Main duties of the job
Essential requirements of a Floating Mental Health Support Worker
- Flexibility - Your role will involve working night shifts including weekends and bank holidays
- Full UK Driving Licence and access to a car
- Must be able to attend training throughout employment which will be held on weekdays
- Good written skills and the ability to write basic reports and record information accurately
- Basic IT skills (Your role will involve some computer work, completing forms, using an online care system, and writing emails)

Job summary
Cruddas Park Surgery are excited to recruit a Salaried GP for six sessions including one fellowship session (negotiable/pro-rata). The post would offer a new challenge to an established GP or the opportunity to flourish as a newly qualified GP.
Cruddas Park Surgery believe we offer a unique approach to Deep End primary care supporting the most vulnerable and invisible through our motivated and expert clinical and non-clinical staff, specialist clinics (including Health Inclusion drop ins, Learning Disability and Special Allocation Scheme), links with the VCSE and Local Authority, and innovative approaches to tackling mental illness through The Space, our multi-agency one stop mental health hub.
The fellowship session has been a successful model in the past and would include a mutually agreed period of protected non-clinical time and regular senior supervision on joining the practice. This session would then become an opportunity to develop an area of clinical or non-clinical interest which was of benefit to our patients.
Employment is subject to the terms of the BMA contract at £12,925 per session including paid CPD time and 6 weeks holiday pro rata. Current routine sessions are 11 new patient face to face appointments. A shared practice electric car is available for home visiting. We have a clear desk policy for holidays and active social calendar including a running club.
Main duties of the job
The post-holder will manage an unselected caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients in line with local and national guidelines as well as practice policies. They would be expected to contribute to the pro-rata Duty and Triage rota and home visits; assist in education, supervision and training; as well as play a full role in achieving local and national targets and quality improvement targets. The CRHT team is a part of acute care and works closely with the acute hospital and community teams to support clients in a mental health crisis. It offers clients care and support in their own homes as an alternative to hospital admission.
CRHT are able to offer all the benefits of a hospital admission while remaining at home including home visits from our experienced team, with support from doctors and psychology. We also facilitate supportive discharge from hospital, managing the transition back to community-based service.

Main duties of the job
As an Urgent Care Practitioner in our team you will work in partnership with service users with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives.
You will be responsible for the assessment, planning and implementation of top-quality care to service users. This will include working alongside service users, carers, families and partner agencies such as voluntary sector agencies, schools and colleges.
As well as providing specialist nursing advice and carry out nursing procedures, and monitoring the effectiveness of treatments in order to progress and ensure effectiveness of intervention.
You may also be involved in providing clinical supervision to staff and students. The HCA works within the organisation and supports the practice nursing and administrative teams in the delivery of clinical and administrative services. An exciting opportunity for a highly motivated, enthusiastic, Lead Diabetes/Endocrine nurse to join a well-established specialist nursing service. The service includes Diabetes, Rare Diabetes, Endocrinology, CEW and Metabolic Bone which provide a local and regional service
You will work with the specialist nursing team to lead on the continued high quality and specialist care provided across theseservices within BWC. You should be clinically up to date in one of the specialties with excellent knowledge of current guidance.
You will need to be a RSCN/RN child branch with proven post registration experience in care of the child and young person with Diabetes or an Endocrine condition. You will also need to have experience of managing and leading teams and /or caseloads alongside a recognised management course. Having experience of working in communityand a current driving licence, would be advantageous but not essential. The post is involved in specialist education so experience of delivering teaching to wide range of professionals and settings would be desirable. Extensive safeguarding experience is important as the postholder will need to lead and support the specialist team with complex cases, alongside the Trust safeguarding team.

The post holder will work within the Diabetes/Endocrinology footprint in outpatients, wards and allocated offices, in the community and across the West Midlands.

Job summary
The Non-Specific Symptom Pathway (NSSP) offers a single point of access to a diagnostic pathway for patients with non-specific symptoms. The post holder will need to work in close collaboration with care of the elderly Consultant at North Bristol NHST Trust and, Primary and secondary care to support a tailored pathway of clinically relevant diagnostic tests towards a timely diagnosis. The post holder will utilise skills of clinical expertise, education and management to ensure a high quality service for patients, carers and staff.
The NSSP CNS will undertake a pivotal role alongside the Pathway Navigator, liaising with patients who are referred, acting as the main point of contact, offering support and keeping patients informed at each stage. Once diagnosis is known, to refer to the appropriate team or back to the GP's for ongoing management ensuring a seamless transition of care. The post holder will work closely with the NSSP GP to support triage of referrals, liaising with colleagues within diagnostic services across Trusts to ensure that investigations and tests are timely and co-ordinated.
This post would be open to nurses with primary or secondary care experience as much of the role focuses on timely telephone triage and detailed record keeping. Whilst office based initially, WFH days could be considered for the right candidate and this role is a great step-up into a career within Oncology.Main duties of the job
The NSSP Clinical Nurse Specialist role is varied and constantly developing to meet the speciality needs and evolving site specific referral pathways within Bristol, North Somerset and South Gloucestershire. 3. To deal with clinical queries from new and long-term CPAP and NIV users over the telephone and by email.
4. To provide Home sleep studies and oximetry on an outpatient basis.
5. To Ensure simple fault remedy, cleaning, and maintenance of recording equipment within the service.
6. To Download home sleep monitoring equipment and ensure cleaned and ready for next patient.

The Children's Clinical Research Facility
The Children's Clinical Research Facility (CCRF) forms part of the Children's Hospital,based at The Royal London Hospital, but working across other Barts Health hospitals, particularly Newham and Whipps Cross University Hospitals.
Key objectives of the CCRF are to:
- Support experimental medicine research in children, including the timely and efficient recruitment for, and completion of, commercial and non-commerical studies

Join a dedicated team providing clinical health care support for Acute Virtual Ward patients' in their homes across Plymouth, South Hams, and West Devon. This part time role (up to 30.00 hours per week, 7.5-hour shifts) is ideal for an experienced and motivated health care assistant eager to contribute to a growing Urgent Care Nursing service.
You will deliver high-quality, patient-centered care in the community, helping individuals remain safe and independent at home while supporting their health and wellbeing. The role involves utilising clinical skills and expertise within the Urgent Care Nursing Service.

Main duties of the job
Responsible for the delivery of high quality clinical care for patients within their homes.
Liaise with the nurse in charge and referral coordinator within the Urgent care nursing service team, and will be supported by senior clinicians & clinical manager.
Responsible for effectively assessing, monitoring & escalating concerns regarding patient care using SBAR tool.
To prevent & reduce hospital admissions & minimise hospital lengths of stay by supporting the multi disciplinary team with care planning using clinical skills such as manual observations using NEWS 2, venepuncture and ECG's.
Responsible for reporting to senior clinician on duty.
The Urgent Care Nursing team work on a flexible rota shift system.
The service covers 24 hours per day over seven days a week. However the Acute Virtual Ward health care assistant role runs from 08:00 - 17:00, this post is for 7.5 hour shift per day.
The post holder will need the ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
