Job summary
We are looking for a compassionate and motivated Link Worker to join our friendly Social Prescribing team.
We have an exciting opportunity as a part time (20 hours per week) Gambling Harms Link Worker on a 12 month contract, with a possible further 12 month extension, dependant on funding.
Gambling can seriously impact people who are harmed by it, including mental wellbeing, finances and relationships. In this role, you'll work closely with GP practices within Primary Care Networks (PCNs) and the wider multidisciplinary team offering training to professionals and community groups, whilst supporting people whose health and wellbeing are impacted by gambling.
You will support patients aged 18+ navigate non-medical needs and explore "what matters to the patient" looking at supporting behaviour change and connect patients to empowering local community groups and support networks.

Main duties of the job
The Gambling Harm Link Worker will lead the delivery of the Stockport Gambling Harm Partnership, supporting individuals, families and communities affected by gambling-related harm.
The post holder will provide prevention, early intervention and recovery support through a social prescribing approach, whilst embedding gambling harm awareness and identification across primary care, community organisations and voluntary sector services.
The role will work closely with GP practices, Primary Care Networks, Social Prescribing Link Workers, Health Coaches, Care Coordinators, community organisations and specialist gambling support services to improve awareness, increase referrals and strengthen local support pathways.
The post holder will also contribute to community engagement, workforce development, monitoring and evaluation activities to support a whole-system approach to reducing gambling-related harm across Stockport.

Job summary
We are looking for a proactive and skilled Practice Support Pharmacist to join our PCN teams, providing expert medicines optimisation, conducting structured medication reviews, supporting safe and evidence-based prescribing, and collaborating with GPs, community pharmacies, and other healthcare teams across Stockport to improve patient outcomes. We support patients and care settings to make best use of medicines, ensuring safe and effective repeat prescribing.
Our service operates between 8am and 6.30pm,with working patterns typically comprising a mix of8am-4pmand 10:30am-6:30pm shifts.

1 x 12 month fixed term position.
This is full time (37.5 hours) and is available at (equivalent to) AFC Band 7. Salary is usually at minimum scale but may be higher dependent on primary care experience.

Main duties of the job

Ensure that effective and relevant analytical information is provided regularly to prescribers to support quality and cost-effective prescribing decisions. In this role, you'll work closely with GP practices within PCNs and the wider multidisciplinary team to support people whose health and wellbeing are affected by social, emotional or practical issues.
You will help patients aged 18+ navigate non-medical needs such as long term conditions, loneliness, mental wellbeing, debt, housing concerns, or physical inactivity. Rather than focusing purely on medical symptoms, you will give people the time and a safe space to explore "what matters to the patient" and connect them to empowering local community groups and support networks. You will also have line management responsibilities within this role.

High Lane Medical Centre is situated in a semi-rural area, 10 mins from Stockport and 30 mins from Manchester. Job summary
We are looking for a proactive and skilled First Contact Physiotherapist to join our team, providing expert patient care with exemplary MSK knowledge, diagnostic experience, and management skills based on best current evidence. You will excel in verbal and non-verbal communication and collaborate closely with GPs, and other healthcare teams across primary care in Stockport.
We provide a face-to-face, in-house FCP service to all PCN's within the Stockport borough and also hold the specialist spinal service for the area requiring expert knowledge in clinic diagnosis, imaging and management of complex spinal conditions. You will have or be expected to work towards MSK injection therapy and Non-medical prescribing qualifications with clinical application.
The team solely consists of Specialist level physiotherapists and Advanced musculoskeletal practitioners working autonomously in GP practices, with weekly supervision and training sessions and a pro-rota allocation of Learning and development days per year. We are part of a GP federation (Viaduct care CIC) and align our pay grades to agenda for change and offer the NHS pension scheme, we also offer flexible working patterns within the hours of the GP practices.
This is a 9month fixed termvacancy at an equivalent to AFC Band 7, salary is usually at minimum scale but may be higher dependent on experience. The post is Part-time (30 hours per week).

