Job summary
The post holder will be based within the Learning Disability and Forensic directorate. Working closely with the Learning disability Service Manager to review the current business activities within the service. The post holder will also be the lead in promoting the service, identifying good practice, and working closely with the team to implement any recommendations. The post holder will have direct line management responsibilities for the agreed service and robust management of the team. The community team manager will assist and oversee the day-to-day operational responsibilities for the clinical services within the community team and the wider multi-disciplinary team.
Main duties of the job
To contribute to the development of the community health team services.
To lead and embed the principles of transforming care across the learning disability service.
To provide support to the service manager to meet the aims and objectives of the service including deputising and delegated responsibilities
To provide support to the service manager in the effective day to day management of the team within the resources available and agreed governance frameworks.
To review staffing levels and skill mix as necessary.
To be responsible for the leadership, mentorship, and support of staff daily.
To manage and provide management / clinical supervision to staff under your line management.
Work within the infection control agenda to improve standards.
Improving environments for patients.
To work with patients and their carer/relative in addressing concerns by building closer relationships.
To promote partnership working with patients, carers, and stakeholders

Prepare all loan sets for HSDU by checking all instrumentation ispresent and correct as per supplies checksheets and bar code and listall trays.

Receipting of Purchase Orders once loan accurate costs have been obtained from the supplier.

Specialist Speech and Language Therapist (Band 7)12-Month secondment / Fixed term post
An exciting opportunity has arisen for an experienced Speech and Language Therapist to join the specialist multidisciplinary team at Parkview Clinic, a regional inpatient mental health service for children and young people. The service provides specialist care for young people across both general adolescent and eating disorder inpatient wards.
In this role, you will deliver specialist assessment, formulation and intervention for young people with speech, language and communication needs, including autistic young people and those presenting with complex mental health needs. Working autonomously and as an integral member of the multidisciplinary team, you will contribute to the delivery of high-quality, evidence-based care, support service development initiatives, and provide clinical leadership through supervision, training and consultation to colleagues and partner agencies.

Job summary
We are currently seeking an experienced Social Worker/CPN, at Band 6 to work via a Bank Contract, based in Watford for our Community Adult Mental Health Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
As a Social Worker, you will work as a member of the multi-disciplinary team, providing high quality social care services to adults of all ages experiencing mental ill-health. The core functions of the role will be to provide assessment, care planning and coordination of care from a social perspective to adults accessing the service. You will delivery recovery-focused care and support to service users and their carers that is focused on the principles of choice, control, and independence, as well as ensuring safety. Main duties of the job

Main duties of the job
As a Healthcare Assistant within our Short-Term Care Beds service, you will provide high-quality care and support to patients requiring short-term inpatient rehabilitation. Working alongside nurses, therapists, and other healthcare professionals, you will support patients with personal care, mobility, nutrition, rehabilitation activities, and daily living tasks, helping them regain independence and achieve their recovery goals.
You will assist with implementing care plans and rehabilitation programmes, undertake and record delegated clinical observations, maintain accurate documentation, and promptly report any changes in a patient's condition. You will also contribute to creating a safe, clean, and welcoming environment, whilst supporting safe and timely discharge planning.
A key aspect of the role is motivating and encouraging patients to maximise their independence and actively participate in their recovery. You will work closely with patients, families, and carers to provide compassionate, person-centred care.
This role is part of a 24-hour, 7-day service. The post holder will be required to work a mixture of long day shifts (07:45 to 20:15) and night shifts (19:45 to 08:15), including weekends and bank holidays, on a rotational basis. Flexibility to work both day and night shifts is essential.

Job summary
We are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care.As part of the wider left shift ambition, this service will support earlier discharge from hospital and improve outcomes through timely, multidisciplinary rehabilitation in the community.Our Year 1 focus is on building a more responsive and integrated local service, increased MDT capacity and improved continuity of care across the stroke pathway.You will be joining a service at a key point of development, with the opportunity to shape how care is delivered and contribute to a more integrated, forward-thinking model of community stroke rehabilitation.
A seven day service will be developed in the future.
Main duties of the job
Working alongside the other MDT members including nurses, speech and language therapists and occupational therapists and assistant practitioners your duties will include:
- To promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using health coaching techniques and meaningful activity.
- To be responsible for delivering Physiotherapy in the assessment and treatment of stroke patients, some of which may have complex and/or chronic presentation.
- To collaborate with other services to support the stroke pathway.
- To assist with teaching and supervision of other team members and students.
- To assist evaluation of the Integrated Community Stroke Service and to be actively involved with service development within the specialty and to implement changes in stroke services delivery as required.
- To work as part of an integrated Stroke MDT, following evidence based practice. This will include a combination of face-to-face and telerehab sessions, devising therapy programmes carried out by yourself or an Assistant Practitioner, writing self management plans for patients and signposting to community-based communication and support groups (such as those provided by voluntary organisations).

We are recruiting for a Emergency Department Stores & Equipment Administrator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
You will support the Emergency Department Inventory & Supplies team with the distribution of stock and replenishment of medical consumables across the Emergency Department clinical floor.
Your role will involve working within a team to provide a high quality service which offers a timely responsive and pro-active service for the movement of stock, materials and equipment within the Emergency Department and Emergency Floor.

Main duties of the job
To assist clinical staff to ensure the environment is safe.
To daily check and ensure that any broken equipment is taken to EBME and replacement equipment is loan to the department by EBME where appropriate and have and keep a list of equipment not in the department.
Collection of medical devices from other areas of the trust.
For the day to day safety and security of the ED equipment and store facilities.
To order, receive and process internal requisitions via the Trusts online computerised system for all ED stores items (excluding drugs) in a cost effective and timely manner and in line with Trust procedures.
Responsible for monthly stock take and reconciliation with the basic inventory package. Investigating of non-balancing stock

Main duties of the job
The Hospice at Home service is a 24 hour/7 days a week service, seeing patients across our whole geographical area of East Dorset, predominantly visiting patients and families in their own homes to provide end of life care. The patients will have complex needs and referred by teams within Specialist Palliative Care Service.
We are seeking applicants who have experience of providing palliative and end of life care, who have confidence to work independently and are passionate about "What matters" for patient's.
The service was started over 3 years ago and has developed significantly during this period. As a Staff Nurse you will complete initial and ongoing assessment of care required. Provide hands on physical end of life care including single administration of medications, Verification of death and last offices. You will need to be flexible with work pattern to include nights.
We are looking for enthusiastic, compassionate, motivational nurses to be involved in providing excellent end of life care. A background in Palliative Care, in providing end of life care or working in the community would be advantageous.

Job summary
The Crane Valley District Nursing team have a vacancy for a band 3 Community Healthcare Assistant.
We are seeking a highly motivated, enthusiastic individual with good communication skills and a passion for caring for patients in their own homes. Experience of clinical work in the community is desirable but training will be offered to the successful applicant. There is an expectation that the successful candidate will undertake our in house development program for extended skills which will involve insulin administration, injections and catheterisation.
Under the supervision and support from the team you will be required to visit patients in their own homes providing clinical care. Working as a HCA within the District Nursing Team, the role involves visit patients in their own homes providing clinical care, under supervision.
Access to a car with a valid driving license is essential for this role.
Main duties of the job
To undertake care, following the instructions of the team leader ornominated deputy according to the needs of the patients as identified onthe care plan, providing evidence based, high quality, holistic nursingcare for patients in their own homes
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
