As an Advanced Nurse / Clinical Practitioner, youll be part of a dynamic, multidisciplinary team working within a telephone and digital triage environment. Youll help ensure patients receive the right care, in the right place, at the right timereducing unnecessary hospital visits and improving outcomes across the system.
Practice Plus Group is one of the UKs leading independent healthcare providers, and were proud to support the NHS with innovative services that put patients first.
Our OOH service is open from 18.30-08.00 Monday to Friday and 24 hours over the weekend. We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you.

Main duties of the job
Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs.
Making safe, evidence-based decisions to direct patients to appropriate services, including self-care, primary care, urgent treatment centres, or emergency services.
Applying advanced clinical knowledge to assess undifferentiated presentations and manage risk effectively.
Supporting clinical governance, contributing to service development, and mentoring colleagues.
Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
About us
Registered Nurse or Paramedic with MSc in Advanced Clinical Practice or equivalent
Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre
Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.
Ability to autonomously see and treat patients of all ages
Confident clinical decision-maker with strong assessment skills.
Excellent communicator with a patient-centred approach.
Flexible, self-motivated, and committed to ongoing professional development.
Live within a 1-hour commute to site in Ipswich

Are you an experienced Healthcare Support Worker looking for a new challenge in a community setting? Come and join our Community Care Night Service Teams and help us to continue providing high quality and innovative care to the people of Harrogate and Rural area, in the comfort of their own homes.
As a Community Healthcare Support Worker with Harrogate and District Foundation Trust, you will be based in one of our four Community Care Teams. The night service runs between 19.45 and 07.15 every day of the year.
Whilst we are able offer flexible shift patterns, please note that this role is nights only.

Main duties of the job

- To assist in planning, delivering and evaluating nursing care and rehabilitation programmes to address peoples' health and social needs according to their care plans.

Trainee Clinical Respiratory Physiologist (Band 4)
The post of Trainee Clinical Respiratory Physiologist is an exciting opportunity for an individual whose ambition is to become a Clinical Respiratory Physiologist. Our Respiratory Physiology Department undertake a wide variety of tests including routine and complex lung function, sleep and ventilation. You will be provided high quality in-house and external training with the opportunity for progression to Band 5 and Band 6 Respiratory Physiologist grades, upon successfully completion of, and gaining competency in, incremental training goals.
You will be self motivated and have excellent interpersonal skills. You will have the ability to work under pressure and at times autonomously, as well as be able to work effectively within a team..
The post requires cross-site working (Luton and Dunstable Hospital and Bedford Hospital). You must have a BSc degree Human Physiology or Sports Science or equivalent and ideally relevant NHS healthcare experience.

Main duties of the job
Main Tasks & Responsibilities:
1. To train to become proficient in performing the physiological tests listed below.
2. To perform all relevant quality control procedures within the department.
3. To help maintain departmental equipment in conjunction with more senior staff and the Clinical Engineering Department.
4. Sterilise and disinfect all departmental equipment as and when necessary using correct guidelines set out by the trust.
5. Perform administrative duties such as sending out patient letters, telephoning patients regarding their tests and answering patients via telephone.
6. Ensure all patient data is stored in a suitable manner in accordance with Trust policy.
7. To take personal responsibility for compliance with all Trust and Departmental policies and procedures.

Job summary
Working in the Histocompatibility & Immunogenetics (H&I) department, you will coordinate the provision of HLA selected products throughout the UK when requested. Supervise the work of assistant HLA selected product coordinators, ensuring a safe, reliable, and efficient service for patients requiring these blood products.
The role involves communicating with NHS Blood and Transplant (NHSBT) consultants, Hospital clinicians, hospital transfusion departments, and blood services in Northern Ireland, Scotland and Wales regarding the treatment of patients requiring HLA selected blood products.

Main duties of the job
Key duties and responsibilities for the role include:-
- Liaise with NHSBT departments to provide information on HLA selected product availability and provision.
- Communicate sensitive information regarding patient diagnosis and treatment internally and externally.
- Manage queries and complaints regarding product provision, escalating to scientific colleagues where appropriate.
- Use information from the different departments within NHSBT and hospital demand to ensure the timely and continued supply of HLA selected products.
- Collate and analyse data related to products issued and interaction with hospital users to improve user experience and efficiency of service to patients.
- Analyse trends in hospital interactions to assist in decisions on what data are compiled.
- Co-ordinate the movement of HLA selected blood products between NHSBT sites and between Uk Blood Services.
- Use planning and organisational skills to ensure Service Level Agreements for the provision of HLA selected products are met.
- Participation in NHSBT public holiday planning meetings to ensure the provision of HLA selected products is maintained, and product wastage is avoided.

Job summary
We are looking for a medical summariser to join our team for 20 hours per week.
We are looking for someone that is able to use the Emis system and has done the role of summariser before.
The candidate we are looking for will be motivated and able to work under pressure. They must be able to work well on their own, but also be able to work as part of the larger team.
Main duties of the job
The role will be to review patients medical records and produce an accurate summary of the patients medical summary - ensuring that the appropriate read codes are added.
The role will also include:
- Receiving the medical records into the surgery, ensuring that they are stored correctly
- Recording immunisation details for each patient, as well as any other important information that needs to be coded
- Scanning and filing paperwork/notes
- Providing information where necessary to the clinical team
- Dealing with documentation that arrives in the surgery via post and electronically

About us
Our practice is a busy training practice that looks after around 12,500 patients across 2 sites as we have a smaller surgery at Barugh Green. However, the post holder will be based at Huddersfield Road Surgery.

