We are seeking an experienced, proactive Clinical Pharmacist to deliver high quality, patient-centred clinical pharmacy services within a practice based role.
This role is not hybrid/remote, it isbased within a practice within Greater Derby PCN practices in Derby City.
Reporting to HR Alexin and the PCN Management, the post holder will provide clinical pharmacy support across Greater Derby PCN in line with the priorities and direction set by the Greater Derby PCN Board. This is an exciting opportunity to work within a well-established multidisciplinary team and contribute to the ongoing development of safe, effective, and innovative medicines optimisation services.

About Greater Derby PCN
Greater Derby PCN consists of 10 GP practices with a collective list size of over 100,000 patients across the Derby area.
The PCN has an established leadership team and currently employs Social Prescribers, Clinical Pharmacists, First Contact Physiotherapists, Health and Wellbeing Coaches and a centralised administration team.
At present, Greater Derby PCN has chosen not to form a separate legal entity. All PCN staff are therefore employed by Alexin Healthcare CIC Limited.
Alexin Healthcare CIC Limited is a large and successful GP Federation covering Southern Derbyshire, East and South East Staffordshire. The organisation is dedicated to delivering high quality health services to the community, improving patient experience and keeping Primary Care at the forefront of NHS reform.

An exciting opportunity has arisen for experienced SCPHN school nurses to join the Bromley 0-19 service. We are looking for dynamic, confident and compassionate nurses to join the well-established team within the Bromley borough. The service is approaching a period of exciting change and innovation, and the post holder would have the opportunity to help influence and transform the service offer available to children, young people and their families.
The post-holder will be the named School Nurse for schools within the area. This role will incorporate a supervisory role for staff nurses working within the school nursing service.
The post holder will work as a member of the School Nursing Service and will work with schools, families, and the wider community to improve the health of school-aged children identifying the more vulnerable, and developing clear referral pathways so that all children and young people can reach their full potential.
This will be achieved by delivering the requirements from the service specification. They will work closely with the identified leader in each school providing expert information, guidance, and assessments as required.

Main duties of the job
As a highly motivated and dynamic school nurse, you will play a leading role in shaping our services as we are committed to meeting the health needs of children and young people through the delivery of the 5-19 Healthy Child Programme.
The school nurse will deliver high-quality evidence-based skilled nursing care within the schools of Bromley. You will be responsible for a defined caseload of secondary schools, and supporting junior staff with the management of primary schools. Applicants must hold a Specialist Community Public Health Qualification in School Nursing, and have post-graduate experience of working within a School Nursing team.
Adheres to the NMC Code and maintains own professional requirements in line with revalidation procedure.

We are seeking a highly motivated and enthusiastic paediatric physiotherapist to help lead our musculo-skeletal service for children aged 0-12 years. You will be based at the Phoenix Children's Resource Centre in Bromley and be part of a dynamic, friendly physiotherapy and multi disciplinary team, and have the opportunity to develop your skills as an MSK lead and as a children's physiotherapist, seeing children and young people with a wide variety of conditions within a variety of settings including schools, within the home and as out patients. Experience and further professional development in working with children with musculo-skeletal problems would be required.

Main duties of the job
To provide a high standard of physiotherapy assessment, treatment and advice to children and young people, carers and families. Undertaking the lead role in the assessment, diagnosis and formulation of physiotherapy treatment plans for children 0-12 years musculo-skeletal cases.

Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across Warrington.
The Clinical Pharmacy in General Practice model is supported by the direction of national policy the NHS Long Term Plan where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
Clinical Pharmacists play an important role within Primary Care Networks (PCN), working alongside pharmacy technicians, community pharmacists and other members of the PCNs multi-disciplinary team. You will directly contribute to improving patient care and supporting the efficient running of a new model of general practice. You will become the practice expert in medicines optimisation and have input into the following areas but not limited to:
undertaking structured medication reviews
medication queries
reconciling medications at discharge from hospital
improving medicines optimisation and safety
supporting care homes
running practice clinics
Main duties of the job
To improve patients health outcomes and the efficiency of the Primary Care team by providing direct, accessible and timely Clinical Medicines expertise, by working as part of a Multi-Disciplinary Practice Team.
To pro-actively transfer workload relating to medicines optimisation issues from Practice clinical staff to improve patient care, safety, thus freeing up Practice clinical staff to spend more time on other clinical care.
To promote safe, evidence based, cost effective prescribing in line with NHS Cheshire & Merseyside requirements
To provide expert pharmaceutical advice to Practices and the Medicines Optimisation Team
To provide prescribing support to the designated Practices and team members in areas such as:
Managing and reconciling medications from Hospital discharge letters
Medication reviews/structured medication reviews and follow up both over the phone and face to face
Implementation of prescribing changes
Review of repeat prescribing
Audit of practice prescribing
Medication queries
The post holder will provide leadership on quality improvement and clinical audit, as well as managing some aspects of the Investment Impact Fund (IIF).
Experience is preferred but all applications will be considered as training is provided to all new starters and you will be enrolled on to an 18-month primary pathway.

