We are currently seeking an experienced and motivated Registered Manager to lead our team at Granville Respite/Short Breaks Service in Eccles. Granville is a 14-bed service offering short-term respite care for individuals aged 14 and above, with learning or physical disabilities, Autism, complex behaviours, and/or complex health needs. The service is currently rated Good by the Care Quality Commission.
This is an exciting opportunity to join a forward-thinking organisation in a role that brings purpose, challenge, and the reward of making a positive difference in peoples lives.
Key Responsibilities:
- Provide strategic and operational leadership to the team at Granville.
- Ensure the delivery of high-quality, person-centred care in accordance with all relevant legislation and CQC standards.
- Adapt effectively to changing care needs, ensuring continuous quality improvement and service development.
- Build strong, collaborative relationships with families, carers, health professionals, commissioners, and other stakeholders.

Cumbria Medical Services is looking to recruit a Patient Booking Coordinator. Primarily the Booking Coordinator ensures the effective management of all incoming NHS referrals using the E-Referral system and ensures that adequate clinic slots are available at all times.
The role also involves handling of patient enquiries and requests courteously and efficiently and maintaining confidentiality of any information in connection with the work.

This is a permanent position for 3 days a week/22.5Hrs (Between Tuesday-Friday). The working days/pattern will be discussed with the successful applicant.
Our basic salary is £13.75 per hour, which may rise, dependent on the skills and experience of the candidates.

Holidays are 26 days per year plus 8 bank holidays, rising after 5 and 10 years.

Job summary
We have a vacancy for a Senior Pharmacy Assistant to work in our busy and friendly department.
This position is based in Winchester, however there is occasionally a requirement to work across sites.
We are looking for you to accurately dispense in-patient, discharge, and out-patient prescriptions, supporting patient flow in a fast-paced environment.
Prioritisation skills are valued and personal development is encouraged and supported.
You will need to possess an NVQ Level 2 in Pharmaceutical Sciences (or equivalent), and all applicants must have excellent communication skills, be flexible and be able to work accurately under pressure.
A good level of English and Maths (GCSE level or equivalent) and computer skills are essential. Physical fitness is required as there is some manual handling involved.
Main duties of the job
To accurately dispense (including generation of labels) in-patient, discharge, and out-patient prescriptions.
To answer telephones promptly and politely, dealing with queries, relaying complete and accurate information.
To recognise self-limitations and refer to an appropriate person when required.
To dispense controlled drugs.
To receive out-patient prescriptions in the dispensary.
To hand to the patient or carer, out-patient prescriptions if deemed appropriate by the Responsible Pharmacist.
To be responsible for the accurate input of data into the pharmacy JAC computer systems for all duties requiring its use (labelling, returns etc.).

The transition from a single national centre to a hub-and-spoke network enables patients to receive high-quality specialist care closer to home, while maintaining the expertise and clinical oversight of the UK National Amyloidosis Centre (NAC) at the Royal Free London NHS Foundation Trust.
As MDT Coordinator, you will support the coordination of patient pathways and multidisciplinary team activity across the network, helping to ensure seamless communication, continuity of care, and an excellent experience for patients and clinicians alike.
Main duties of the job
Coordinate and administer the Amyloidosis Multidisciplinary Team (MDT) meetings across the UK Amyloidosis Network (UKAN), ensuring all eligible patients are identified, reviewed and discussed in a timely manner.
Work closely with Amyloidosis clinicians, specialist nurses and MDT members to establish and maintain processes for identifying patients requiring MDT review.
Prepare and distribute MDT agendas, patient lists and supporting clinical information within agreed timescales.
Ensure all relevant clinical information, including medical records, pathology, genetics, imaging and investigation results, is available to support MDT decision-making.

