Hereford City District Nursing Team
Have you always wondered what community nursing entails? Would you like a new challenge or have you had experience on the community and wish to pursue it as a career, then this is an opportunity for you. Due to the promotion of the current post holder we have a part time Band 5 community staff nurse vacancy within City District Nursing Team. As a Community Nurse you will become highly skilled in many different areas using many extended clinical skills and where no two days are the same.
The team supports patients registered with the 2 Primary Care Networks which are co-located in the City of Herefordshire. It is a large team with a caseload of around 850 patients, this allows a higher level of support for staff new to the community, allowing supervision for competency achievement.
Our base is at Belmont and we share a large office with neighbourhood team therapists which enables excellent levels of communication regarding shared patient care and concerns.
Main duties of the job
Most of our work is with housebound patients but, we also run a District Nurse Clinic for non-housebound patients who need the very specific skills of a District Nurse to support their treatment.
The focus of District Nursing is to support patients towards self-management, enabling them to continue to live as full and productive a life as possible, through education, equipment and or lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with the Hospice Teams and Specialist Palliative Care Nurses.
As community teams we enjoy working with many other health and social care professionals, Care Homes and Care Agencies. All staff are expected to record their work electronically on EMIS, our electronic patient record system which links into the GP services to allow a seamless service provision. Training and on-going support will be given to support staff with EMIS and other electronic systems they will need to use.

This is an exciting time to join a busy and well-established multi-disciplinary health team, supporting the health care needs of the people living in Herefordshire. We have a brand new opportunity to join our virtual ward team as an associate practitioner. Our aim is to deliver high quality care to improve health & wellbeing and to support our patients to remain living at home. The post holder will work within clearly defined boundaries of their associate practitioner competencies and carry out specific delegated clinical tasks and responsibilities as defined by the advanced clinical practitioner. The assistant practitioner will be assessed as proficient to work at a higher competency than expected of a band 3 health care worker, they will work in partnerships with team members, service uses, general practice, multidisciplinary teams and private/voluntary agencies. The post holder will be expected to work autonomously in a variety of setting including the patient's own home. Being able to work alone and travel in a rural area is essential. Main duties of the job
To carry own caseload of patients, as delegated by the Team Leads or senior clinicians on a daily basis.
Communicate with patients and their carers using tact, diplomacy, empathy and reassurance during potentially difficult or stressful situations or where there may be barriers to understanding, i.e. confusion, physical impairment, cognitive failure or language barrier.
Referral of patients to internal/external clinical and non-clinical team and/or other agencies.
Assist with effective and timely discharge planning for patients returning home from Hospital or to prevent hospital admission.
To be responsible for organising own caseload, ensuring all allocated tasks and visits are completed in discussion with the Registered health Professionals/ team lead.
Report untoward incidents and risks according to Trust/employer policies and procedures and participate in incident and complaint investigations as required
Identify potential risks for all staff and patients and adhere to relevant incident reporting procedures taking appropriate action/advice as needed.
To undertake administrative duties including answering telephones, taking messages, processing referrals and booking appointments as appropriate and maintain departmental paperwork as required.
Contribute to research, audits and clinical trials programmes carried out within the ward/department.

We have an exciting opportunity that has arisen in our Community Referral Hub (CRH), we are looking for an enthusiastic team player to become a call handler in our hub. CRH supports the delivery of care to people in their own homes who need urgent community services to align with Hospital Admission Avoidance.
The successful candidate will take calls from HCP'S, Paramedics, GP's, District nurses, Carers and members of the public and will be instrumental in capturing the required information to inform our clinical teams. You will need a friendly manner, have excellent IT skills and a positive attitude. The hub is open 7 days a week 08:00-20:00 and a variety of shifts can be offered to suit candidates and provide flexible working.

