The Trust is seeking a specialty doctor to join our Forensic Community Team service based in Norwich. The role offers the opportunity To provide specialised psychiatric care to patients with mental disorders who have offended to pose a high risk of harm.Secure and Forensic Services for NSFT are rated as Good by the CQC. We provide a wide and growing body of clinical services operating in a variety of settings linked to criminal justice services.
Main duties of the job
Assisting with discharge of patients from secure and in-patient care. Facilitating step-down care from medium or low secure units, supporting patients with severe mental illnesses living in the community, and preventing hospital readmission. The specialty doctor is expected to provide critical psychiatric care to mentally disordered offenders. They lead risk assessments, manage caseloads, prevent relapses, and coordinate with multi-agency teams, including probation and police, to safely integrate patients into society and prevent reoffending.

Teldoc Primary Care Network (PCN) are recruiting for a Social Prescribing Link Worker to join our small, dynamic team as part of the continuing expansion of our flexible and multi-professional workforce.
We are keen to hear from caring, enthusiastic and empathetic individuals who ideally have previous experience in social prescribing and care-coordination or of working in similar roles in healthcare, social care, housing and the community and voluntary sector.
The social prescribing link worker (SPLW) is a key part of the modern general practice approach to empower people to take control of their health and well-being and access services closer to home. The service works alongside our clinical team to provide patients with support with wider needs by linking them with appropriate support available in the local community. In this exciting and dynamic role youll be helping to address the wider determinants of health, such as debt, poor housing and physical inactivity and will be at the forefront of the emerging neighbourhood health agenda.
Main duties of the job
This role offers the opportunity to work in a dynamic environment where the quality of patient care is at the heart of everything we do. We are seeking a well-rounded Social Prescriber who is confident managing a diverse caseload and able to support individuals presenting with a broad range of needs and varying levels of complexity. The successful candidate will be flexible, proactive, and willing to contribute across the service to ensure the best outcomes for our patients. - To provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes

Main duties of the job
The role involves working within the community as an autonomous practitioner within a multi-disciplinary team. It also involves maintaining a good relationship with other members of the team ensuring the women receive safe care.
You will be need to be able to provide high quality women centred maternity care , effective leadership, optimise teaching opportunities and give support to all grades of staff; planning and directing activities to provide the optimal standard of care.
The role involves giving appropriate education to women and their families and giving necessary care, advice and supervision throughout the pregnancy continuum. Transcend is a newly commissioned service established to explore how Gender Incongruence can be best supported and treated within a Primary Care setting. We are seeking a Specialist Nurse to help deliver this care. The postholder will work closely with our MDT, local GPs, relevant partner agencies, and of course directly with service users.This is an exciting opportunity to contribute to the development of not only a new service, but a new model of care for a disadvantaged and underserved service user group. To provide direct clinical nursing care to the service users of Transcend. This will include both direct and indirect interventions to support individuals accessing the service. An excitingopportunityhas arisen for a Specialty Doctor post in the Urology Department at the Royal Lancaster Infirmary, working with a supportive and friendly team in the newly built Urology Unit.
The post includes on call, assisting in theatre and supporting clinicians in clinic. Day to day work will cover outpatient, elective & emergency aspects of the service.
This post is not a training post but the team will provide support with training for diagnostic procedures such as local anaesthetic prostate biopsies.

Main duties of the job
We are looking for an enthusiastic individual to join our team.
The post, including on call commitment, would cover clinical duties across our sites.
The post holder will be expected to support the Urology services offered by the Trust and provide the highest quality of care to the local population.
The post holder will be supported by our Consultants and our urology specialist nurse practitioners with ward support provided by the Matron's.
The post holder will be supported to take part in medical audits and in continuing medical education, (including self-development and maintaining accreditation).
There is a requirement for regular appraisal to be undertaken by the lead responsible for these services.

An exciting opportunity has arisen for a Locum Consultant ENT Surgeon to join the Head & Neck Service at Pilgrim Hospital, Boston, part of Lincolnshire Community and Hospitals NHS Group. Main Duties of the Job
- Deliver high-quality elective and emergency ENT services.

