Overall Purpose of the Role:
The post holder will work as an effective member of the Hospice IPU Team.
The post holder will also work in partnership with the wider primary healthcare team and other specialist palliative care services to meet the end-of-life care needs of the local population.

Job summary
The Health Care Support Worker Manager provides day-to-day leadership and support for the Health Care Support Workers within either the Inpatient Unit or Hospice at Home service. They ensure that the fundamental aspects of care are delivered consistently and compassionately, and that patients and families feel respected, listened to and well supported.
Working closely with the Head of Inpatient Unit / Head of Hospice at Home and the Nurse Manager, the Health Care Support Worker Manager coordinates the deployment, supervision and development of Health Care Support Worker s, helping them to work safely and confidently within their roles. The Health Care Support Worker Manager is first and foremost a people manager for the Health Care Support Worker team. Job summary
The Palliative Care Pharmacist is the lead pharmacist for Rotherham Hospice and provides strategic and operational leadership and medicines advice for safe, effective and person centred use of medicines across all hospice services.
Main duties of the job
Working as part of the Care Navigation hub, the post holder works with the Head of Care Navigation, Triage Manager, Doctors, Advanced Clinical Practitioners (ACPs) and Clinical Nurse Specialists (CNS) with expert advice on medicines, prescribing and stewardship. They oversee the onsite drugs room, work closely with TRFT pharmacy services for medicines supply, and lead the hospice transition to e prescribing.
The Palliative Care Pharmacist is responsible for clinical pharmacy services, medicines governance, policies and audits, and for ensuring that medicines support Living Lifes Wishes, helping patients to manage symptoms and maintain quality of life in the safest and most effective way possible.