Job summary
An excellent opportunity has arisen for a Band 5 Podiatrist to join our dynamic team. The role is a development role, leading to a Band 6 Podiatrist role on successful completion of the required training, and development of relevant skills ,within a twelve month period of being in post. The role will involve timely assessment and treatment of a wide range of conditions utilising specialist skill sets and provision of specialist advice. It also involves collaborative working with Acute Services including James Paget University Hospital, and Norfolk and Norwich Hospital
Main duties of the job
The post will be based in a clinical setting and the postholder will carry and be responsible for their own caseload. They will assess, diagnose, develop and implement individualised care programmes for a caseload of patients who have complex podiatry presentations including acute and chronic pain patterns; significant physical impairments due to pain or injury and psychological issues and maintain own records as an autonomous practitioner.
The postholder will work with the Clinical Leads to further develop, in line with evidence-based clinical best practice, ECCH's integrated Podiatry Service.
Main duties of the job
- To carry and be responsible for own caseload. Assess, diagnose, develop and implement individualised care programmes for a caseload of patients who have complex Podiatry presentations including acute and chronic pain patterns; significant physical impairments due to pain or injury and psychological issues and maintain own records as an autonomous practitioner.
- To be a source of expertise for ECCH's integrated Podiatry service, facilitating an efficient and effective care pathway for patients.
- Access to arrange imaging, as appropriate, either independently or through our MAT's service/leads
- Developing individualised care and treatment programmes with suitable progression in line with patient expectation and ability
- Ensure effective communication to enable patients to achieve their maximum potential for self-management particularly where this is impaired due to pain and / or fear.
- A high standard of communication skills and clinical work is required which will also include teaching and supervision of other health professionals, non-registered staff and students.
- Work with the Clinical Leads for Podiatry to further develop, in line with evidence-based clinical best practice.

Bromley Hospital at Home is an integrated service delivered through Bromley Healthcare and One Bromley partners for avoiding acute hospital attendances and admissions while also supporting patients' early discharge from acute hospital beds.
The service provides diagnosis, treatment and monitoring of patients in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm. Referrals are accepted to the service, from the Acute Trust - Princess Royal University Hospital (PRUH) & Bromley Healthcare Urgent Community Response - Rapid Response Team.

This is an exciting opportunity for Registered Staff Nurses to join East Coast Community Healthcare's (ECCH) bank database and provide staffing in a variety of community nursing roles. The flexibility of bank means it can provide the perfect work life balance and gives you control of the shifts you work.
ECCH operates 24/7 working which includes weekends and bank holidays. Example of shift times are:- core shift 08.30 - 16.30, early 07.00 -- 13.00, late 13.00 -- 20.00. You will be given the flexibility to choose to work the shifts that suit you.

We are recruiting a Bank Band 7 Community Diabetes Specialist Nurse to work within East Coast Community Healthcare Specialist Diabetes Nursing Team.

We are seeking an enthusiastic, flexible and competent Dental Nurse to join our Special Care Dental Service. We are based across 5 sites in Bromley, Bexley and Greenwich. Even though the successful candidate would be ideally based at 1 or 2 sites on a regular basis but may on occasions be asked to cover for colleagues at different sites. We would expect you to make your own travel arrangements to meet the needs of the job. A valid driving licence and access to a car is essential for this position.

Excellent communication skills, compassion, empathy, good organisational skills, IT skills and being a team player are integral to this post. We run our General Anaesthetic sessions from PRUH in Orpington, we would expect the post holder to part take in these session when required. We aim to provide high quality dental care for our special needs patients and children who are referred into our service.
We carry out domiciliary visits to meet the needs of the patients in their own settings- their own homes, care/residential homes and sometimes hospitals. We also provide treatment under conscious sedation. Any previous experience of conscious sedation will be desirable.

