Job summary
Job overview
The LGI Cancer band 7 Clinical Nurse Specialist (CNS) will be an integral part of the LGI CNS team, looking after patients with a high suspicion of / confirmed diagnosis of Colorectal Cancer. The post holder will work closely with the Lead LGI oncology CNS, Enhanced Recovery team, Faster diagnosis triage CNS team, stoma nursing team and Cancer Support team.
The post holder will be a highly competent, credible, and experienced autonomous practitioner who uses their breadth and specialist knowledge and professional visibility and advanced clinical skills of practice to support patients on the LGI pathway.
The LGI Cancer CNS will specifically manage the surgical part of the Patient Pathway, supervising the band 6 LGI CNS and managing the Band 4 CEA navigator, managing the patient through from Endoscopy (probable cancer) diagnosis and up to the first adjuvant oncology appointment. The CNS will also ensure an improved service for patients who are diagnosed through the emergency pathway.
The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role, within a defined framework.
Main duties of the job
- The CNS will demonstrate a higher level of practice through the provision of specialist nursing knowledge and expertise in the management of patients having surgery and other cancer therapies, liaising with colleagues across care settings to expedite specialist assessment, providing holistic packages of care to ensure patients receive prompt and evidence-based care.
- The CNS role is divided into four general areas - clinical excellence, leadership, education and research/ clinical audit.

About your new team and department
The Lower GI Oncology Multidisciplinary Team (MDT) comprises a group of expert doctors, nurses and other health care professionals with a special interest in the diagnosis, treatment and management of people with LGI cancer diagnosis.
This MDT approach across all services ensures that all necessary investigations are carried out as quickly as possible and an agreed programme of treatment is planned which is specific to the patient's needs. Treatment options are then discussed with the patient and their family and started without delay. The Oncology MDT's link into other health care professionals in primary and secondary care as appropriate, to ensure a seamless service is provided which meets the patient's needs.
We are a dedicated group of enthusiastic and lifelong learners, who work well together in an expanding team to deliver high quality, individualised care to patients with Colorectal Cancer. We work as part of the Cancer Support team based within the Acute trust.

Communication and working relationships
Co-ordinate multidisciplinary practice to implement colorectal cancer pathways in specified clinical areas as defined by the Lead LGI CNS.
Participate as a key member of the multi professional team within colorectal cancer pathways, ensuring appropriate referral and discussion with other members of the team to optimise patient care.
To ensure a clear meeting structure exists within areas of responsibility and that both formal and informal lines of communication exist with the multi-disciplinary team (MDT).
Plan and organise own and associated team workload in conjunction with Trust requirements.
Ensure implementation of agreed systems and processes to provide safe, effective, high quality patient care and embed a culture of improvement, focusing on the Trusts harm free indicators.
To be a visible, accessible and approachable leader to staff, patients, carers, families, and the public ensuring that open channels of communication are created and sustained.
Promote a learning environment for the professional development of staff interacting with the colorectal cancer team, leading by example and mindful of the effect of change upon all bands of staff.
Ensure all complaints both formal and informal are dealt with promptly and courteously in accordance with Trust policy.
To respond in writing to complaints by interviewing staff, collating information and writing statements to provide evidence, reviewing practice where appropriate in response.
Lead and contribute to developments that are relevant to colorectal cancer as dictated by local and national initiatives and undertake benchmarking.
Identify and co-ordinate opportunities for patient and public involvement and engagement in service development, leading where appropriate.
Act as a role model, demonstrating high standards of care and providing clinical leadership
Analytical and judgement
- Actively participate and contribute to departmental clinical supervision groups to ensure a regular evaluation of practice through peer review.
- Regularly participate in the departmental Clinical Governance programme including presenting research findings, evidence-based practice workshops, clinical audit, and reporting of clinical risks.
To develop knowledge and skills within the LGI specialty, consulting with clinical supervisors as appropriate.
Planning and organisation
- Plan and organise complex activities, programmes and those requiring adjustments.
- Generate and implement new solutions that will best suit the needs of patients and carers, through thinking laterally about practice.
- Act as a positive role model and provide highly specialist advice and consultancy for multidisciplinary colleagues including colleagues working in the Acute Trust, Primary Care, Social Care, charitable sector and other external agencies.
- Work with colleagues in the development of regional and national guidance in Oncology service.
- Review and monitor standards for best practice contributing towards preparation for performance evaluations.
- Overcome barriers to understanding using leadership and management skills.
- Provide expert advice and information to other professionals and agencies to ensure appropriate referral to the service.
Physical skills
Due to the hospital layout and requirement of patient facing support, a degree of physical fitness is important as well as having good dexterity for administrative tasks.
The ability to work under pressure both in terms of time and at a time when emotions for patients/those affected by cancer are escalated.
Patient and client care
Be responsible for a clinical caseload, providing specialist LGI assessment and intervention to patients with a LGI diagnosis
To monitor and review the progress of clients against the targets set and agreed and to modify these as appropriate to meet individual patient circumstances
Maintaining high levels of evidence base LGI practice, contributing to and facilitating care processes and pathways. Demonstrate understanding of LGI underpinned by theoretical knowledge and relevant clinical experience and competence.
Display specialist knowledge of individuals physical and mental health needs and appropriate evidence informed interventions

