Job summary
This is an exciting opportunity for an experienced Support time Recovery worker wishing to join the community mental health primary care team in Conwy. Applications are welcomed from enthusiastic practitioners who want to make a difference in primary care service provision. You will have excellent communication skills, be self-motivated, proactive and committed to promoting innovation for service delivery.
The successful candidate will support and facilitate the organization of clinical activity, assist in the delivery of group and individual work whilst continuing to support and deliver the clinical activity as required. The post holder will be required to work independently and liaise with various third sector agencies and organizations as well as carers. Excellent skills in multidisciplinary working and the requirement to establish and maintain excellent links with GP practices are essential.
You will have excellent communication skills, and the ability to build and maintain effective working relationships, demonstrating a 'positive attitude'.
Main duties of the job
- Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community.
- Support service users to meet their identified needs and outcomes, paying attention to and respect their values, wishes and aspirations to lead their lives in the way they choose, supporting them to access community resources and services, such as housing, benefits,leisure, education and employment.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job summary
This vacancy is for a permanent contract, Band 6 musculoskeletal physiotherapist working in Wrexham. You will have responsibility for supervising junior members of staff and students on placement.
You will join a large supportive innovative team.
The physiotherapy service in BCUHB offers a whole career pathway to Band 8A as a clinical advanced practitioner. Service improvement and development is encouraged and all staff have access to regular supervision, mentorship and training.
This is an opportunity to work in a large established and supportive team in a beautiful part of the UK.
Part time applications will be considered.
Main duties of the job
- To carry out physiotherapy assessments and treatments; developing individualised therapeutic treatment programmes for a specialised caseload in the relevant clinical area. Patients may include those with physical and psychological problems in area of speciality; some of whom may have a level of complexity. Team leader; clinical specialist or advanced practitioners are available for clinical and managerial support.
- To ensure a high standard of care for each patient working with patients, families or carers, multi-agency teams and disciplines involved.
- To communicate with these patients, carers, agencies and other disciplines in an appropriate manner, often with sensitive information, diagnoses or outcomes.
- To supervise, teach and appraise peers, physiotherapists, physiotherapy assistants, technical instructors, students and other members of the multidisciplinary or multiagency teams.
- To train and supervise patients, families or carers and staff from other disciplines or agencies to carry out programmes of care devised by the therapist.
- To deputise and support the Clinical Team Leader as required in the daily management of the designated team.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Stourside Medical Practice is based in the centre of Halesowen and supports the health needs of 6000 patients . We require an enthusiastic experienced and flexible Receptionist in a temporary role which may be able to be extended on a month by month basis after the 3 months. We are delighted to advertise two Psychologist/Psychotherapist posts, working with people in the gaps between NHS Talking Therapies (formerly IAPT) and community mental health services. We have a 1.0 wte post based in the West of Leeds and a 0.6 wte post based in the East.
The post holder will be part of the Primary Care Mental Health Team which is a multidisciplinary service that works closely with GP practices using a holistic person-centred model of care. The postholder will work alongside psychological therapists and other colleagues from the NHS trusts and a range of third sector partners. The aims of the service are to:
Provide therapeutic input (individual and group) for people presenting with complex needs. Clients may present with co-occurring physical, social and mental health issues and/or neurodiversity-related issues, requiring a flexible, multi-modal approach.
Work proactively to ensure that our services are acceptable and accessible to people from minoritised backgrounds who may not be well served by standard service models.
Provide psychological consultation and supervision to primary care staff, including GPs and other allied professionals.
Work collaboratively to embed a flexible, psychologically informed model of care.
Main duties of the job
The postholder will work closely with the Consultant Clinical Psychologist and the senior psychological therapists in each cluster to develop a flexible and creative therapeutic offer, tailored to individual need. A particular focus will be on working to address health inequalities and ensuring equitable access to psychological support and interventions.
This is an exciting time to be joining this service as we have received significant investment in psychological therapies, in line with the NHS long-term plan and the national programme of community mental health transformation. The aims of the new transformed services are to foster integration between the NHS, the third sector and social care and to increase access to specialist interventions, including psychological therapies. These roles are 'early implementer' roles that will play a key part in piloting and evaluating this evolving service.
In addition to direct clinical work, the role will involve supporting the work of primary care mental health workers through supervision, consultation, and training. It will also involve collaborating with other stakeholders and third sector providers to develop effective pathways and networks.

Job summary
The post holder will be responsible for undertaking clinical/administrative duties relating to immunisation, primarily delivered on site for primary and secondary children in schools and in community clinic settings across Brentwood & Billericay.
Main duties of the job
- Vaccinate 20 to 25 school children per hour (Flu season).
- Work collaboratively to support the team, organisation, and company stakeholders, to facilitate the smooth continuity of service delivery.
- Promote and support the operation of our School Aged Immunisation Service.
- Support Clinical Lead, Team Lead and Programme Managers as applicable.
- Attend sites across different locations as directed by your line manager.
- Deliver an effective, evidence based and competent level of care to patients.

