The Administrative Care Coordinator will help improve patient outcomes reduce health inequalities and support proactive care for people with complex needs long term conditions frailty cancer and care home residents ensuring patients receive coordinated compassionate and effective care across health and community services
Main duties of the job
Main Duties and Responsibilities
- Book appointments for patients requiring review through recall and screening systems.
- Process clinical documents, tasks, referrals and administrative workflows in line with practice procedures.
- Manage incoming and outgoing correspondence, drafting and initiating responses where appropriate.
- Maintain accurate and up to date patient records, ensuring all actions are recorded and completed in a timely manner.

We are looking to recruit an members of the Patient Services Team to join our practice in Wellington.We are happy to consider either full-time or part-time hours, provided that the working pattern covers the surgerys opening hours, from 8:00am to 6:00pm.

Reception Front Desk & General Duties
- A large proportion of your time will be taken assisting patients via the telephone or face to face in an efficient and caring manner