Job summary
This is a role within our Plus Team which provides an enhanced service to all children and families subject to Safeguarding Plans.
You will be a Specialist Community Public Health Nurse (SCPHN) Health Visitor (HV) within the integrated 0 to 19 Torbay Partnership and provide the expertise, knowledge and resources to deliver the Healthy Child Programme (HCP) to families living within Torbay.
Working with the 0 to 19 Torbay workforce to ensure pathways are followed in supporting children, young people, families and carers. Templer Ward is a 30 bedded in patient ward within Newton Abbot community Hospital. 20 beds provide care to specialist stroke and neuro rehabilitation patients following treatment in an acute hospital setting. The Urology Nurse Practitioner's main aim is to provide specialist advice and clinical practice to patients within the urological outpatient setting and enabling optimum quality of life.

The Nurse Specialist for the Urology Team will maintain excellent working relationships with patients, their relatives, and a multi-disciplinary team of professionals working across the trust, including:

Main duties of the job
- Progress towards providing high quality diagnostic imaging under supervision within the required scope of practice in any of the TSDFT sites. The successful candidate will work in the Main X-ray Department of Torbay Hospital. The BSc Degree course will be provided by the University of Exeter and the practical training being carried out within Torbay and Newton Abbot Hospitals supported by the existing Radiology Team. Academic work is largely via distance learning, though there will be attendance at the University a number of times throughout the 3-year training course. The practical training will initially focus on Plain Film, but will include other areas such as CT and MRI.
As a trainee, you will join 2 existing Apprentices and work alongside Student Radiographers who are on the traditional BSc Degree course, also via Exeter University. Practical training is provided by TSDFT Radiographers who have been enthusiastically and successfully supporting such training for many years.

Job summary

Job summary
You will work collaboratively, under direct and indirect supervision of Registered Nurses as a member of the Lower Limb Therapy Service, to deliver gold standard leg ulcer treatments to patients. You will cover across TSDFT Outpatient Clinics
You provide leg ulcer management treatments as required, to include limb reshaping, delegated patient specific compression therapy, and measuring for compression hosiery/wraps as required. To assist in yearly well leg assessments for patients under the LLTS leg leg program

Main duties of the job
- Work as part of a peripatetic specialist team in Trust wide outpatient clinics delivering leg ulcer management treatments, to include wound bed preparation and compression therapy treatments.
- Be responsible in the preparation of clinics at the beginning and end of the working day. Complying with infection control protocol.
- Actively support patients encouraging self-care to prevent leg ulceration recurrence, post healing with the LLTS.

Job summary
The Torbay Adult Health and Social Care Team is recruiting Health and Social Care Co-ordinators to work Monday to Friday between 8am and 6pm, with some weekend and bank holiday working. Hours and working patterns can be discussed at interview.
The role involves acting as a single point of contact for community health and social care referrals, undertaking assessments, applying Care Act eligibility criteria, identifying risks, providing advice and signposting, developing support plans, and acting as a Keyworker.
Applicants should have excellent communication and IT skills, a good understanding of the Care Act and eligibility criteria, and experience working within a health and social care setting. The ability to prioritise workloads and produce clear, accurate records and reports is essential.
Please note:
We welcome applications from candidates who do not yet hold a relevant Health and Social Care qualification. Successful applicants may be appointed to a Band 4 developmental position, with full support to achieve the required qualification and competencies. Once these requirements have been successfully completed, the post holder will progress to a Band 5 role.
Main duties of the job
- To provide a consistent point of contact for members of the public or professionals. To undertake conversations to identify needs and risk and collate accurate and appropriate information to refer to the multi- disciplinary team or other Community Led and Voluntary/ Statutory services. To carry out social care assessments, apply eligibility and contribute to multi-disciplinary assessments in accordance to the Care Act 2014. To Provide advice and information where appropriate and to provide short-term management of a caseload as well as formulating support plans.

