Serving people with learning disabilities in the London Borough of Hackney, the Hackney Integrated Learning Disability Team is an integrated health and social caremultidisciplinary service. The service is committed to facilitation and liaison with local health andsocial care provider services in order that people with learning disabilities can access mainstreamservices where possible and that their experience and outcomes are good. Additionally, the serviceprovides specialist input to clients where this is required.
Main duties of the job
Providing a highly specialist clinical psychology service to people with learning disability and their families/carers including assessment, development and implementation of specialist psychological interventions, the post holder will develop a key role in the Hackney Integrated Learning Disability Service. The postholder will contribute to the continuing development of psychology andof the service.Acting autonomously the postholder will manage a defined caseload of complex casesindependently, evidencing problem solving and clinical reasoning skills in line with evidence basedand client-centred principles. S/he will be involved in clinical research and development whereappropriate.

We are seeking a creative, adaptable and innovative Mental Health Occupational Therapist to join our service within Community Mental Health transformation in Newham. Job summary
The Tobacco Dependency Advisor will support the delivery of a Trust-wide Tobacco Dependency Service, helping patients to quit smoking and improve their overall health.
The postholder will provide evidence-based behavioural support and treatment to patients identified as smokers, primarily within acute hospital settings, ensuring all eligible patients are offered appropriate interventions in line with national guidance.
Working as part of a multidisciplinary team, the role involves engaging patients in their quit journey, delivering personalised care plans, and supporting the implementation of effective tobacco dependency pathways across the Trust.
Main duties of the job
Deliver smoking cessation interventions, including behavioural support and advice on pharmacotherapy (e.g. nicotine replacement therapy).
Assess patients, develop personalised quit plans, and support individuals through their quit journey in both inpatient and community settings.
Provide one-to-one and group support sessions, including follow-up care (e.g. post-discharge telephone support).
Identify and engage patients who smoke, offering evidence-based advice and promoting harm reduction approaches.
Support the implementation of the Tobacco Dependency pathway and Ottawa Model of Care within the Trust.
Work collaboratively with clinical teams and community services to ensure continuity of care and effective referral pathways.
Monitor patient progress, record outcomes, and contribute to reporting requirements and service evaluation.
Promote smoke-free policies and contribute to health promotion campaigns (e.g. Stoptober).

The WorkWell Triage Officer will manage referrals, complete eligibility checks, coordinate onboarding and provide high-quality administrative support. As a first point of contact, you will ensure timely access to support and efficient referral processing.
Working with NHS organisations, Primary Care Networks, local authorities, employment support providers and voluntary sector partners, you will maintain accurate records, support programme administration and signpost individuals to alternative services where appropriate. Main duties of the job
You will require excellent organisational skills and the ability to manage sensitive information whilst providing a positive experience for participants and stakeholders. You will work collaboratively with WorkWell colleagues and partner organisations, providing support to inductions, and sharing knowledge of programme processes where appropriate. You will participate in training and development activities to maintain specialist programme knowledge as well as share learning and good practice across the programme team.
You should be confident in using your own initiative and working independently as needed to deliver on requirements of the role. You will be proficient in using software required to develop reports to deliver high quality documents through Microsoft suite (Word, PowerPoint, Excel). Strong attention to detail is essential, as you will be required to maintain accurate records to support contract monitoring and programme assurance as needed. You should possess good negotiation skills and the ability to build strong working relationships, enabling you to influence when needed, in order to deliver effectively through others.

The Essex Integrated Care Board (ICB) is a new organisation formed through the consolidation of three ICB areas in response to the NHSs ambition to deliver a more streamlined, strategic and cost-effective model of health commissioning. The ICB will oversee the commissioning of care for the approximately 2 million people who live in Essex. Operating within a reduced running cost envelope and aligned to the new requirements of ICBs in England, the ICB will focus on improving population health outcomes, health equity and smarter resource allocation.

Job summary
We are seeking a Consultant psychiatrist to join the Primary Care Liaison Service in Swindon. The post is an established post which has become vacant due to the previous incumbent moving to another service within AWP. The post involves working as the Primary Care Liaison consultant for a single CMHT catchment are in Swindon.
The post holder will provide clinical leadership in the team, help triage referrals, assess patients and make recommendations on care, provide advice to colleagues on assessment and management, liaising and working closely with the Intensive Service, Community Mental Health Team and Primary Care. The consultant will work closely with the PCLS Manager to ensure that safe, efficient and high-quality service is delivered by the Team. The Consultant will also provide clinical supervision and management supervision for doctors reporting to them.
Main duties of the job
The Consultant will deliver advice to service users including undertaking assessments, anddetermining the most appropriate service provision to meet identified needs, including briefinterventions where required. The Consultant will supervise any medical staff providing inputinto the service. Work collaboratively with the Recovery, Intensive and Inpatient Services tofacilitate transitions through the care pathway The Consultant is expected to contribute tothe development of services in line with best practice and nationally accredited standards. The post holder will be expected to support the senior management to and participate instrategic development both within the local service and the wider Trust. The Consultant willalso be expected to provide leadership to their Team. The Consultant is expected to respondto Advice and Guidance requests within 3-5 working days in accordance with the PCLSStandard Operating Procedure. The requestors will primarily be GP, ARRS' Workers, RMN'sbased in GP surgeries, and at times, although scarce, other medical specialists based in Acuteand Community Hospitals. Regular attendance at the PCLS Team MDT meetings. The postholder will manage and coordinate complaints relating to both clinical and non-clinical aspects of our services.