The Special Care Dental Service is looking for an experienced and caring clinician with GDC specialist registration in Special Care dentistry, to join our team of Dentists. Your main area of responsibility will be to provide support and advice, including treatment planning/case discussions on complex paediatric patients with/for colleagues working within the service.
As part of the NHS England Special Care Dental Care pathway, you will be a team player, supporting the head of the service with guideline updates in relation to Special Care dentistry, acting as safeguarding lead, and providing staff training as required by the Clinical Director.

Specialist in Paediatric Dentistry

The Specialist in Paediatric Dentistry is a senior clinician within the Community Dental Service, responsible for providing high-quality, evidence-based dental care for children and young people with complex oral health needs, behavioural challenges, medical conditions, disabilities and dental trauma. The postholder will provide specialist assessment, diagnosis, treatment planning and clinical management across a range of settings, including dental clinics, schools, domiciliary visits where appropriate and hospital environments. A key component of the role is regular participation in the weekly Paediatric General Anaesthesia service, including patient assessment, multidisciplinary treatment planning, provision of dental treatment under general anaesthesia and post-operative care. The postholder will also provide dental care using behavioural management techniques, inhalation sedation and intravenous conscious sedation where appropriate.
As part of the multidisciplinary team, the postholder will provide specialist advice and support to colleagues, manage referrals for complex paediatric cases and contribute to the delivery of safe, effective and patient centred care. The role includes educational supervision, teaching and supporting the professional development of the wider dental team.

Main duties of the job
Provide specialist assessment, diagnosis, treatment planning and dental care for children and young people with complex oral health needs.
Deliver specialist dental care in community clinics, schools, domiciliary settings where appropriate and hospital environments, including treatment under inhalation sedation, intravenous conscious sedation and general anaesthesia.
Assess and manage children with dental trauma, developmental dental anomalies, significant dental anxiety, complex medical conditions and disabilities.
Provide specialist advice, support and clinical guidance to colleagues and accept referrals for complex paediatric cases.
Contribute to teaching, educational supervision and the professional development of dentists, trainees and other members of the dental team.
Support the Clinical Director in the delivery and continuous improvement of the Paediatric Dentistry service through clinical governance, safeguarding, audit, quality improvement, guideline development and service evaluation.
Work collaboratively with paediatricians, GPs, health visitors, school nursing teams, social care, education services and other healthcare professionals to deliver integrated, child-centred care.
Maintain the highest standards of clinical practice, professionalism and record keeping in accordance with GDC Standards, national guidance and Bromley Healthcare policies.

Main duties of the job
We are currently developing a Universal, Targeted Specialist model in mainstream schools, delivering EHCP intervention in mainstream schools, as well as expanding our buy-in service in mainstream schools, special schools and provisions.
Day-to-day, you will provide assessment and therapy services to a mixed paediatric caseload with speech, language and communication needs (SLCN), as well as report writing and opportunities for training parents/carers and those working with children with SLCN. Special school/provision work would involve working withchildren with a variety of severe, additional and complex needs, such as Autism Spectrum Disorder, Learning Difficulties, Developmental Language Disorder and Speech Disorders. Occupational Therapist - Home Pathway

The post holders will promote independence utilising rehabilitation and enablement interventions, working collaboratively with a vast range of key health and social care teams and services within the community, as well as voluntary and public sector providers, to ensure the successful delivery of the Home Pathway Service in Bromley.
Your role will encompass completing complex assessment and treatment plans, developing and leading education and treatment, and occupational therapy service development. The post will primarily involve working with elderly patients and their families with a focus on goal-orientated treatment planning within their home.
Band 5 Occupational Therapist with previous rehab and rotational experience are welcome to apply as this will be a supported position.
This is a static community rehab post however; there is a possibility this will become a rotational post in the future. It is expected that the job holder will work across and support all community based and in-patient therapy services, as operationally required.
Main duties of the job
The successful applicant will be supported and upskilled to gain experience in a leadership role to include the following responsibilities;
- Supervision of junior staff
- Individual caseload management and supervision of team caseload management.
- Completing return to works and managing sickness as per the trust's policy
- Carrying out detailed and timely appraisals in keeping with KPI's
- Co-lead on the coordination of all discharges from the Home Pathway Team.
- Lead on therapy input in the weekly consultant led MDT.
- The post holder will be expected to streamline processes to support efficiency within the Home Pathway.
- Service development; a review of current Occupational Therapy services and an innovative approach to developing new aspects of the service.
