Job summary
We are currently seeking an experienced, enthusiastic and motivated practice nurse to join our forward thinking team at Westbury Group Practice.
The successful candidate will work with a range of health and care professionals and will be supported within our strong nursing team. You will be part of the team responsible for helping people manage long term conditions, health screening, family planning, running vaccination programmes and providing wound care to the Practice population. The successful candidate must have skills and experience in caring for people with diabetes in the community, administering child immunisations and competency in compression bandaging.
Main duties of the job
The postholder will undertake clinical nursing practice, within scope of competence, to deliver holistic care to people accessing primary healthcare services. They will operate practice nurse clinics specific to competencies and needs of the service. For example; immunisation clinics, travel clinic, wound care clinics, general practice nurse clinics and diabetes clinics.

Job summary:
The post holder is a registered nurse who is able to work independently and in conjunction with other healthcare professionals in the assessment, diagnosis, planning, implementation and evaluation of patients’ care needs. They will work within their scope of competence, in the primary care setting.

The Role
As a Clinical Pharmacist, you will play a key role in supporting the management of long-term conditions, medication optimisation, structured medication reviews and medicines reconciliation. Person Specification Qualifications Essential * Registered Pharmacist with the General Pharmaceutical Council (GPhC). * Independent Prescriber qualification (or working towards).