An exciting opportunity has arisen to join the ward 11 team. Ward 11 is a busy 25 bedded ward that specialises in stroke and medical patients. We work closely with other members of the multi-disciplinary team to provide quality, planned care for the patients. The successful candidates will work as part of a team of nurses responsible for providing, maintaining and co-ordinating a high standard of planned holistic care and nursing treatments, under the indirect supervision of the nurse in charge of the ward.
You will need to be flexible and dynamic in your approach in delivering care to patients. The post holder needs to have good communication skills and be an effective team player.
Main duties of the job
The role involves assisting trained staff in delivering care to patients at their most vulnerable times, ensuring dignity and respect is maintained throughout. Assisting patients to meet their personal care needs, assisting with meals and drinks throughout the day, recording observations and completing bedside documentation. Health Care Support Workers are also required to carry out general cleaning in the clinical environment by preparing bed spaces and cleaning equipment. The post holder needs to have good communication skills and be an effective team player. Staff will be supported to complete mandatory training.

Living Well Consortium is recruiting for Trainee Children's Wellbeing Practitioners (CWPs) to work within their NHS Talking Therapies service. The Traineeship involves teaching with University of Northampton as well as self-directed study days and a placement with Living Well Consortium. The Traineeship will start in January 2027.
The Trainee CWP role will include brief therapeutic early interventions for children and adolescents with mild to moderate anxiety and depression as well as parents of children experiencing mental health problems. The role also involves preventative work within communities, supporting mental health promotion in community settings though psychoeducation, training, group work, and supporting CYP/parent participation.
Training delivered by the University of Northampton is a combination of face-to-face, remote/online teaching, and university directed independent learning sessions. The Traineeship is assessed in numerous ways including module tests, video recordings of clinical assessments and intervention sessions, written work, case reports, presentations, and submission of portfolios.
The Traineeship is one year long, and, at the end of the programme, successful candidates will be fully qualified CWPs.
Successful candidates will be employed with a Living Well Consortium Service Provider.

To participate in teaching led by Northampton University and successfully complete the Postgraduate Diploma in Children and Young Peoples Wellbeing Practice (CWP) training.

Due to internal promotion, we are now looking to recruit Care Navigators to join our team on a permanent basis.

Main duties of the job
The core function of this post is to support the Treating Tobacco Dependency (TTD) Programme within University Hospitals Plymouth (UHP) inpatient services.
The postholder will also contribute to workstreams across the wider Wellbeing Team, providing an exciting opportunity to work across both secondary care and community settings.
As part of the national drive towards a smokefree nation, this role will focus on smoking cessation, particularly among marginalised groups and individuals with complex needs. You will influence, support and promote positive lifestyle choices and behaviours, leading to improved health outcomes and a reduction in preventable ill health.
The role involves working with a diverse range of clients in a variety of settings, using different interventions and approaches to identify potential risks to health and wellbeing. Wrexham (East) CAMHS are seeking an experienced Family Therapist to join our service. You will have a main base and work across our other base, as needed, alongside our other senior Family Therapist.
This is a full-time, permanent post. Relocation packages are available.
You will deliver high quality family therapy to children, young people and families experiencing a wide range of mental health difficulties, alongside a multidisciplinary team. We are seeking a dynamic Family Therapist to have a key role in leading and developing our systemic and family therapy offer across East CAMHS.
Wrexham CAMHS offers a 0-18 service to children, young people and families. We provide an extensive offer to our local community including significant early intervention and prevention services, unscheduled care, an Early Years 0-7 service and a broad range of NICE recommended and other evidence based interventions. Significant joint working takes place with multi-agency partners with whom we have very good relationships.
Regular in-service training takes place, alongside weekly peer case discussions, in addition to multidisciplinary team meetings and team away days.
We welcome applications from those newly or recently qualified as these posts can be offered, based upon experience, at a Band 8a or Band 7 Preceptorship arrangement.
Main duties of the job
The successful candidate will have a significant role in the provision of family therapy, offering clinical supervision and consultation, developing and leading on appropriate training, in addition to being a key member of our multidisciplinary team to support our systemic thinking and formulations. Involvement in developing and maintaining clinical skills within the wider team and service development will form a key part of this role. Regular supervision will be provided.

Job description
Job responsibilities
- Help collate referrals, make initial contact, and triage participants to the correct service, complete triage calls for all services within Wellbeing Hub.
- Work closely with GPs and primary care professionals to identify and manage a caseload.
- Demonstrate flexibility/adaptability in a dynamic, evolving service.
- Develop trusting relationships with participants; take a holistic, person-centred approach.
- Support development of Personalised Care and Support Plans, liaising with practice teams, patients/carers, and the Neighbourhood Team.
- Proactively outreach to participants on a regular, agreed basis.
- Use knowledge of local services for signposting/referrals.
- Support PCN targets for numbers seen by a Social Prescribing Link Worker and Health and Wellbeing Coaches
- Scope and map local services (welfare, employment, healthy lifestyle, social, support); maintain databases of services/activities.
- Engage other services to raise awareness of The Wellbeing Hub.
- Manage effective use of resources/services for consistent quality and quantity of care.
- Adopt multi-disciplinary, multi-agency approach.
- Know when to refer to other health professionals/agencies.
- Accurately maintain participant records.
- Manage waiting lists (if applicable).
- Understand barriers to accessing community support and use insight to shape support.
- Promote the service within the PCN (to users and clinicians); build positive working relationships.
- Reflect on and evaluate the service for continual improvement.
- Communicate effectively with colleagues, patients, and carers.
- Coordinate/navigate care for people and carers across health and care services, working closely with link workers, health and wellbeing coaches, and other primary care roles.
- High level of written/verbal communication skills.

- Support with smooth running, organisation, and facilitation of The Pain Cafe.
- Hold regular triage meetings with the Mental Health social prescribing team
- Ability to travel across the local area when required.