Main duties of the job
The post holder will receive and process referrals for the Community Referral Hub (CRH) via a call handling system, effectively communicate with a wide range of healthcare professionals and outside callers including members of the public. The Golden Valley District Nursing Team are a small but very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes. Due to the small size of the team with a very large geographical area to cover, community experience/knowledge is beneficial and advised for this post.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners.

Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.

The Golden Valley cover one of the largest areas within the service, this post will include a high amount of travel.

We are looking for a motivated and compassionateCommunity Support Worker to join our falls response service, within the Urgent Community Response Team. The successful candidate will respond to adults who have experienced a fall within the community, providing timely assessment, support, and intervention to help avoid unnecessary hospital admissions and promote recovery at home.When you're not out supporting our patients, you will be in the Single Point of Access HUB as part of the call handling team, you will be answering the phones, taking referrals from member of the public and healthcare professionals and supporting the wider teams.
The Community Referral Hub will soon be operating 24/7, 7 days a week, 365 days a year. Due to the role requirements, you ideally need to be able to work 12 hour shifts and be able to work 1 weekend a month. This role will involve working rotational day and night shifts.
Flexibility is a vital part of this role to support service needs such as annual leave, sickness or service pressure. Flexibility goes both way, we offer a range of flexibility to support you. This can be discussed at interview.
Main duties of the job
Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.
- Assess the patients home to ensure that they are not injured and it is safe for you to assist
- Support the patient in regaining an upright position, with the support of a multitude of equipment
- Support the patient with any basic care requirements
- Complete manual handling as appropriate for the scenario and in line with manual handling guidelines
- Complete the IStumble evaluation form using our EMIS system for the patient
- Complete the patient referral in its entirety
- Complete any onwards referrals for the patient
- Add any follow up calls & complete when necessary
- Be part of the whole CRH Team
- Take phone calls from community services, patients and other HCP's and ASC professionals and complete the referrals on the EMIS system

Main duties of the job

As part of the Trust's Information Management and Technology (IMT) Strategy, the IM&T Clinical Programme will radically alter the way that Trust staff interact and use technology to inform and support the care they provide.
Although Information Technology (IT) provides the opportunity to achieve these changes, the primary aim is to provide more effective and efficient patient care, not just implement new IT/clinical systems. Subject matter expert input is fundamental to the successful completion of these changes, and this role is crucial to the successful and safe implementation of the IM&T Clinical Programme.
The post holder will be one of the project managers for the IM&T Clinical Programme and will act as a role model and expert for all staff.
The core function of this role will be to enable the successful delivery of IM&T systems and functionality within the scope of the Programme and to support change as a result.
Main duties of the job
Use persuasion and negotiation skills to engage key stakeholders with complex and/or sensitive service changes in relation to process and/or service re-design and overcome resistance to change from staff (technical and non-technical) at all levels of the organisation.
Communicate, in the most appropriate manner, technical and non-technical information, and sensitive issues to internal and external stakeholders, and suppliers.
Ensure effective communication of business change plans and the associated benefits to staff and other identified stakeholders.
Lead multiple work streams ensuring projects are supported and delivered in-line with the agreed scope and timescales, delivering the functionality and benefits identified.

This roles requires you to have excellent organisational skills, an ability to build relationships with internal and external colleagues and an appetite for innovation. Job summary

You will work solely in the MSK physiotherapy specialism and we will begin your journey toward MSK specialisation within our department at Hereford County Hospital.
The department supports several of our band 6 MSK rotations and our MCATS clinicians. You will also be allocates an MSK mentor and participate in the preceptorship programme at the trust. Main duties of the job
Your main duties will include the MSK assessment and treatment of a range of patients from primary and secondary care. Your workload will be varied and include some 1:1 sessions and involvement with some group activities. You will have an MSK mentor and will set your own learning objectives alongside those required as we develop the rotation.
You will also have an opportunity to shadow the MSK clinical specialists and an MSK career pathway leading to band 6 and 7 rotational opportunities within the organisation including post graduate course funding . There is no on call or 7 day working required in this role.