Haemodialysis
The haemodialysis unit is co-located with the renal inpatient ward (L50). This is a designated 5 station unit, including an isolation cubicle with a separate entrance for family and patients. Haemodialysis (haemodialysis and haemodiafiltration) is delivered in centre with routine haemodialysis delivered Monday to Saturday (6-day service) with a 7 day/week daytime urgent dialysis cover. The number of children receiving dialysis ranges from 8-12. Leeds is one of the KQuIP (Kidney Quality Improvement project) establishing home haemodialysis and as part of this there is a haemodialysis nursing staff expansion programme including rotational nursing staff from the ward and an nurse educator role.
Peritoneal dialysis
There is an established peritoneal dialysis programme with patient numbers ranging from 4-10. There is urology surgical support for catheter insertion for acute and chronic peritoneal dialysis. There is a training package for carers led by the specialist nursing team with a combination of in hospital and at home training. There is ongoing support from the specialist nursing team at home with electronic access to each dialysis session. There is monthly outpatient review of all patients receiving peritoneal dialysis in our MDT clinic.
Transplantation
There is an active transplantation programme in Leeds with an average of 10 transplants per year, with a current cohort of 65 transplant patients under follow up. There is specialist nursing support for transplant work up and a comprehensive MDT transplant listing programme involving nephrologist, transplant surgeons, immunological laboratory, specialist nurses, psychology and social workers. The transplant programme is supported by the transplant surgical team Mr Masood, Ms Martinez-Lopez and Mr Falconer.
Nephro-Urology
There are close links with the paediatric urology MDT. There is a monthly nephro-urology MDT to discuss complex cases and a monthly joint nephro-urology clinic. There is access to non-invasive and invasive urodynamics and an evolving medical bladders service.
There are a number of specialised clinics including antenatal counselling, regional stone service, Tuberosclerosis clinics and joint rheumatology clinics. As well as specialist renal services, the department also provides secondary referral care for a large range of paediatric nephrology referrals including urinary tract infection, haematuria, proteinuria and problems with incontinence
Outpatient services
In Leeds the paediatric renal service outpatient clinics are based within Children’s outpatients in Martin Wing on Leeds General Infirmary site with a dedicated paediatric play support and phlebotomy service. There are weekly transplant, general nephrology and chronic kidney disease clinics. In addition, there outreach joint clinics across Yorkshire and the Humber based in Airedale, Bradford, Dewsbury, Grimsby, Halifax, Huddersfield, Hull, Pinderfields, Scunthorpe, York (Scarborough).

Job summary
We are looking for an organised, patient, flexible and efficient person with excellent customer service skills to support us with day to day admin within the Residences Team. The Residences Team looks after doctors on-call rooms, longer term accommodation via tenancy agreements and the Bexley Patient Hotel.
Main duties of the job
The role requires liaising with a wide range of people across LTHT, both in person and via email/phone and beyond, and therefore excellent communication and interpersonal skills are essential. This is a very busy and demanding service, and the successful candidate will be required to use their initiative to prioritise work and meet deadlines. A major part of the role is to prepare long term accommodation between tenants. This involves raising maintenance issues and ordering any new items required. All stock for the properties then needs to be unpacked and the property staged for the next incoming tenant. There is also a requirement to move between sites in addition to responding to daily requests from tenants in our accommodation, therefore you will be required to be mobile and on your feet for some of your day. The post is full time. Working Monday Friday, 8am 4pm with a 30 min lunch break. This will be a challenging role, but for the right person offers an excellent development opportunity. Nucleus 7&8 Theatres provides General, Urology, Gynaecology, Plastic and Breast surgical procedures, and is the home of our 2 Davinci Xi Robots. Main duties of the job
We are looking for theatre practitioners with scrub and anaesthetic skills
You will need to be a registered nurse or ODP with a minimum of 3 years theatre experience at band 5.
Successful candidates will need to demonstrate that they are motivated, able to work on their own initiative, are a team player with the ability to prioritise workload and support the team leader, theatre Coordinator and team to provide a safe and effective service for our patients.
Following our most recent CQC inspection the division of surgery was rated as Good across the 5 key standards so you would be joining a high performing team and would be expected to support the Team Leader to influence and drive improvement within Nucleus 7&8.

We are looking for a creative, innovative and motivated advanced practitioner to support the delivery of Occupational Therapy (OT) within Long term conditions Neurology team, based at St Woolos Hospital.

The post holder should show commitment to continually developing knowledge, skills and experience in condition management, complex moving and handling, adaptations, specialist seating and support the development of team members and students as part of their clinical leadership role.
Postgraduate experience of working with individuals with neurological conditions is essential as is the ability to work flexibly and proactively to deliver a prompt and clinically effective service.
Main duties of the job
Professional aspects of the Occupational Therapy role include:
Utilise advanced knowledge and skills from a varied clinical background, including in a neurological setting, to assess, treat and manage a varied caseload.
Identify and prioritise the people who will be most likely to benefit from occupational therapy intervention.
Independent, confident decision-making and clinical reasoning with a complex and varied caseload.
Work jointly with the multi-disciplinary team to optimise the impact of occupational therapy.
Identify and implement changes to support the development of this emerging role and service.
Utilise and implement the growing evidence base for occupational therapy practice using a consultancy model, contributing to this where possible.

