This role is 16 hours. As a Receptionist/Administrator, you'll be the welcoming face and reassuring first point of contact for patients across our Park Leys sites. This is a role where you'll make a genuine difference every day, helping people navigate their healthcare, supporting our clinical team and keeping the practice running smoothly. As the practice operates across Keresley Green, Keresley End and Fillongley, you will need to be able to work at any of our sites, giving you variety in your day and the chance to support our wider team.
You'll be part of a friendly, professional environment where high standards, compassion and teamwork are at the heart of everything we do. Whether you're helping a patient book the right appointment, supporting admin processes behind the scenes or ensuring information is handled accurately and securely, your contribution will be essential to delivering outstanding care.
You'll benefit from ongoing, practice-specific training that supports your development over time and ensures you stay confident and up to date as systems, processes and patient needs evolve. If you're organised, caring and enjoy a varied role where no two days are the same, this is an opportunity to grow your career while helping your community.
Main duties of the job
In this role, you'll support patients through every stage of their contact with the practice, whether greeting them at the desk, answering calls, or helping them access the right services. You'll manage appointment requests, pass on messages, and provide clear, calm guidance during busy periods.
Behind the scenes, you'll help ensure the practice functions efficiently by maintaining accurate records, supporting the flow of correspondence, scanning clinical documents and keeping systems organised and up to date. You'll also help prepare consulting rooms, handle routine administrative tasks, and support colleagues during periods of leave or high demand.
Its a role suited to someone who enjoys working with people, thrives in a busy environment and takes pride in keeping things running smoothly and professionally.
About us
At Park Leys Medical Practice, you'll join a warm, supportive and professional team that prides itself on collaboration and high-quality patient care. Our reception, administrative and clinical staff work closely together across our three sites, creating an environment where communication is open, everyone is respected and teamwork is central to how we operate.
We're a practice that values reliability, integrity and a strong work ethic. We recognise the pace can be busy, so we focus on supporting one another, maintaining high standards and ensuring a calm, organised atmosphere for both staff and patients. From the moment you join, you'll receive structured training, clear guidance and encouragement from experienced colleagues.
Working with us means being part of a practice rooted in its community, with strong patient relationships and a commitment to continuous improvement. You'll enjoy a varied role, a stable working environment and the opportunity to build a meaningful career in primary care within a dedicated and welcoming team.

Reference: 2384116
Job summary
The Hospital at Home (H@H) service is an adjunct pathway to Urgent Community Response (UCR), delivering hospital-level monitoring and clinical intervention to acutely unwell adults in their own homes across the London Borough of Hillingdon. Operating 08:00-20:00, seven days per week, H@H provides an integrated step-up (admission avoidance) and step-down (early supported discharge) model in accordance with the NHS England Virtual Wards Operational Framework and the Hillingdon Health and Care Partnership Borough-wide Reactive Care Service.
The Band 7 Advanced Occupational Therapist is a senior clinical role at the heart of the UCR and H@H MDT. As the most senior therapy clinician on the H@H establishment, the post holder is the primary OT lead for the service, providing expert occupational therapy assessment, intervention, clinical reasoning, and discharge planning for complex, frail, and acutely unwell patients in their own homes. The role encompasses autonomous clinical practice, H@H Initial Visits, clerking of step-up patients, caseload prioritisation, and senior clinical oversight of therapy delivery within the team.
The post holder must be able to work flexibly over a 7-day week including weekday and weekend shifts and will frequently work as a lone practitioner in community settings.
Main duties of the job
o Be professionally and legally accountable for all aspects of own clinical work, managing an autonomous and complex caseload of UCR and H@H patients in accordance with HCPC standards and the RCOT Code of Ethics and Professional Conduct. Ensure a high standard of care and support more junior staff to do likewise.
o Perform the UCR / H@H Initial Visit for patients stepped up to H@H following the 48-hour C2R review, within 4 hours of the MDT decision - including OT-specific components of the clinical clerking, functional status, environmental risk, equipment, and advancing goal setting.
o Conduct advanced occupational therapy assessment for patients with acute illness, frailty, cognitive impairment, complex multi-morbidity, and diverse psychosocial needs, using validated tools (Rockwood Clinical Frailty Scale, Barthel Index, 6CIT, MUST, FRAT, environmental and manual handling risk assessment, seating and postural assessment) and applying higher-level clinical reasoning to formulate accurate occupational profiles and intervention plans.
o Deputise for the UCR / H@H Clinical Lead as needed in the day-to-day clinical leadership of the H@H pathway, including triage and prioritisation of referrals, chairing the morning MDT in the Clinical Lead's absence, and supporting the H@H COD in complex case management and caseload prioritisation by acuity.

Our service provides expert diabetes assessment, management, education, and support through nurse-led clinics, home visits, and collaborative working with other teams. We are committed to improving outcomes for people living with diabetes through evidence-based practice, innovation, and education.
We are looking for a nurse with demonstrable experience in diabetes care and management, who is passionate about supporting patients and developing the knowledge and confidence of healthcare professionals. - Assess complex health needs and develop, implement, and evaluate individualised care plans.
- Provide expert advice and support to patients, carers, community services, and primary care colleagues.
- Promote self-management and empower patients to achieve positive health outcomes.
- Contribute to the education, training, and mentorship of healthcare professionals and patients.
- Support service improvement initiatives and contribute to the continued development of diabetes services.

We offer great career development for both clinical and non-clinical roles, with ongoing training and support to help you learn and grow.
We are 'United in our Uniqueness' and committed to creating a workplace where everyone feels welcome and can be themselves, no matter their background or identity. We want our staff to feel supported and valued, and we aim to build a team that reflects the communities we serve. Together, we can make sure every voice is heard, and every difference is respected.
