Job summary
The Experienced General Practice Nurse plays a pivotal role in delivering safe, high-quality, and person-centred care. This role is integral to achieving the objectives of the 2026/27 Network Contract DES, with a focus on continuity of care, health inequalities reduction, digital transformation, and long-term condition (LTC) optimisation.
Full-time (37.5 hours per week), or part-time (minimum of 20 hours per week) considered in line with service requirements.
This role is included within the extended hours provision and will involve working an evening on a rota basis.

Main duties of the job
The postholder will work as part of the existing practice nurse team on long-term condition management, champion public health and prevention, and embed population health principles within general practice.
They will support the development and supervision of new-to-practice nurses and other multidisciplinary staff, contributing to workforce capability across the Primary Care Network. The role also involves fostering integration with community, secondary, and social care systems to ensure proactive and joined-up care delivery.
The post holder will be working at registered nurse level practice as detailed in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.
One of 13 PCNs in Somerset, we are a collaboration of five GP practices in West Mendip: Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice, Wells Health Centre and Vine Surgery Partnership (Street).
At Wells City Practice, you'll be joining a friendly, supportive team committed to providing high-quality patient care. Serving around 8,500 patients, the practice has a 'Good' CQC rating, high levels of patient satisfaction and strong QOF achievement.
Our multidisciplinary team includes six partners, two salaried GPs, a paramedic practitioner, nurses, HCAs, FCP, Clinical Pharmacist and an experienced administrative and management team. We are actively involved in training, research and collaborative working through our local Primary Care Network. We offer opportunities for professional development and training, alongside flexible working arrangements that support a positive work-life balance.

Job summary

Main duties of the job
The post-holder will provide clinical expertise as an integral member of the Wells Health Centre multidisciplinary team, to assess, diagnose, treat, refer or signpost patients/service users, who attend the surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
A paramedic is authorised to undertake treatments and procedures as directed by the GPs for which they are trained in, and which are within the Royal College of Paramedics guidelines and in line with the HCPC standards framework. The post holder will use advanced clinical skills to provide education to patients and service users, promoting selfcare and empowering them to make informed choices about their treatment.
The post-holder will also perform any other clinical and administrative duties, where trained and determined safe to do so by the partners. This may include assisting with medical reports, safeguarding, and clinical support.
Through these activities, the post-holder will support the delivery and development of high-quality care within Wells Health Centre while contributing, where appropriate, to relevant West Mendip PCN priorities and services.

Job responsibilities
- Assess and triage patients, using same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the Practice, outside agencies and the PCN team.

Job summary
As a Health Connector, you'll play a key role in the PCNs social prescribing service, supporting people across West Mendip to improve their health, wellbeing and independence through non-clinical, community based support.

Main duties of the job
You'll work directly with individuals and groups, offering time, space and practical support to help people identify what matters to them and access services, activities and resources that can make a positive difference. This includes one-to-one support, facilitating group based wellbeing activity, and helping people take small, achievable steps towards improving their physical, emotional and social health.
The role involves managing a varied caseload, working closely with GP practices and the wider PCN team, and contributing to group sessions that focus on wellbeing, building healthier habits and supporting people with ongoing or pain related conditions. You'll also help strengthen local connections by promoting community resources, contributing to the development of new opportunities, and supporting a prevention focused, neighbourhood approach.
This role forms part of the PCNs social prescribing service, supported by the locally commissioned Health Connections model.

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations.