The RRDNs have three key roles:

This includes bringing research to under-served regions and communities with major health and care needs. Working jointly with the Coordinating Centre in the strategic oversight of the NIHR RDN. This ensures that the Portfolio is maintained as a cohort of high-quality, fully funded, viable and deliverable studies. This role offers an exciting opportunity for a motivated Occupational Therapist to join Bromley's evolving Experts at Hand (EaH) model and play a central part in the borough's SEND transformation. You will be part of a multidisciplinary team working to strengthen early identification, targeted intervention and inclusive practice across education and early years settings. As a School-Based OT, you will help shape a new, prevention-led offer that brings specialist expertise closer to classrooms, families and school communities. You will deliver evidence-based occupational therapy within a tiered Universal-Targeted-Targeted+-Specialist framework, providing coaching, consultation and modelling of OT strategies in real school environments. Day-to-day practice includes supporting whole-school approaches, delivering targeted interventions, implementing therapy programmes, and collaborating with teachers, SENDCOs and families to improve functional outcomes for children and young people. You will hold a targeted+ and specialist caseload, completing assessment and intervention in context for children with occupational performance needs and functional difficulties.Main duties of the job
1. To work autonomously and without direct supervision in a defined clinical area, recognising professional boundaries and seeking guidance where necessary, but having accountability for own clinical actions.
2. To be responsible for a clinical caseload of paediatric occupational therapy including assessment, interpretation of assessment results, evaluation, treatment, and advice, through the use of evidence-based practice and outcome measures.
3. To work with education partners using evidence-based approaches such as coaching, modelling and guiding to enhance capacity in the educational context and support children's functional participation as part of the OT offer.

Job summary

This advert is recruiting for a Peer Support Worker (PSW) within the Oxford Health Buckinghamshire Individual Placement & Support Service (IPS). Peer Support Workersare people who have their own lived experience of mental health challenges or have supported/cared for someone with mental health challenges. They use empathy informed by their own experiences to support service users/families who are accessing mental health services. Peer Support Workers model the recovery principles together with personal responsibility, self-awareness and self-care whilst undertaking the duties of the role. The IPS service knows that being in employment can positively impact a person's recovery and wellbeing. The Individual Placement & Support service supports people receiving care from Oxford Health mental health teams who are unemployed and motivated to find paid employment, or whose current employment is at risk. Main duties of the job
We're looking for people who are compassionate, can work well in a team, highly motivated and have excellent written and verbal communication skills. You will be provided with training for the role and given the opportunity to complete the national recognised care certificate.
Oxford Health offer the Peer Support Training programme as a nationally recognised accredited Level 3 Apprenticeship (fully funded by Oxford Health) for those that are eligible and interested in completing a qualification in addition to their in-house training. As the first point of contact for our patients, Care Navigators play a vital role in supporting people to access the care and services they need. This is a busy, but rewarding role working as part of a supportive team. Every day brings opportunities to help people navigate the healthcare system, access the most appropriate service.
This is a part time role of 34.75 hours per week with a working pattern of 4 days as below:

To ensure enhanced patient access to the practice we ask our Care Navigators to work around 1-2 Saturdays shifts per year.

Individuals may present with long-standing acquired brain injury, cognitive impairment, dementia or other neurodegenerative conditions, sometimes alongside complex physical health needs. Afon Las will provide a structured and therapeutic hospital environment in which individual needs and risks can be safely supported. Referrals may be received from a range of pathways, including community, neurosurgical, neurorehabilitation and custodial settings.
Through interdisciplinary working, the service will maximise person-centred outcomes, physical function, mobility, independence and quality of life, while keeping each individual at the heart of their care and rehabilitation.
The dedicated clinical team will provide interdisciplinary input, including occupational therapy, physiotherapy, speech and language therapy and nursing.

The service will provide specialist assessment, treatment, rehabilitation and longer-term support for individuals with complex and enduring neurocognitive needs who may be unable to access more traditional neurorehabilitation services or live safely and independently within the community.Individuals may present with long-standing acquired brain injury, cognitive impairment, dementia or other neurodegenerative conditions, sometimes alongside complex physical health needs. Some may also have complex risk histories, including previous offending, and may be subject to Ministry of Justice restrictions.Afon Las will provide a structured and therapeutic hospital environment in which individual needs and risks can be safely supported. Referrals may be received from a range of pathways, including community, neurosurgical, neurorehabilitation and custodial settings.Through interdisciplinary working, the service will maximise person-centred outcomes, occupational participation, independence and quality of life, while keeping each individual at the heart of their care and rehabilitation.As the Occupational Therapy Assistant, you will provide direct clinical interventions delegated by the Highly Specialist Occupational Therapist. You will contribute to the assessment process, deliver and review graded occupational interventions, and support individuals to work towards their agreed care and rehabilitation goals in collaboration with the interdisciplinary team.Occupational Therapy DirectorateUnder the leadership of our occupational therapy directors, we support over 165 Occupational Therapists.
