Minimum of 22.5 hours per week, with potential of up to 37.5 hours per week in line with service requirements. Please get in touch with the individual listed in the contact section if you would like to discuss this further.

This role forms part of the PCNs social prescribing service, supported by the locally commissioned Health Connections model.
