An exciting opportunity has arisen for a Therapy Assistant Practitioner to join a forward thinking and dynamic Community Stroke Team within Heywood, Middleton, and Rochdale.
We would welcome applications from an organised and motivated person that has experience of working with individuals post Stroke, either in a community or inpatient setting.
The ideal applicant would be passionate about post-Stroke recovery and enthusiastic about rehabilitation to help Stroke survivors achieve their goals, works well as part of a team and can contribute to innovative ideas on clinical pathways and can help drive forward excellence.
We are a dynamic and welcoming MDT where a supportive learning environment and continuous professional development is actively encouraged. Regular supervision and clinical support are provided both formally and informally.
The team provide goal orientated Stroke rehabilitation within the community setting to both acute stroke presentations through their hospital discharge pathway and those with stroke specific long-term rehabilitation needs.
You will need to be able to work flexibly, hold a full UK driving license and have access to a vehicle.
This is a full-time position advertised at 37.5 hours per week however, applications from suitably experienced candidates who are seeking a minimum of 30 hours per week will be welcomed.
Main duties of the job
Work as an active member of the Stroke Team assisting in the delivery of integrated rehabilitation in the Heywood, Middleton and Rochdale area, supporting qualified therapists (occupational therapists, physiotherapists and speech and language therapists), and specialist nurses in assessing and treating individuals post stroke.
Support implementation of therapy-specific programmes devised by qualified staff that works towards the individual's goals. This may be intensive, daily or weekly and may include multiple therapy-specific programmes within a treatment session. Support social inclusion as part of rehabilitation that promotes independence and improvements in health and well-being.
The post holder will:
- Use technical and creative skills in the provision of therapeutic activities for individual patients and will be responsible for evaluating and amending treatment programmes and interventions within guidelines and competency frameworks supervised by the qualified staff.
- Will take responsibility for their own workload within the framework laid out by qualified staff.
- Will be responsible for delivering defined therapy input without direct supervision and will have access to a therapist for advice and direction as required.
The work will take place in a variety of settings including, in-reach to hospital, patients' own homes and other community settings often as a lone worker.

Job summary
We are looking for registered health or social care professionals to work in a team in a fast-paced criminal justice environment. You will need to be a registered NMC, SWE, HCPC or OT with significant Post-registration experience in mental health.
For this post, you will work in police custody and complete triage assessments with people whilst they are detained there (from the age of ten). You will work closely with the police, your assessment and information gathering will support that person across the offender pathway. Depending on a person's criminal justice journey this may include Courts and Probation and the wider health and social care systems in the community.
In this role your criminal justice knowledge will greatly expand, so we welcome applicants who have not worked in this area as we can provide training and your knowledge will develop as you work across the team.
Main duties of the job
We are looking for 1x Secondment/Fixed Term position for six months across the Norfolk Police Investigation Centres. Your primary role will be to complete holistic triage assessments for people who have been arrested and brought to police custody. You will possess excellent interpersonal skills that will enable you to engage, challenge, motivate, influence, and inspire at all levels. As part of a small team, you will work various shift patterns comprising of mostly 9-hour shifts, to enable the service to cover extended hours, seven days per week, 365 days a year.
The working criminal justice locations include Police Investigation Centres (PIC's), Courts and other supporting criminal justice/health environments in line with service need. However, this role will primarily work inside the PIC (custody). The service covers both Norfolk and Suffolk and may require some cross county working. It is essential that you can travel independently.
Travel to other PICs may be necessary from time to time. You may also be required to complete remote telephone assessments depending on service needs at other PICs located further away.

- Accurately record and feedback findings, identifying any complex issues or changes that require further assessment, intervention, or review by a qualified clinician.
Therapy Service Development and Sustainability
- Contribute to and support any projects for improving the Therapy Service and the development of the therapy group programme and seek out and develop potential collaborations with other services within the organisation eg: art and music projects.

- Support individuals to practise and consolidate therapeutic strategies developed by qualified therapy staff, such as memory aids, anger management techniques, anxiety management strategies, and other rehabilitation approaches. Facilitate the application and generalisation of these strategies across a range of environments, including activities in the day service, at home, community settings, educational placements, and other relevant contexts. Monitor progress, provide feedback on effectiveness, and identify any difficulties or emerging needs that require review by a qualified.
- Contribute to the evaluation of therapeutic interventions using appropriate outcome measures, adjusting interventions as appropriate and working towards achieving personalised goals.

Job summary
Are you passionate about promoting the welfare and safety of adults, children and young people, and survivors of domestic abuse? An exciting opportunity has arisen for a registered health professional with experience in safeguarding to work in Nottinghamshire Countys Multi Agency Safeguarding Hub (MASH).
The post will provide the successful candidate the opportunity to work within a multi-agency team in partnership with a range of agencies from across the safeguarding partnership, such as the Police and Adult and Childrens Services.
You will coordinate and undertake operational management within the MASH, sharing relevant health information to help ensure that adults, children and young people in Nottingham City receive the right help at the right time.
The post holder will need to have robust safeguarding knowledge, and a solution-focused approach which will enable them to develop effective professional relationships with safeguarding partners. You will have experience working directly with adults, children and families - applying safeguarding policy and processes.
Interviews are expected to be held on 8th October 2026.
Main duties of the job
The MASH Coordinator will be responsible for the coordination and delivery of CityCares key functions within the MASH including:
- The day to day operational management of the MASH Health Team, allocating the workload and prioritising work accordingly.
- Ensuring health records are reviewed, analysed, and relevant information shared with Children and adults Social Care in a timely manner where there are safeguarding concerns, domestic abuse incidents, or a request for additional support services has been made.
- Attending strategy discussions as the health representative where there are concerns that an adult or child has, or is likely to suffer, significant harm. You will contribute to the multi-agency decision making process, including any immediate or short-term actions required to safeguard the adult or child.
- Ensuring domestic abuse notifications are reviewed and recorded on SystmOne and are shared with the appropriate services.
- Adherence to CityCares safeguarding and domestic sexual violence and abuse (DSVA) processes and policies, current legislation, and national and local guidance/procedures.