Main duties of the job
Responsibilities:

The successful applicant will provide a comprehensive ENT service, including outpatient clinics, theatre activity, ward care and emergency ENT provision. The role includes participation in multidisciplinary team meetings, clinical audit, teaching, research and service development. The post holder will work closely with Consultant colleagues, junior doctors and the wider multidisciplinary team to ensure the delivery of safe, effective and patient-centred care. Participation in the Trust-wide ENT on-call rota is required.

Stratton Medical Centre is a busy town practice on the outskirts of Bude.
We are a well-organised, forward-thinking practice led by two GP Partners and an experienced Practice Manager. The GP Day is carefully managed, with a balanced mix of routine and urgent appointments, triage sessions and follow up telephone consultations. Workload is continually monitored and our wider disciplinary team, including home visiting clinicians, pharmacists and administrative team, helps reduce the pressure on GPs and supports efficient patient care.

Main duties of the job
The role is to provide excellent patient care via a mix of:
Pre booked.
Routine appointments.
Urgent on the day appointments.
Triage duties that are part of a rota.
Managing all associated paperwork and test results.
Care planning for more complex patients.
Home visits (occasionally when the home visiting team are not available).
Peer support via coffee breaks and regular clinical meetings.
Supporting GP trainees with education and supervision.
About us
Stratton Medical Centre is a busy town practice on the outskirts of Bude. We have over 10,700 patients who are looked after by an integrated team of 9 GPs, 2 Nurse Practitioners, a Paramedic, 2 Nurses, 3 Health Care Assistants and a Phlebotomist.
As part of a wider Primary Care Network (PCN), our GP team also benefit from a clinical pharmacist, a home visiting paramedic, a social prescriber and health and wellbeing coach. We also have a team of staff who work with more vulnerable adults including diabetics, learning disabled and the frail elderly. All these staff contribute to managing our patient group.
We are a well organised team that is led by a Practice Manager and 2 GP Partners. We benefit from being able to respond quickly to situations and to listening to our staff to ensure that we remain a happy, well functioning team.
We all meet every morning to ensure we are aware of the issues for the day and pick up any concerns that may be present.
We also have regular team meetings, for all staff, to ensure that everyone has the opportunity to input into decisions or raise concerns.

Job summary
The Enhanced Frailty Team, in conjunction with the wider multi-disciplinary team, comprises of a Consultant Geriatrician, Chief Medical Officer, Enhanced Frailty GPs, Enhanced Frailty Nurse Practitioners, and Frailty Care Coordinator / Prescribers.
The purpose of the Frailty Team is:

Job summary
We are a small, dedicated team which consists of a senior partner, 2 salaried GP's, an ANP, a senior Practice Nurse, pharmacists, a physiotherapist and an admin team. Our list size is roughly 5400 at the moment.
The Practice Nurse will work as part of our clinical team at Grovehill Medical Centre. The role will be for 14 hours over 2 days.
Main duties of the job
The ideal candidate will provide services in long term condition management, child immunisations, travel vaccines, smears, dressings, blood tests, national vaccination programmes (shingles, pneumo, flu etc). Also, they would be responsible for clinical stock management and ensuring CQC compliance.

University Hospitals Plymouth NHS Trust seeks applications from knowledgeable, innovative, enthusiastic applicants to join a team of eight clinical perfusionists in delivering a comprehensive perfusion resource to an expanding cardiac surgical programme.
There are 1000+ adult cardiac surgical procedures per year currently being undertaken in three cardiac theatres at the SouthWest Cardiothoracic Centre with an increasing commitment to minimally invasive cardiac surgery.

Main duties of the job
You will join a supportive and developing Systems and Purchasing Team that plays a vital role in ensuring clinical and non-clinical services have access to the goods and services they need. This role offers an excellent opportunity to combine team leadership, customer service, problem-solving and procurement expertise in a fast-paced NHS environment.
We are looking for an individual with procurement or purchasing experience, ideally gained within a large and complex organisation. You will have experience of supporting or managing staff, working with purchasing systems and developing effective relationships with customers and suppliers. Knowledge of NHS procurement processes and systems would be advantageous.
You will have strong communication and organisational skills, a commitment to customer service and the ability to prioritise competing demands. You will be confident analysing information, resolving problems and supporting colleagues to achieve high standards of performance and compliance.
