This is an exciting opportunity to make a real difference to patients across our eight GP practices in inner-city Bradford. You will work closely with patients, carers, GP practices and our wider multidisciplinary team to help people navigate health and care services, understand the next steps in their care and access the support they need.
You will support patients with a range of needs, including people living with long-term conditions, frailty or dementia and those who may experience barriers to accessing healthcare. You will manage an agreed caseload, coordinate appointments and referrals, support multidisciplinary team working and make sure agreed actions are followed through.
You will join an established PCN team including Social Prescribing Link Workers, Health and Wellbeing Coaches, Clinical Pharmacists and other healthcare professionals.
We are looking for someone with excellent communication and organisational skills who is passionate about personalised care, reducing health inequalities and improving patients' experience of healthcare.
Main duties of the job
The Care Co-ordinator will work across Bradford City 5 PCN, supporting patients to navigate health, care and community services and ensuring that agreed actions are followed through.
The postholder will manage an agreed caseload of patients and work closely with GP practices and the wider multidisciplinary team. Key duties will include contacting patients and carers, coordinating appointments, referrals and reviews, helping patients prepare for care conversations and addressing practical barriers that may prevent them from accessing support.
The role will support multidisciplinary team (MDT) working by preparing relevant information, recording agreed actions, identifying responsible professionals and tracking actions through to completion.
The Care Co-ordinator will maintain accurate and timely patient records using SystmOne and agreed PCN templates, and will support the PCN to monitor activity and outcomes.
The postholder will work particularly with patients who have complex needs, long-term conditions, frailty or dementia, and people who may experience health inequalities or barriers to accessing services.
This is a non-clinical role. The postholder will work within agreed boundaries and promptly escalate clinical, safeguarding or safety concerns to the appropriate professional.
About us

Our member practices are Avicenna Medical Practice, Bevan Healthcare, Bradford Moor Practice, Dr Hamdani's Practice, Moor Park Medical Practice, Peel Park Surgery, Primrose Surgery and Valley View Surgery.
We have a growing multidisciplinary PCN team which includes Social Prescribing Link Workers, Health and Wellbeing Coaches, Clinical Pharmacists, Mental Health Practitioners, First Contact Physiotherapists and other professionals working alongside our GP practices.

We value collaborative working, new ideas and continuous improvement. The successful candidate will be supported by the PCN management team, receive appropriate induction, training and regular supervision, and have the opportunity to help shape and develop the Care Co-ordinator role as our services continue to evolve.

Consultant Psychiatrist in Adult Mental Health - In Patient
Tees Esk and Wear Valleys NHS Foundation Trust View all jobs
Posted 21 day(s) ago
Reference: 2371761
Job summary
The post is based at Cross Lane Hospital, which is a modern, custom-built, AIMS-accredited psychiatric unit. The Ayckbourn unit houses the male and female AMH wards.
The job is split into 3 sessions of supporting professional activities (SPA) and 7 sessions of direct clinical care (DCC).
You will provide expert assessment and care to patients admitted to the wards, specifically to 6 patients, 3 on each ward. They will have a dedicated slot for discussion of their patients in the daily report out. The daily report out is attended by a Multidisciplinary team comprising the people described in Section 3. They provide input based on a biological, psychological, and social model to support the treatment and recovery of each patient. The two nurse consultants would have similar slots to discuss their patients. There is an expectation of the postholder supporting the Specific MHA requirements that the Nurse consultants portfolios, such as sectioning of patient, does not permit.
It is hoped that you will be encouraged to use the enhanced SPA provision within this post to engage in research, education or service development in conjunction with local colleagues in order to ensure that this can occur in formal protected time. You will be expected to provide key medical leadership to the wards alongside the MPACs and a multi-disciplinary team.
Main duties of the job
Offer senior advice and support in the assessment, formulation, diagnosis and management of presenting conditions.
Support the team and other prescribers to make safe and effective prescribing decisions where required.
Senior medical care/supervision in the assessment, diagnosis, care planning and delivery of pharmacological and psychosocial interventions.
Take part in morning report out meetings with the team as well as attending MDT meetings, professionals meetings, and the team leadership meeting.
Take part in and provide insight and direction into complex case work, contributing to the overarching formulation of patient care and planning within the scope of MDT approach.
Join the wider team in providing relevant teaching and training when required.
Provide support to the Nurse Consultants in their role as Approved Clinicians and Non-medical prescribers.
About us
We are one of the largest specialist mental health and learning disabilities trusts in the country, with an annual income of £320m and a workforce of some 6,700 staff operating from around 100 sites in Durham, Teesside, North Yorkshire and York and Selby. We provide a range inpatient and community services to 2m people living in County Durham, the Tees Valley, Scarborough, Whitby, Ryedale, Harrogate, Hambleton and Richmondshire. We also provide additional specialist services to other parts of Northern England.
We deliver our services by working in partnership with eight local authorities and clinical commissioning groups, a wide range of voluntary organisations, as well as service users, their carers and the public.
Psychiatrists are essential contributors to our mental health teams. Service users expect and are justified to expect collaborative relationships with dedicated psychiatrists who work consistently as part of effective services. The Medical Directorate and Medical Development Team are striving to improve medical workforce recruitment and retention in a variety of ways and the cornerstone of these efforts is The TEWV Charter for the Medical Workforce.
