As a Physiotherapist, you will play a key role in assessing, treating, and managing individuals with a wide range of conditions, helping them to achieve their maximum potential. This role offers a diverse caseload, working within our cutting-edge Neuro Hub and delivering high-quality care to clients in their homes and communities, and therefore a full UK driving licence and access to a vehicle is essential for this role.

We are delighted to offer this exciting opportunity, for an experienced nurse to join our Specialist Palliative Care Team (SPCT) as Clinical Quality & Standards Nurse Lead. SPCT currently consists of Hospital and Community Macmillan Palliative Care Nurse Specialists, Consultants in Palliative Medicine, and Admin and Clerical Team.
The SPCT provides specialist support, care and advice to patients and their families living with life limiting illness in North Wales, across primary and secondary care.
The post holder will support Head of Nursing and each of the Specialist Palliative Care Teams with overarching quality strategy, Datix, Complaints, Mortality reviews, quality and safety reporting and presenting in the absence of HoN/Team Leads at various meetings, supporting with departmental risk assessments and supporting the ongoing document library. The post holder will work closely with HoN/Team Leads to identify themes and support with developing Quality Improvement projects to improve care delivery and knowledge.
The postholder will be expected to travel across BCUHB to support the SPCT pan service as needed with a primary base in Wrexham Maelor Hospital.

Main duties of the job
- Hold a caseload of service users for up to 12 weeks (for Liaison & Diversion cases) and up to 6 months (for Reconnect cases) to support them to access a wide range of agencies and services that can provide help and advice.

Any request for specific working pattens can be considered in line with our flexible working policy.
This role operates across 9am to 5pm from Monday to Friday.

Liaison and Diversion (L&D) services identify people who have vulnerabilities when they first come into contact with the criminal justice system as suspects, defendants or offenders.
L&D services aim to improve overall health outcomes for people and to support people in the reduction of re-offending through assessing, proactive onward referrals and informing decision makers. It also aims to identify vulnerabilities in people earlier on which reduces the likelihood that people will reach a crisis-point and helps to ensure the right support can be put in place from the start.

Liaison and Diversion (L&D) services identify people who have vulnerabilities when they first come into contact with the criminal justice system as suspects, defendants or offenders.
L&D services aim to improve overall health outcomes for people and to support people in the reduction of re-offending through assessing, proactive onward referrals and informing decision makers. It also aims to identify vulnerabilities in people earlier on which reduces the likelihood that people will reach a crisis-point and helps to ensure the right support can be put in place from the start.
The successful applicant will work in the courts, prison and the community; working with people within the criminal justice system where vulnerabilities are suspected and further assessment is indicated to inform disposal of the case and to identify care pathways for appropriate onward referral/support.

GP Training Programme Director - Southampton (Mat Leave Cover)
The role of GP Training Programme Director is to work with and support the Heads of School & of Primary Care Dean in leading the delivery of a wide range of functions, aligned to the NHS England mandate. The Training Programme Director is professionally and managerially accountable to the Head of School. The Training Programme Director will focus upon improvement of learner supervision, assessment and experience, engagement of faculty and ensuring effective educational outcomes, both now and in the future. To support the Primary Care Dean & Postgraduate Dean, Training Programme Directors will work across the spectrum of health and where relevant, social care, within the context of a team, so that the provision of education reflects changing service models. This will deliver an integrated workforce comprising individuals from a spectrum of professional and other backgrounds.
Main duties of the job
The TPD role involves working with the local education team supporting Doctors in GP Training to perform and achieve their potential during GP Training hopefully graduating with MRCGP.
The role involves preparation and delivery of teaching sessions as well as pastoral support for GP resident doctors and individuals within the GP Training Programme. There is an expectation to work within the local programme team, help support trainers and be involved in the assessment process.
We are looking to recruit a TPD on 2 sessions for maternity cover for 4 months for the Southampton GP Training Programme.
Remuneration will for 2 sessions at GP00 point of national GP educator pay scale. These sessions are usually ascribed to the educational day release which is currently held on Tuesday and as Programme Director, you would be expected to be available to participate unless on leave.
Please note although based in the local programme there will be travel around the Wessex region when attending development days, Trainers conferences and meetings.

