Job summary
Join our team and play a key role in delivering and developing our specialist Neuro-Orthoptic Service, providing expert, evidence-based care to complex patients while supporting, leading and developing colleagues within a friendly, collaborative and forward-thinking Orthoptic team.
Main duties of the job
To lead and deliver a high-quality, evidence-based Low Vision service, providing highly specialised Orthoptic assessment, diagnosis and management to patients across a wide range of complex ophthalmic, neurological and medical conditions, including tertiary referrals.
The role provides clinical and professional leadership to the Low Vision team, including training, supervision, audit and quality improvement, while supporting the Orthoptic Professional Service Manager with the operational management and development of the wider Orthoptic service. The postholder works autonomously and collaboratively with Ophthalmologists and the wider multidisciplinary team, provides specialist clinical advice, manages dedicated Low Vision clinics, and contributes to extended roles within the scope of practice and Agenda for Change banding.

An exciting opportunity has arisen within Sunderland Talking Therapies to appoint a Qualified Psychological Wellbeing Practitioner. The position is full time hours (37.5 per week) for either a qualified PWP who holds a PWP accreditation or a Trainee PWP who is nearing the end of training due to be ratified, and wants to develop their skills and experience within our team. The post holder will work with people with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities.
The successful candidate should:
- Have successfully completed an academic course of study in line with the IAPT Low Intensity National Programme and be qualified as a PWP (e.g. PG Cert Low Intensity Psychological Therapies)
- We are also interested in receiving applications from current Trainee Psychological Wellbeing Practitioners who are enrolled on a PG Cert in Low Intensity Psychological Therapies and are due to qualify. If you are a current Trainee PWP you would initially be employed at Band 4 and once marks have been ratified by your university examination board, and you have gained PWP registration this would be uplifted to a Band 5.
- Have a current registration with the BABCP or BPS, or be able to immediately apply for this once in post (PWPs will remain at band 4 until registration has been submitted and confirmed).
Main duties of the job
In this role you will:
- Undertake triage screening assessments and make onward referrals
- Provide a range of low intensity interventions e.g. guided self-help (both telephone, online consultation and face-to-face), psycho-educational courses and computerised CBT
- Have the opportunity to contribute to a number of innovative service improvement projects, champion roles and specialist interest groups
- Liaise with external agencies and services to promote Sunderland Talking Therapies
- Have the opportunity to act as a mentor to trainee PWPs
- Work closely with other PWPs to support one another and utilise skills and knowledge
- Work closely with the wider multidisciplinary team and engage in team huddles

As an Occupational Therapy Assistant Practitioner, you will manage a varied clinical and ward-based caseload within an acute multidisciplinary environment. You will take a lead role in discharge planning and undertake access visits in the community.
The post-holder will be required to work Bank Holidays and weekends as part of a rota.
The base for this role will be Pilgrim Hospital in Boston, with an expectation to travel to and work at least one other site--most likely Lincoln County Hospital.

Main duties of the job

Previous health service knowledge is essential. You will need a technical or similar qualification and be prepared to undertake the Health Screener qualification.
You will be expected to run eye screening clinics, to include administering eye drops and taking an accurate patient history. You will also be responsible for capturing high quality digital retinal images using both fundus and OCT cameras for diabetic patients within the county of Northamptonshire.
In addition you will be responsible for your own clinics, by reminding patients of their appointments and ringing DNAs in the clinic.
The Retinal Screener will responsible for triaging and recognising diabetic abnormalities for fast track referral.
You will be required to drive between the 9 screening venues in the Northamptonshire, based in Corby, Kettering, Wellingborough, Irthlingborough, Northampton x 2, Daventry, Towcester and Brackley.
This is a countywide post and the successful candidate will be expected to work on a rota basis. The screening clinics run Monday - Friday with a minimum of one Saturday morning per month.

Main duties of the job
This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting.
This is an excellent opportunity for dedicated and committed registered nurses who are looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence through management of long term conditions.
We are able to provide an individually tailored preceptorship program to experienced nurses.
Consideration of Flexible working would be considered where applicable and on an individual basis. Daily working hours are between 8:30 and 23:00.

Job summary
An exciting opportunity has arisen for an experienced speech and language therapist to work within our All Age Forensic Community Service. The post holder will work in our Forensic Child and Adolescent Service. This newly established role will be pivotal in establishing a high-quality speech and language therapy pathway offer, providing specialist assessments and interventions to patients currently receiving care in FCAMHS. The role will also involve providing day to day support and clinical leadership to SALT staff working within service.
You will have experience of working with individuals with a range of mental health conditions and either experience of, or transferrable skills to work with individuals with identified learning and communication needs. You will have the ability and motivation to develop and shape the direction of the speech and language provision, ensuring it is evidence based and meets the individuals needs. The post holder will be required to liaise effectively with all relevant professionals, carers and family members to ensure the needs of the individual are met.
We are looking for a flexible, confident, enthusiastic and proactive speech and language therapist with established or developing leadership skills. Effective communication, organisational and IT skills are essential.
The successful candidate will be passionate for improving the lives of vulnerable children and have a strong understanding of local and national drivers and legal frameworks relating to FCAMHS.
You will provide supervision to the speech and language staff.

As a CRHT Mental Health Practitioner, you will:
Work across a 24/7 rota, inclusive of Bank Holidays, weekends, and nights and to meet the needs of people in crisis at all times.
Work to professional and regulatory body codes, standards and guidance at all times ensuring clinical practice is grounded in evidence based theoretical and practical knowledge.
Work at a high level of skill in completing comprehensive assessments of needs identifying symptoms relating to both mental health and physical health of people referred to the service.
Be a Gatekeeper service for hospital admissions 7 days per week and join Mental Health Act Assessment.
Accept referrals and provide further assessments for service users on the caseload and are proactive in making autonomous clinical decisions, with high risk user population.
Provide In-Reach into the Adult in patient wards, which will support Bed Management and facilitate Early Discharge.
Contribute to effective multi-professional working with the Crisis Resolution Home Treatment Team and the service generally. Specifically, this requires flexibility in working practices and a willingness to acquire familiarity with all areas of the Team's work. This will involve advising and taking advice from colleagues in order to provide a well-integrated and rapid service.
Advise on issues relating to medication, to administer or supervise the taking of medication as appropriate and to offer advice to other members of the team on medication and other health issues.

Main duties of the job
- Conduct initial screening and assessment of all service users, providing intensive short-term treatment as needed.
