Job summary
We are looking for an organised, proactive and highly capable Neighbourhood Team Coordinator to provide dedicated coordination, project, governance and administrative support to the PCN management team.
The role will provide additional capacity to the PCN management team, supporting the PCN Clinical Director, Strategy & Transformation Manager and Neighbourhood Lead with the coordination and delivery of their work.
The postholder will take responsibility for a significant proportion of the day-to-day coordination and administration associated with PCN, neighbourhood and transformation activity. This will enable the PCN management team to focus on clinical leadership, strategic development, neighbourhood relationships, transformation and delivery.
This is not simply an administrative role. We are looking for someone who can coordinate projects and activities, bring the right people together, maintain momentum, follow up actions, support governance and quality processes, and build effective working relationships with a wide range of stakeholders.
The successful candidate will be naturally organised and proactive, able to understand what needs to happen next and ensure that agreed actions are progressed.
Main duties of the job

Provide a central coordination and administrative function for the PCN
2. Meeting and stakeholder coordination
3. Neighbourhood Integrated Care support
Support the PCN management team as the PCN develops its approach to Neighbourhood Integrated Care.

Support the delivery of PCN projects, service developments and transformation programmes.
5. Women's health projects
The post holder will support the development and delivery of women's health projects and initiatives across the PCN, supporting the Womens Health Lead
6. Digital Transformation Support
7. Governance, CQC and quality
Provide coordination and administrative support to maintain effective PCN governance, quality and compliance arrangements.

We are an established Primary Care Network (PCN) in South East Telford, serving approximately 38,000 patients across our three member practices: Stirchley Medical Practice, Court Street Medical Practice and Woodside Medical Practice.

Main duties of the job
We provide General Medical Services, Enhanced Services, and actively contribute to Primary Care Network initiatives, in line with our GMS and other contractual commitments. We also offer regular practice meetings and strong support for continuing professional development.
About us
We are a well-established team with a real family feel. The practice is currently led by 2 GP Partners and supported by 4 Salaried GPs. Our clinical team includes 1 Practice Nurse, a Pharmacist Technician, a Treatment Co-ordinator, and a First Contact MSK Specialist.
Our experienced administrative team provides strong operational support, with an Admin Leader in post to oversee day-to-day coordination.

Role Overview