You will be working in our neuro stream and supported with regular clinical supervision and in service training from Clinical Specialists. You will assess and treat people in their own homes, out-patient clinics and using telephone and video calls, and will manage your own caseload of patients. You must be well organised and an excellent communicator and be willing to work flexibly. Remote working is part of this role, you will require a willingness to learn to use IT applications to carry out this job effectively. It is essential that you are a car driver and can travel independently between sites and to patient's homes.
Main duties of the job
You can expect to build links with tertiary, secondary and voluntary sectors as you support patients with neurological diagnoses e.g. local spasticity clinics. You will work closely and effectively with the wider Multidisciplinary team in the community, this may include; Orthotists, Wheelchair services and Social workers. The community neuro steam aims to provide high quality, responsive physiotherapy services, for patients with a wide range of complex neurological impairments including Parkinson's disease, Stroke and MS. Patients may require active rehabilitation, as well as disability management. We are a forward thinking service who welcome service development for the benefit of our patients, we welcome candidates who are like minded. Main duties of the job
We are looking for a Phlebotomist/ Care Co-Ordinator to join our team, working across sites to help improve health outcomes for our patients.
You must have excellent phlebotomy skills, excellent communications skills as well as patience and understanding, put patient care at the centre of everything you do and support the service as required.
Duties may be varied from time to time under the direction of the Lead Clinician/ Practice Manager, dependent on current and evolving Practice workload and staffing level.
The successful candidate must understand confidentiality as dealing with patients sensitive information and follow GPRD. Support the equality, diversity and rights of patients, carers and colleagues.

Job OverviewThis is an exciting opportunity to join the new Countywide Home First Front Door Service, launching on 1 September, and play a key role in shaping and supporting this innovative service from the outset. As part of the Home First Team, you will work within a multidisciplinary service that brings together Occupational Therapists, Physiotherapists, Therapy Assistants, Home First Assistants, Planners and Reablement Workers to support people across Gloucestershire to remain independent and receive care in their own homes whenever possible.
Working as the first point of contact for patients, carers, healthcare professionals and partner organisations, you will manage referrals and enquiries into the Home First service, ensuring they are handled safely, efficiently and directed to the most appropriate clinical team. This is a fast-paced and rewarding role requiring excellent communication, organisational and decision-making skills, with a strong focus on delivering high-quality, person-centred care and support within the community.
Main duties of the job
- Act as the first point of contact for Home First referrals and enquiries from patients, carers, healthcare professionals and partner organisations.
- Manage incoming calls and referrals, ensuring information is accurately recorded and processed in a timely manner.
- Support the safe and effective management of urgent referrals in collaboration with clinical leads.
- Use agreed pathways, protocols and decision-support tools to direct referrals to the most appropriate service or clinical team.

Main duties of the job

The Care Home Liaison Team is a small but very busy multidisciplinary team comprising of Community Psychiatric Nurses, an Occupational Therapist, a Support Worker and a Consultant Psychiatrist, providing a liaison service to more than 70 care homes across Kirklees. The team is committed to delivering a high standard of care to older adults living in care homes who have mental health needs.

Main duties of the job
Duties will include:
Answering and directing incoming telephone calls, taking messages, and handling queries effectively.
Minute Taking
Processing letters, Treatment Advice Notes and emails.

Communication: Act as a first point of contact for internal and external enquiries, ensuring professional and sensitive communication with service users.
Administrative Support: Provide comprehensive secretarial service including, audio typing, minute-taking, diary management, and handling correspondence.

Main duties of the job
In this rewarding role, you will work independently but under the expert guidance of a qualified Speech and Language Therapist. You will be a vital support link in the triage, assessment, and follow-up care for children referred to our community paediatric dysphagia services. You will also support babies who have spent time on neonatal or special care baby units, helping them and their families navigate early eating, drinking, and swallowing (EDS) challenges.
Your day-to-day work will be varied and impactful, involving:
- Gathering information from families and professionals directly and indirectly for the purpose of contributing to clinical documentation.
- Independently complete protocol driven feeding observations in a range of locations.
- Supporting Parent/carer education.
- Supporting families to implement specific home programmes supported by SLT.
- Completing operational tasks as required.

Job summary
One full-time vacancy have arisen within the Estates Department for a Multi-Skilled Maintenance Technician (Electrical) to provide technical support services for the reactive/planned maintenance work in a variety of settings to ensure compliance with all statutory requirements. Operating as a mobile Maintenance Technician, you will receive your work from the Estates and Facilities Helpdesk.
The Estates Management Team of 40 directly employed staff forms part of the Estates and Facilities Directorate within Nottinghamshire Healthcare NHS Foundation Trust. The Team provides planned/reactive maintenance, and emergency services 24/7 365 days a year to 54 buildings across Nottinghamshire utilising mobile engineers across all building disciplines, we ensure that the Trust's buildings are fit for purpose and compliant with current legislation. This is achieved by assigning jobs via the Facilities Helpdesk to our engineers out in the field onto their handheld devices.

An exciting opportunity has arisen for suitably qualified and experienced candidates who are registered with the NMC within our new Specialist Outreach Psychosis Service within the Borough of Ealing. Ealing is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users with complex psychosis who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support.
Successful candidates will show knowledge and experience of the clinical processes and treatments that support the care of people experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.
Experience of working with complex mental health needs and high risk clients is essential as well as strong skills in risk assessment, care planning and crisis management. Experience of community based and outreach working is also essential.