Main duties of the job
As a Medical Secretary within our specialist Eating Disorder Intensive Treatment Team, you will play a key role in supporting the efficient delivery of patient care and ensuring the smooth running of the service. Working closely with clinicians, particularly the new to recruit consultant psychiatrist and the wider multidisciplinary team, you will coordinate a range of administrative activities, and help maintain effective communication across the service.

You will contribute to a busy specialist service that supports individuals with eating disorders, helping to ensure patients receive timely and well-coordinated care while working as part of a supportive and dedicated team.

Main duties of the job
Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). You will be supported by the non clinical team who have a good knowledge of our extensive local hospital and community services.
The clinical team includes GP's, Practice Nurse, Pharmacist, Physician Associate and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.
In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.
You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP. You are also allocated CPD time weekly.

Job summary
We have an opportunity for an enthusiastic, motivated Mental Health Professional to join the Wellbeing Team (Mental Health and Psycho-Social Substance Misuse) at HMP Eastwood Park.
If you are a friendly, compassionate and empathetic individual, who would like to work with vulnerable and chaotic individuals in a recovery focused model, then you may well be extremely suited to working in ourTeam.
You will be based with our multi-disciplinary team within the prison and will work closely with a variety of other professionals, including Psychiatrists, Psychologists, Non-Medical Prescribers, Social Prescriber, Social Workers, Neurodevelopmental Practitioners and Recovery Co-ordinators.
NOTE: This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.
Main duties of the job
This role is focused on providing appropriate, effective interventions and treatments to women with a range of mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and wellbeing needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
The post holder will work autonomously to undertake emergency assessment in a wide range of locations, planning and delivering defined, intensive, specialist interventions in line with the CPA framework to people with severe and enduring mental health needs, who maybe distressed and in crisis. Where appropriate the post holder will work alongside other services to plan, deliver and review effective "wrap around" intensive interventions to manage risk and prevent hospital admission for those with the most complex needs. The prison houses up to 442 women. In addition to the specialist unit for substance misuse, the prison offers a Drug Recovery Community, Mother and Baby unit and health care clinic. Job description
Job responsibilities
1. In a wide range of situations and locations to undertake the full range of activities required to deliver comprehensive mental health assessment, in an emergency for service users, who may be distressed and in crisis. This will include:
a. The use of standardised assessment tools i.e. Cluster Allocation Support Tool (CAST), KGV
b. Recovery Star
c. History, strengths and aspirations
d. Mental state
e. Impact of culture and diversity
f. Functional needs
g. The needs of family and carer
h. Evaluation of risk
i. Physical health
j. Complicating factors
k. Identifying and ensuring specialist involvement where required.
l. The interventions and treatments required to enable positive change.
m. Social Care
n. Safeguarding and public protection
o. Capacity under the Mental Capacity Act
2. To be responsible for developing delivering and reviewing comprehensive and complex treatment programmes using appropriate frameworks in line with evidence based practice, including strategies to manage risk for service users with complex needs and carers, bringing in other resources as required.
3. To act as care coordinator for service users as appropriate, also providing defined interventions to individuals on other workers caseloads.
4. To plan, deliver and evaluate defined, specialist therapeutic interventions as indicated in the assessment process ,in line with personal recovery plans, including to service users who maybe on other caseloads. This might include: a. Individual or group therapeutic intervention b. Psychological treatments such as CBT, DBT approaches, family interventions c. psychosocial interventions d. Motivational and coping enhancement strategies. e. Medication management f. Interventions under the Mental Health Act.
5. To deliver a range of defined activities/interventions to improve the carers ability to support the service user and to enable them in their relationship with the service user.
6. To develop and maintain good partnership working with other services throughout all treatment episodes, including regular liaison within Primary Health Care Team, inpatient and intensive services, day services, voluntary sector and with nominated carers/advocates.
7. In collaboration with service users and carers, to be responsible for facilitating the development of comprehensive crisis plans, rapid access plans, advance statements etc., involving other agencies such as primary care etc. where appropriate.
8. To personally build and lead others to build, hope inspiring relationships with service users, which acknowledge the personal journey of each person, and focus on strengths and aspirations to allow the creation of meaningful personal recovery plans.