The HSCC Torquay Team respond in a timely and efficient way to the needs of our Community, by providing easy access to advice/information & support relating to Health & Social Care, including O.T., Physio, Community Nursing and Social Care enquiries and referrals for the Population of Torquay.
The team aim to deal with what they can today and only pass on work where necessary. They respond to incoming enquiries in relation to Community Health and Social Care needs, carry out social care assessments as well as identifying risk, priority and needs. The team co-ordinate hospital discharges for patients and the Intermediate Care caseload. The team work on a rota basis 7 days a week.

This challenging fast pace unit sees a combination of directly referred medically expected patients and those referred from our emergency department. This is an exciting opportunity to become part of the Obstetrics & Gynaecology team at Torbay and South Devon NHS Foundation Trust.

Main duties of the job

The Obstetrics & Gynaecology team is supported by a dedicated group of midwives, nurses and Resident Doctors. The unit includes a dedicated theatre within the labour ward and facilities for water births.

Gynaecology Services
The Women's Health Unit houses a Gynaecology Clinic which is fully equipped for colposcopy and minimally invasive surgical procedures. A separate, fully functional Urodynamic Unit provides specialised investigations and care.
The department runs an in-house Termination of Pregnancy Service, an Early Pregnancy Service and a Fertility Clinic, to ensure continuity of care across women's health.

We are seeking a qualified Reporting Sonographer to join Torbay South Devon Foundation Trust. This is an exciting opportunity to join the team and aligns with significant change in leadership in the Ultrasound team and wider Radiology management, driving the service forwards with ambitions to make significant quality improvements.
The preference is for full time employment working at Torbay and Newton Abbot sites, we are prepared to be flexible for the right candidate. This may be single site working or compressed hours.
You will work independently acquiring and interpreting diagnostic ultrasound images and then issuing clinical reports to referring clinicians. The role includes acute and elective general ultrasound scans and obstetric work and includes some weekends. You will be responsible for performing all designated ultrasound scans to the required departmental standards, following departmental protocols and national guidance/best practice.
Working in cooperation with Radiology support workers, sonographers, the lead sonographer and consultant radiologists, you will be undertaking delegated tasks as required, engaging in all four pillars of practice to contribute to a high-quality ultrasound service that is resilient, progressive and committed to providing the best care possibl

Main duties of the job
- You will be responsible for the safe and efficient use of expensive and complex technical equipment- each fragile transducer costs typically £6000 to replace and machine cost from £20,000-£120,00.
- Provision of a Sonographer led, diagnostic Ultrasound imaging for TSDFT Radiology department.
- Provide independent clinical opinion, via the CRIS system, on ultrasound examinations carried out, seeking senior colleague advice where required for management of patient care pathway.

About us
The Sonographer team consists of 12 WTE Radiographer and Midwife Sonographers, who undertake around 30,000 examinations/annum.
There are a total of 11 ultrasound rooms, 6 within the Women's Health Unit, 3 general ultrasound rooms in the main Radiology Dept and 2 at Newton Abbot Community Hospital. This service provides scanning support for the Acute medical, surgical, DVT, paediatric, Obstetric Gynaecology teams, GP, OP and IPs.
We have sonographers who are MSK specialists undertaking a wide range of diagnostic scans and therapeutic ultrasound guided steroid injections. One sonographer currently provides head and neck scanning and undertakes fine needle aspirations.
Our general Sonographer team also scan general paediatric patients, including baby hip screening. We have developed specialist training in Gynaecology service with endometriosis investigation which has vastly improved detection and avoids women waiting too long for treatment.
All general Sonographers take part in weekend and Bank Holidays working. The team are dedicated and proactive in providing a fluent service to meet the demands on service.
