
- Experience of collaborating across healthcare, social care & voluntary sectors
- Knowledge & understanding of disease management & MND treatment
- Leadership, interpersonal, & communication skills
- Organisation and problem-solving skills, to be able to manage complex caseloads
- UK driving licence & use of a suitable vehicle for work
This role will support our work by enhancing care & support for MND & improving patient & carer experience.
An enhanced DBS check will be required, including the adults barred list, because you will be providing an healthcare service for adults.
Main duties of the job
This role will involve:
- Supporting people with MND & their families to manage symptoms
- Holistic needs assessment & management planning, from diagnosis to end of life
- Receiving, making & coordinating referrals
- Developing & coordinating seamless specialist care pathways between all external & internal stakeholders
- Establishing & providing an advisory service & a single point of contact for patients
- Monitoring outcomes & reporting
- Leading service development & improvement
You will be based on site at our Coventry Hospice, but with regular travel across the region, including home visits.
It is a full-time role at 37.5 hours, over 5 days, per week. Job share will also be considered.

Key areas of responsibility and accountability:
- Co-ordinate care and provide a single point of access from diagnosis for patient, their families and health and social care professionals.
- Establish a relationship with patients and their families helping to enable them to make informed decisions about treatment/care and advance care planning.

Job summary
We are looking for a Registered Nurse or Allied Health Professional to join us in this new, advanced clinical role. This is an exciting opportunity to play a crucial role in providing equitable care for Motor Neurone Disease across the Coventry and Warwickshire region.
This post has been made possible by the MND Association for the next four years, to establish this service and work with patients, carers & other services to enhance MND care and support.
You'll be working collaboratively with the Care Coordinator at Mary Ann Evans Hospice, where there is already an MND service, as well as other regional partners, multidisciplinary teams, neurology, respiratory, speech therapy, & palliative care services.
We would love to hear from you if you have:
- Valid registration with a relevant professional body (e.g. NMC or HCPC)

This role will support our work by enhancing care & support for MND & improving patient & carer experience.

Main duties of the job
This role will involve:
- Supporting people with MND & their families to manage symptoms
- Holistic needs assessment & management planning, from diagnosis to end of life
- Receiving, making & coordinating referrals
- Developing & coordinating seamless specialist care pathways between all external & internal stakeholders
- Establishing & providing an advisory service & a single point of contact for patients

You will be based on site at our Coventry Hospice, but with regular travel across the region, including home visits.

We are a much-loved and well supported charity, at the heart of our community, with three hospices, a range of community services, & 26 charity shops across the region.
We are looking for an experienced Registered Nurse to provide clinical leadership and operational support across our specialist palliative care Inpatient Units during night shifts.
As a Senior Night Nurse, you will be an accessible point of contact for colleagues, ensuring high standards of care, assisting with management of complex clinical situations, and co-ordinating resources across both units.
Various positions are available (hours to be agreed with successful candidates). You will be required to work at both of our hospices (Coventry and Warwick).

We are looking for an experienced healthcare chaplain of any faith tradition to lead our team andto work with us in providing spiritual, pastoral and religious care to patients, relatives and staff. This post will be based in our acute team and will work across our three hospitals (Warwick, Stratford and Leamington Rehabilitation Hospital).

The Lead Chaplain is responsible for leading the delivery of multi-faith and belief Chaplaincy services across the Trust.

Job summary
We are looking for an experienced chaplain of any faith tradition to join our team and to work with us in providing spiritual, pastoral and religious care to patients, relatives and staff. This post will be based in our acute team and will work across our three hospitals (Warwick, Stratford and Leamington Rehabilitation Hospital).
You will join a well-established, creative and committed team in a Trust which is very supportive of the chaplaincy service. The wider team includes over 40 very well trained, active chaplaincy volunteers and part of the role will be to provide training and supervision for them. There is some change ahead, with a new Lead Chaplain to be appointed within the next six months.
You will be expected to participate in our emergency out of hours on call provision, which also includes call-outs to the Myton Hospices at Coventry and Warwick. We anticipate this to require being on call one night a week and one weekend a month.

We are seeking an experienced & motivated Senior Staff Nurse to provide clinical leadership & operational support across our inpatient services during night shifts.
This is a key leadership role where you will act as a visible and accessible point of contact for night staff, ensuring high standards of patient care, supporting colleagues with complex clinical situations, & coordinating resources across both inpatient units.

Main duties of the job
Our Senior Night Nurses:
- Are clinically based on the inpatient unit with the highest level of complexity, patient acuity, dependency needs, or staffing pressures.
- Provide leadership, advice, guidance, and hands-on support to nursing and care staff working night shifts.
- Act as the primary point of contact for night staff requiring clinical or operational support.
- Assist with the management of complex patient care issues and challenging situations as they arise.

- Current NMC registration

- Relevant specialist course, e.g. Palliative care / advanced communications.
Experience
Essential
- Significant post registration experience
- Experience of needs assessment / care planning
- Experience of Multi-disciplinary Team working
- Caring for patients at the end of life
- Experience of caring for patients and families with life limiting / life
- threatening conditions
- Good knowledge of assessment and symptom management
Desirable
- Experience caring for patients with complex health needs
- Experience working in palliative care setting