Main duties of the job
You will work closely with GP Practices to include First Contact Physiotherapy as part of our patient offer. This will include seeing patients, without prior contact with their GP, to establish a rapid and accurate diagnosis and management plan and is an ideal role for a motivated individual who is passionate about delivering high quality, joined up clinical management of musculoskeletal conditions.
Patients are signposted directly to the FCP clinics by GP reception and practice staff and you will be providing frontline expert screening, assessment and management including diagnostics and injection therapy.
Regular clinical supervision, mentoring and professional development is a key part of the service in keeping with the FCP road map and MSK competency framework; weekly team meetings are diarised with in- service training and case discussion.

Job summary
We are looking for a proactive and skilled First Contact Physiotherapist to join our team, providing expert patient care with exemplary MSK knowledge, diagnostic experience, and management skills based on best current evidence. You will excel in verbal and non-verbal communication and collaborate closely with GPs, and other healthcare teams across primary care in Stockport.
We provide a face-to-face, in-house FCP service to all PCN's within the Stockport borough and also hold the specialist spinal service for the area requiring expert knowledge in clinic diagnosis, imaging and management of complex spinal conditions. You will have or be expected to work towards MSK injection therapy and Non-medical prescribing qualifications with clinical application.
The team solely consists of Specialist level physiotherapists and Advanced musculoskeletal practitioners working autonomously in GP practices, with weekly supervision and training sessions and a pro-rota allocation of Learning and development days per year. We are part of a GP federation (Viaduct Care CIC) and align our pay grades to agenda for change and offer the NHS pension scheme. We also offer flexible working patterns within the hours of the GP practices.
This is a Permanent vacancy at an equivalent to AFC Band 7, salary is usually at minimum scale but may be higher dependent on experience. The post is Full-time (37.5 hours per week).

Main duties of the job
You will work closely with GP Practices to include First Contact Physiotherapy as part of our patient offer. This will include seeing patients, without prior contact with their GP, to establish a rapid and accurate diagnosis and management plan and is an ideal role for a motivated individual who is passionate about delivering high quality, joined up clinical management of musculoskeletal conditions.
Patients are signposted directly to the FCP clinics by GP reception and practice staff and you will be providing frontline expert screening, assessment and management including diagnostics and injection therapy.
Regular clinical supervision, mentoring and professional development is a key part of the service in keeping with the FCP road map and MSK competency framework; weekly team meetings are diarised with in- service training and case discussion.

Job summary
High Lane Medical Centre is looking for an experienced receptionist/administrator to work in a busy GP surgery based in High Lane. Previous experience is desired along with experience of practice IT systems; EMIS, DOCMAN, ACCURIX and TQuest.
Minimum of 24 hours over three days; Mondays, Thursdays and Fridays.
Mondays 8-1pm, Thursdays and Fridays 8-6pm with half hour lunch.

Main duties of the job
As first point of contact for patients and visitors. The receptionist managesthe front desk and greets all visitors to the surgery, answering any queries and enquiries and taking messages for the wider team. Answering the phones, scheduling appointments and monitoring the patient flow to the consulting and treatment rooms. They must demonstrate confidence, have a pleasant personality and possess good communication skills, creating a positive first impression.

We are a forward-thinking training practice, supportive of continued education, personal development and focused on providing high quality patient care to approx. c.7,500 patients. You will be joining an enthusiastic, well led and innovative practice team comprising: a principal GP, eight assistant part-time GPs, practice manager, six receptionists, 2 administrators, two secretaries, two practice nurses and a healthcare assistant. We have 2 small residential care homes and a large number above average of elderly patients.
Our local federation is Viaduct Care who provide us with a pharmacy hub team, seasonal respiratory hub clinics, First Contact Practitioner (physio), Well-being Coordinator, Mental Health practitioner, and additional care coordinators. We host other services, in house; minor surgery, Midwifery services, Ultrasound, Audiology, joint injections, coil insertions and implants.
We were ranked No. 1 surgery in England in 2022. Our last CQC rating was GOOD in 2020. We were recently ranked joint number 1 practice in Greater Manchester, and 12th In England. We are an active member of our local Primary Care Network which includes practices from Marple, Romiley and Hazel Grove.
